Background: Nowadays the number of adult patients with complete and partially edentulous, complicated by the defects of the jaws, which need rehabilitation with the use of dental implants is still very high. More stable, aesthetically satisfactory result, and early restoration of function can be achieved with a combination of a multistage surgery with rational prosthetic on dental implants. Objectives: Defects of the jaws with extensive bone deficit, caused by trauma, inflammation or operations for malignancies of maxillofacial region, complicates dental implantation. The presence of such defects in jaw bones needs a preparatory phase aimed at increasing the volume of parameters to create optimal conditions for further dental implantation. Methods: During the period 2006 to 2016 more than 450 patients with extensive defects of alveolar parts of mandible and maxilla due to different aetiological factors were treated in our clinic. Surgical planning was carried individually. Computer software, lithographic models, distraction devices and dental implants were used during treatment. Findings and Conclusion: Successful rehabilitation of adult patients with defects of alveolar parts of maxilla and mandible after different aetiological factors was conducted. As one of the step of rehabilitation of this patients was used the method of distraction osteogenesis.Different methods of distraction were used to optimise the parameters of the bone in jaws. Advantages of distraction osteogenesis may provide a solution for many problems associated with the duration of the treatment, with different conditions which cannot be treated with classical preprosthetic surgery.

Сеченова Минздрава России, Москва, Россия Диагностика и лечение первичных артериовенозных мальформаций нижней и верхней челюсти изучены мало, что обусловлено редкой встречаемостью патологии. Следует различать первичные и вторичные поражения челюстных костей сосудистыми мальформациями. Чтобы установить окончательный диагноз, определить сегмент поражения, источник кровотечения, характер местных гемодинамических нарушений, особенности строения аномальных сосудов, скорость и интенсивность кровотока в них, нужно провести ряд специальных и междисциплинарных диагностических исследований. Материал и методы. Представлен опыт лечения 12 пациентов с первичными артериовенозными мальформациями нижней и верхней челюсти с 2007 по 2017 г. Алгоритм обследования включает рентгенографию, компьютерную томографию, мультиспиральную компьютерную томографическую ангиографию, дигитальную субтракционную ангиографию. При предоперационной подготовке проводилась эмболизация афферентных сосудов и сосудистых полостей с использованием Onyx 18 и Гистоакрила. В зависимости от локализации и размера артериовенозных мальформаций определялась тактика хирургического лечения, объем резекции костной ткани. Для реабилитации пациентов, восстановления формы и размера челюстей, функции жевания осуществлялись костная пластика, дистракционный остеогенез, дентальная имплантация, несъемное и съемное протезирование. Результаты. Применение современных эмболизирующих материалов дает выраженный гемостатический эффект, уменьшает кровотечение во время операции. В результате лечения прекращаются кровотечения и рост артериовенозных мальформаций. Большинству пациентов с послеоперационными дефектами нижней челюсти и зубных рядов были проведены неоднократные реконструктивно-восстановительные операции. Выводы. Алгоритм обследования пациентов с первичными артериовенозными мальформациями челюстей должен включать ортопантомографию, компьютерную томографию, мультиспиральную компьютерную томографическую ангиографию и дигитальную субтракционную ангиографию. Хирургическое лечение заключается в резекции пораженного фрагмента челюсти. Целесообразно проводить органосохраняющие операции с возможной реплантацией резецированного фрагмента челюсти. Сочетание хирургического лечения и предоперационной эмболизации снижает риск возникновения массивного кровотечения во время операции.
