<p>La leishmaniosis visceral es una enfermedad multisistémica que en los caninos ostenta signos clínicos tales como esplenomegalia, hepatomegalia, linfadenopatía y glomerulonefritis. Es frecuente la presentación de anemia asociada a hiperproteinemia. La ecografía puede revelar anomalías morfológicas y estructurales de los órganos de la cavidad abdominal. El objetivo del trabajo fue evaluar mediante eco-grafía las lesiones visibles de los órganos abdominales, así como establecer relaciones con los análisis de laboratorio. La detección del parásito se efectuó mediante diagnósticos parasitológico y serológico indirecto. Los órganos abdominales afectados se evaluaron por ecografía (forma, tamaño, estructura y ecogenicidad). En los riñones se exploró la relación corteza/médula. En el laboratorio se investigaron las variables sanguíneas, serológicas y renales. La estadificación clínica de los caninos resultó del 50% en estadio moderado, 40% grave y 10% muy grave. Las principales alteraciones fueron hepatomegalia<br />(8 pacientes), esplenomegalia (1 caso) e incremento de la ecogenicidad de la corteza<br />renal con disminución de la definición corticomedular y linfoadenomegalia (5 animales). El<br />90% de los caninos reveló anemia acompañada de hiperproteinemia. Se concluye que el uso de<br />la ecografía en conjunto con los análisis sanguíneos, pueden ayudar a establecer la severidad<br />de las lesiones de los órganos abdominales en pacientes con leishmaniosis.</p>

FULVIA PEDANI, MD,T DAVIDE CAMOLETTO, MD," AND LUCIAN0 NEGRI, M D t t Eighty-four previously untreated patients (69 males, 15 females) with squamous carcinoma of the tongue (30 patients), floor of the mouth (30), cheek (16), and retromolar region (8) were treated using a protocol comprising cryosurgery + chemotherapy, followed by external 6oCo radiotherapy. The follow-up period was at least 6 months (median, 50 months). Cryosurgery (1-2 sessions in 49 TI-2 cases; 2-4 in 35 T3-4 cases) was accompanied by a C M F (cyclophosphamide, methotrexate, 5-fluorouracil) schedule (TI-2, two courses; T3-4, three courses). Radiotherapy was given 15 to 20 days after combined cryochemotherapy (TI, 50 Gy on tumor and lymph nodes; T2-3-4, same with an extra dose of 10 to 15 Cy on the primary lesion). Complete remission was reached 4 months after treatment in 76 of 84 patients (90.5%). Survival with no evidence of disease (NED) in the 57 patients (27 TI-2, 30 T3-4) with a follow-up of more than 3 years was 59.6% for the series as a whole, 703% for TI-2, and 50.0% for T34, 78.2% for the tongue, 52.6% for the floor, &.6% for the cheek, and 0% for the retromolar region. The picture was much the same after 5 years. Actuarial survival at 6 years was 66% in the series as a whole, 75.5% in TI-2, and 57.5% in T3-4 (tongue 86.9%, floor 56.1%, cheek 68.496, and retromolar region 0%). It is believed that the results obtained in tumors of the tongue, floor and cheek, coupled with the conservative aspects of the protocol, make it a suitable subject for a controlled trial. Cancer W424-431. 1985. ARIOUS MODALITIES have been tried in recent years V in an attempt to improve the treatment of squa-mous cancer of the oral cavity. New surgical methods,' combinations of radiotherapy and surgery,* and adjuvant chemotherapy and irnmun~therapy,~.~ have been experimented in early (Stage 1-11) cases. These approaches do not appear to give better results than radiotherapy or surgery a l ~ n e. ~. ~ In advanced stages (Stages 111-IV), there is a high incidence o f locoregional failures (25%-50%) even when radiotherapy and surgery are ~ o m b i n e d , ~ and distant metastases are encountered in 10% to 20% of cases.6 Several protocols have therefore been tried: intravenous (IV) chemotherapy + radiotherapy;'~~ IV chemotherapy + intra-arterial (IA) chemotherapy + surgery or radiotherapy ; ' i m m u noradiot herapy ; ' chemoim-munotherapy + surgery or radiotherapy.'' Se describe la resolución clínica de una necrosis lingual asociada a insuficiencia renal aguda por leptospirosis. Un canino Boston terrier, macho entero de 4 años, a la exploración de la cavidad oral presentó alteraciones de la coloración, consistencia, sensibilidad y temperatura, así como ausencia de pulso en la punta de la lengua. En el perfil renal se observaron valores elevados de urea (183 mg/dl) y creatinina (1,8 mg/dl). Las pruebas de Ehrlichia y Leishmania fueron negativas, en tanto la prueba de microaglutinación microscópica, arrojó títulos de 1/3200 de los serovares Ballum castellonis y Canicola canicola de Leptospira interrogans. Los títulos elevados de anticuerpos y las manifestaciones clínicas compatibles, confirmaron el diagnóstico de daño renal agudo. Para el tratamiento se realizó fluidoterapia con Ringer lactato y dextosa al 5%. Ante la sospecha de leptospirosis se administró penicilina con estreptomicina. Una vez estabilizada, se realizó una glosectomía parcial. Sin embargo, como la necrosis continuó avanzando, se decidió administrar 1 mg/kg de dexametasona por vía parenteral cada 12 horas, durante 3 días, y luego dosis decreciente. A los 8 días, el paciente estaba recuperado, aunque todavía poseía densidad urinaria isostenúrica. El valor de urea fue de 87 mg/dl y el de creatinina 0,8 mg/ dl, con un paciente recuperado. Si bien, los corticoides no se recomiendan ante un daño renal agudo, el uso de dexametasona estaría indicado para resolver el daño vascular.
