Introduction. Measuring the quality of health care in a country is one of the essential features of health quality improvement systems.The Aim of the Study. Studying the experience of countries with strong economies in monitoring and evaluating the quality of health care, comparing with domestic experience in this area. Materials and methods. The results of a review of English-language sites with data on measuring clinical indicators in the UK, Canada and the USA and websites of the national healthcare system (Ministry of Health of Ukraine, Center for Medical Statistics of the Ministry of Health of Ukraine, State Expert Center of the Ministry of Health of Ukraine, National Health Service of Ukraine) are given. Results and discussion. It was found that in all countries with the data analyzed in our work, the tasks of continuous improvement of the quality of health care are solved due to the connection with clinical recommendations, state support for the processes of measuring the health care quality, the provision of software for calculations and the transparency of the definition of indicators. Since 2018, parallel processes have been going on in the domestic healthcare system: а) clinical quality indicators are approved as part of industry standards in healthcare; b) indicators demonstrating the volumes of medical care are collected and processed by the Center for Medical Statistics of the Ministry of Health of Ukraine, c) The National Health Service of Ukraine generates special indicators based on the data of providers and users of medical services, processes, analyzes and publishes anonymized open data in the Unified State Open Data Web Portal in the form of special dashboards; these indicators are partly relevant to clinical indicators of quality. Сonclusions: Based on a comparison of the indicators used by different countries, the connection of quality indicators with evidence-based data, further steps of a systematic approach in the field of improving the quality of medical care are determined, since the main tool for monitoring and evaluating medical care are special indicators - quality indicators. These indicators are calculated retrospectively according to standardized methods defined at the national level, which contributes to the transparency and accountability of health care. At the same time, the connection with evidence and clinical guidelines, software, templates and calculation formulas, scope and limitations, etc. are clearly defined and easily accessible, the number and types of indicators vary from country to country. In healthcare institutions, indicators are calculated using information technology based on patient data, focusing on aspects of the quality of care such as effectiveness and productivity. A systematic improvement in the quality of health care in practice can be achieved due to the relevance of the indicators calculated in the electronic health system with the evidence-based provisions of national clinical recommendations and the requirements of industry standards in the field of healthcare.

Abstract. In the conditions of the rapid development of modern medicine, the search and implementation of new approaches to the study of the diagnostic and treatment process of diseases with the mastery of manual methods are of great importance. The inability to apply acquired knowledge in practice is often the basis of medical errors and ineffective medical measures. There are new requirements for the training program for medical interns using modern medical technologies. Therefore, ensuring the availability of high-quality training for interns on a clinical internship basis should be considered a mandatory component of maintaining the high-quality level of training of medical interns at the current stage of medical reform in Ukraine. This is evidenced by our survey of intern doctors, who emphasized the importance of the availability and staffing of workplaces with awareness of safety techniques, the availability of sufficient volumes of medical care and the availability of standards of medical care based on internships, training with a sufficient level of comfort and a level of timely awareness.
Мета дослідження: вивчення точки зору лікарів загальної практики-сімейних лікарів щодо проблеми надмірної маси тіла (НМТ) та ожиріння у дітей. Матеріали та методи. Вивчена обізнаність лікарів за гальної практики-сімейних лікарів (анкетування 145 рес пондентів) щодо проблеми НМТ та ожиріння у дітей, їхня думка щодо профілактики/корекції НМТ. Результати. Більшість лікарів вважають, що НМТ є проб лемою сьогодення, з якою вони найчастіше асоціюють роз виток цукрового діабету, хвороб системи кровообігу, ендо кринних захворювань, захворювань хребта та суглобів. Проте тільки 29,3±4,7% лікарів знають критерії визначення НМТ і ожиріння у дітей. Більшість лікарів визначають індекс маси тіла (ІМТ) тільки за умови візуалізації у дити ни НМТ (66,3±4,9%), а 3,3±1,9%-ніколи не визначають ІМТ. Свою роботу за напрямком подальшого спостережен ня за станом здоров'я дітей з НМТ бачать тільки 6,5±2,6% лікарів. Більшість вважає, що такими пацієнтами мають займатися лікарі ендокринологи (68,5±4,8%), дієтологи (16,3±3,9%), гастроентерологи (10,9±3,2%) та лікарі педіатри (9,8±3,1%). Заключення. Існуюча система профілактики аліментарно залежних захворювань є недосконалою, що зумовлено відсутністю стандартизованих програм раннього виявлен ня надмірної маси тіла (НМТ) у дітей та пов'язаних з нею порушень у стані здоров'я, організації обліку таких дітей та медичного спостереження за ними. Особливої актуаль ності набуває запровадження на рівні первинної ланки на дання медичної допомоги заходів з раннього виявлення дітей з НМТ, їхнього обліку та моніторингу за станом здо ров'я з урахуванням ризиків розвитку захворювань. Introduction. The problem of secondary hyperparathyroidism (SHPT) and tertiary hyperparathyroidism (THPT) has drawn the attention of numerous researchers in many countries, including those where kidney transplantation is a routine procedure. The most acute aspects include determining the most effective methods of therapeutic treatment, diagnostic and prognostic criteria, indications for surgery, its scope, and methods of avoiding relapses. The aim. To study the quality of life of patients who underwent surgery for secondary and tertiary hyperparathyroidism on the background of chronic kidney disease. Materials and Methods. During the study, we conducted a survey of 60 patients with secondary and tertiary hyperparathyroidism who underwent surgery between 2020 and 2023. The surveys were conducted twice: before the surgical intervention and within a period of up to 9 months after it. Among those surveyed, there were 35 (58%) women and 25 (42%) men. The average age of the respondents was 51.31±9.16 years. Results and Discussion. Quality of life is one of the most important indicators of the effectiveness of treatment. We observed an improvement in the quality of life in all surveyed patients with Tertiary Hyperparathyroidism (THPT) after surgical treatment. From the physical component point of view, this effect is undoubtedly associated with a decrease in the level of PTH and ionized calcium in the blood. This is evidenced by the significant correlation between these indicators and the obtained values of physical functioning scales before and after the operation. Conclusions. A significant correlation was observed between the levels of parathyroid hormone, ionized calcium, and physical functioning scales before and after surgery.
