Исследования генов-кандидатов и полногеномные ассоциативные исследования выявили несколько локусов предрасположенности к идиопатическому сколиозу, например, рядом с генами LBX1, GPR126/ADGRG6, PAX1 и BNC2. В близнецовых исследованиях для расчета наследуемости использовалась степень конкордантности идиопатического сколиоза между монозиготными и дизиготными близнецами. Исследования проводились в странах Восточной Азии, Европы и Северной Америки. Семь исследований были близнецовыми, а два -семейными. Результаты исследования показывают, что парная конкордантность для монозиготных близнецов (от 0,11 до 1,0) была примерно в два раза выше, чем для дизиготных близнецов (от 0 до 1,0) в большинстве исследований. Наследуемость в широком смысле варьировалась от 0,13 до 1,0, тогда как единственная доступная оценка наследуемости в узком смысле в исследовании близнецов составляла 0,38. Диагноз сколиоза был подтвержден у пробандов с помощью рентгенограмм в обоих исследованиях. Оценки наследуемости в узком смысле составили 0,49 и 0,88 соответственно.
