This chapter reviews research on the psychological impact and treatment of pregnancy loss for women, men, and families. The psychological sequelae of pregnancy loss can include mild to severe grief, complicated grief, depressive disorders, post-traumatic stress disorder (PTSD), and other anxiety disorders. Effects on couples, men, and other family members, including the impact on subsequent pregnancies, parental attachment to subsequent children, and gender differences in how men and women cope and grieve, are discussed. The authors present a conceptual framework for understanding pregnancy loss, positing that reactions largely depend on deeply personal and often unconscious meanings of pregnancy and on how parenthood fits into personal identity and the achievement of adult developmental tasks. The authors discuss unique aspects of pregnancy loss that may explain its broad impact and describe ways in which psychotherapy with pregnancy loss patients differs from psychotherapy with the general population, especially regarding countertransference and attitudes toward self-disclosure.

A plan is being developed for the conversion of the NIST research reactor (NBSR) from high-enriched uranium (HEU) fuel to low-enriched uranium (LEU) fuel. Previously, the design of the LEU fuel had been determined in order to provide the users of the NBSR with the same cycle length as exists for the current HEU fueled reactor. The fuel composition at different points within an equilibrium fuel cycle had also been determined. In the present study, neutronics parameters have been calculated for these times in the fuel cycle for both the existing HEU and the proposed LEU equilibrium cores. The results showed differences between the HEU and LEU cores that would not lead to any significant changes in the safety analysis for the converted core. In general the changes were reasonable except that the figure-of-merit for neutrons that can be used by experimentalists shows there will be a 10% reduction in performance. The calculations included kinetics parameters, reactivity coefficients, reactivity worths of control elements and abnormal configurations, and power distributions.

heavy water moderated and cooled reactor operating at 20 MW. It is fueled with high-enriched uranium (HEU) fuel elements. A Global Threat Reduction Initiative (GTRI) program is underway to convert the reactor to low-enriched uranium (LEU) fuel. This program includes the qualification of the proposed fuel, uranium and molybdenum alloy foil clad in an aluminum alloy, and the development of the fabrication techniques. This report is a preliminary version of the Safety Analysis Report (SAR) that would be submitted to the U.S. Nuclear Regulatory Commission (NRC) for approval prior to conversion. The report follows the recommended format and content from the NRC codified in NUREG-1537, "Guidelines for Preparing and Reviewing Applications for the Licensing of Non-power Reactors," Chapter 18, "Highly Enriched to Low-Enriched Uranium Conversions." The emphasis in any conversion SAR is to explain the differences between the LEU and HEU cores and to show the acceptability of the new design; there is no need to repeat information regarding the current reactor that will not change upon conversion. Hence, as seen in the report, the bulk of the SAR is devoted to Chapter 4, Reactor Description, and Chapter 13, Safety Analysis.

Parenting after pregnancy loss is often complicated for people who have not effectively grieved the loss and worked through the trauma. Reproductive losses can trigger shame and self-doubt, damage the sense of self-as-parent, and inflict narcissistic injuries, which, in turn, may impede the resolution of grief. If not addressed, these unresolved feelings may be projected onto subsequent children, potentially disrupting attachment relationships and the child's sense of self. The reproductive story, a lifelong internal narrative that comprises the thoughts, feelings, and hopes about how parenting and adulthood will unfold, and forms the core of parental identity, can be used as a tool in helping parents understand the depth of their feelings, integrate current and past losses into the self, and resolve grief. The authors discuss 3 crucial aspects of intervention with parents who have had pregnancy losses, aimed both at healing the parents themselves and at protecting attachment relationships with subsequent children: (a) initially staying present-focused and engaging with the painful details of the loss experience, to normalize and validate the patient's grief and trauma, counteract shame, and begin repairing narcissistic injuries; (b) eliciting the reproductive story to identify and integrate past losses, and in its revision, to allow for hope and repair; and (c) attending to both acknowledged and denied grief. Detailed reactor physics and safety analyses have been performed for the 20 MW D 2 O moderated research reactor (NBSR) at the National Institute of Standards and Technology (NIST). The analyses provide an update to the Final Safety Analysis Report (FSAR) and employ state-of-the-art calculational methods. Three-dimensional MCNP Monte Carlo neutron and photon transport calculations were performed to determine the safety parameters for the NBSR. The core depletion and determination of the fuel compositions were performed with MONTEBURNS. MCNP calculations were performed to determine the beginning, middle, and end-of-cycle power distributions, moderator temperature coefficient, and shim arm, beam tube and void reactivity worths. The calculational model included a plate-by-plate description of each fuel assembly, axial mid-plane water gap, beam tubes and the tubular geometry of the shim arms.The time-dependent analysis of the primary loop was determined with a RELAP5 transient analysis model including the pump, heat exchanger, fuel element geometry, and flow channels for both the six inner and twenty-four outer fuel elements. The statistical analysis used to assure protection from critical heat flux (CHF) was performed using a Monte Carlo simulation of the uncertainties contributing to the CHF calculation. The power distributions used to determine the local fuel conditions and margin to CHF were determined with MCNP.Evaluations were performed for the following accidents: (1) the control rod withdrawal startup accident, (2) the maximum reactivity insertion accident, (3) loss-of-flow resulting from loss of electrical power, (4) loss-of-flow resulting from a primary pump seizure, (5) loss-of-flow resulting from inadvertent throttling of a flow control valve, (6) loss-of-flow resulting from failure of both shutdown cooling pumps and (7) misloading of a fuel element.

The analyses employ stateof-the-art calculational methods. Three-dimensional Monte Carlo neutron transport calculations were performed with the MCNPX code to determine homogenized fuel compositions in the lower and upper halves of each fuel element and to determine the resulting neutronic properties of the core. The accident analysis employed a model of the primary loop with the RELAP5 code. The model includes the primary pumps, shutdown pumps outlet valves, heat exchanger, fuel elements, and flow channels for both the six inner and twenty-four outer fuel elements. Evaluations were performed for the following accidents: (1) control rod withdrawal startup accident, (2) maximum reactivity insertion accident, (3) loss-of-flow accident resulting from loss of electrical power with an assumption of failure of shutdown cooling pumps, (4) loss-of-flow accident resulting from a primary pump seizure, and (5) loss-of-flow accident resulting from inadvertent throttling of a flow control valve. In addition, natural circulation cooling at low power operation was analyzed. The analysis shows that the conversion will not lead to significant changes in the safety analysis and the calculated minimum critical heat flux ratio and maximum clad temperature assure that there is adequate margin to fuel failure.
