Цель. Оценка возможности использования разработанного спейсера при лечении инфекционных осложнений после операции эндопротезирования тазобедренного сустава. Материал и методы. Для улучшения результатов лечения парапротезной инфекции разработан спейсер тазобедренного сустава. Он состоит из головки, шейки и ножки, изготовленных из костного цемента. Внутри него имеется каркас, соединенный с помповым микронасосом. Каркас выполнен в виде четырех металлических трубок, внутри которых находятся растворы лекарственных средств. Поршни насосов изготовлены из ферромагнита для возможности взаимодействия с магнитом, подносимым к поверхности тела. В спейсере имеется возможность выделения из него лекарственных средств в послеоперационном периоде посредством воздействия на него внешнего магнитного поля. На базе кафедры травматологии и ортопедии Воронежского государственного медицинского университета им. Н.Н. Бурденко был создан действующий макет спейсера. Для оценки функциональных возможностей спейсера были проведены экспериментальные исследования на кроликах. В брюшную полость животным под внутривенным наркозом устанавливали имплантат, содержащий раствор Пирогенала. Проводили оценку средней температуры в исследуемой группе до начала эксперимента и после введения Пирогенала (под воздействием магнитного поля). Результаты. Температура после начала работы спейсера достоверно отличалась от температуры до начала эксперимента (t-критерий – 17,17, p<0,05), что можно объяснить попаданием в кровь животных Пирогенала, содержащегося в спейсере. Проведенные исследования продемонстрировали возможность выделения лекарственных средств, находящихся внутри спейсера, во внутреннюю среду под воздействием на спейсер магнитного поля. Заключение. Предложенный имплантат позволяет увеличить объем и количество лекарственных средств, выделяемых из него в рану. При этом у хирургов есть возможность после операции неинвазивно контролировать скорость введения препаратов в парапротезное пространство для обеспечения их необходимой концентрации. Научная новизна статьи впервые разработан спейсер тазобедренного сустава, позволяющий под воздействием внешнего магнитного поля выделять в парапротезное пространство лекарственные средства, находящиеся внутри самого спейсера. По сравнению с другими способами введения спейсер увеличивает количество лекарственных средств, поступающих непосредственно в околопротезные ткани, что, соответственно, снижает их возможные побочные эффекты.
The objective is to present 2 clinical cases of complete vision loss after bifrontal access during craniotomy due to vascular brain pathology.Materials and methods. In one case, a bifrontal skin incision was used during subfrontal craniotomy to remove frontal lobe arteriovenous malformation, in the other one – to apply intracranial side-to-side anastomosis between A3 segments of both anterior cerebral arteries, and to clip proximal aneurysms of the anterior connecting artery and the anterior cerebral artery on the right. After the surgery, patients developed bilateral amaurosis and ophthalmoplegia. We analyzed data on patients’ general clinical condition and their ophthalmological status prior to the surgery, as well as surgical videos and anesthesia records.Results. Intraoperative iatrogenic damage of cranial nerves and arteries was excluded. We distinguished that a suspected intraoperative factor, playing a decisive role, was a piece of skin applying direct pressure on the eyeballs. It was removed during the surgery.Conclusion. To prevent the development of such a complication as vision loss, it is necessary to carefully evaluate all possible risks at preoperative and intraoperative stages. During the surgery, all organs and systems at risk should be monitored. It is important to prevent a sudden drop in blood pressure and massive blood loss. During craniotomy, any eyeballs compression must be avoided.Conflict of interest. The authors declare no conflict of interest.Informed consent. Osteoarthritis, both idiopathic and post-traumatic, is currently the most significant problem in orthopedics. It is particularly difficult to treat patient with comorbidities, when it is necessary to decide on surgery for a late-stage joint disease. An operation is associated with a great risk in such patients. Even if the patient does not have serious somatic disorders, the main task of doctors (and this is a multidisciplinary problem) is to prolong joint functioning and maintain the patient’s quality of life.In this article, we report difficult cases, when a complex of non-pharmacological (therapeutic exercise, physiotherapy, orthobiology – PRP therapy) and pharmacological (nonsteroidal anti-inflammatory drugs, chondroprotectors, vitamins, calcium, tissue repair stimulants) treatments ensured a good effect, thereby maintaining the joint function and adequate quality of life without surgery. We also found that the use of Ambene® Bio (a chondroprotector) increased treatment efficacy.
