We conducted an anonymous survey of 140 medical interns at the end of the 2nd year of study in specialties leading in emotional burnout. The results obtained indicate the beginning of the syndrome formation even at the undergraduate level of education, with significant deterioration during the course of primary specialization. Prevention of burnout syndrome should begin with its diagnosis, as well as assessment of the presence of depression, situational and personal anxiety at the early stages of medical education.
We analyzed over 3,000 anonymous surveys of interns who studied the Emergency medicine course this year and 10 years ago. We were studying their psychological features and social behavior in ten-year dynamics. The main characteristics of representatives of generation Y in medicine were determined. Representatives of this generation need an individual personal approach, which would allow transforming the downsides of mosaic thinking, features of behavior and attitude to the educational process, modern passion for gadgets and manner of communication into positive qualities.
Согласно данным Всемирной организации здравоохранения, ежегодно вследствие травм погибают 5,8 млн человек. Летальность при политравме составляет 18,8-36,0 %, из них 50-60 % погибают на догоспитальном этапе [1-3]. Проникающие травмы встречаются в 4-38 % случаев политравмы мирного времени. Ведущими причинами смерти при политравме являются тяжелая черепно-мозговая травма (ЧМТ) и геморрагический шок [2]. В 71 % случаев сочетанные травмы поражают лиц моложе 44 лет, в 80 % -мужчин. Наиболее частыми причинами политравмы мирного времени (66,2 %) являются дорожнотранспортные происшествия (ДТП) и кататравмы [4]. Актуальность. Боевые сочетанные травмы протекают тяжелее, чем политравма мирного времени, из-за разницы в поражающих факторах, условиях получения травмы, сроках оказания помощи пострадавшим, массовости поражений. Цель исследования: провести сравнительный анализ тяжести состояния пострадавших с мирной и боевой политравмой при поступлении в стационар III уровня. Материалы и методы. Обследовано 79 пострадавших с политравмой мирного времени и 65 раненых из зоны АТО. Изучались изменения сознания по шкале комы Глазго, показатели гемодинамики, потребность в вазопрессорной поддержке и искусственной вентиляции легких, перистальтика кишечника, показатели общего анализа крови, печеночный и почечный комплекс, коагулограмма, показатели кислотно-основного и газового состава крови, уровень цитокинов (IL-6, IL-10) в сыворотке крови. Результаты. При мирной политравме достоверно чаще встречались черепно-мозговые травмы и торакоабдоминальные травмы, при боевой -травмы мягких тканей. Среди причин мирной политравмы преобладали дорожно-транспортные происшествия и высотные травмы, боевой -минно-взрывная травма. Раненые на 12,2 % чаще требовали ургентных оперативных вмешательств. Нарушения сознания в группе боевой травмы чаще были связаны с седацией, а мирной -непосредственно с черепно-мозговой травмой. Гипотензия и тахикардия при боевой травме были менее выражены за счет проведенных ранее противошоковых мероприятий и вазопрессорной поддержки. При боевой травме были более выражены гипоальбуминемия, гипокоагуляция и почечная дисфункция. В проявлениях цитолитического синдрома при боевой травме преобладало повышение уровня аспартатаминотрансферазы (АсТ), что, вероятнее всего, также объяснялось разрушением больших массивов тканей. Воспалительная реакция от характера травмы не зависела и ассоциировалась только с тяжестью и локализацией повреждений. Частота осложнений составила 41,8 % при мирной травме и 47,7 % -при боевой, летальность -8,9 и 13,8 % соответственно. Выводы. Background. Perfectionism today is understood as an individual's psychological conviction that the ideal can and should be achieved, and the imperfect result of work (physical, intellectual, etc.), in their opinion, has no right to exist. The purpose of the study: Our goal was to consider this psychological phenomenon in the community of anesthesiology interns in comparison with the indicators of practicing doctors, to investigate its negative and positive features, their prevalence and dynamics. Materials and methods. An anonymous survey of 92 medical interns and 124 participants of short-term trainings in the specialty "Anesthesiology" was conducted according to the M.M. Smith questionnaire. (2016). Results. Interns showed an average level of tendency towards perfectionism on all other scales, except for "other-oriented perfectionism" where the level of the indicator was low. Practitioners showed an average level of tendency towards perfectionism on the scales "self-worth contingencies", "concern over mistakes", and "doubts about actions", according to all other indicators, the level of tendency was low. Among medical interns, the distribution on a Big-Three Perfectionism Scale showed that the degree of their attachment to rigid, self-critical, and narcissistic types of perfectionism was at an average level, including general. Practitioners showed an average level of tendency only to the rigid type of perfectionism, as for the self-critical and narcissistic types, the level was low, as well as the general one, which except for the average tendency to rigid perfectionism can be assessed as hypoperfectionism. Conclusions. 1. Neither interns nor participants in training in the specialty "Anesthesiology" in general are supporters of hyper-perfectionism. 2. Intern doctors showed a rather high tendency to "concern over mistakes", which is explained by the fact that they are not yet specialists, but only learning. This can be assessed as constructive perfectionism. 3. Practitioners are prone to "self-oriented perfectionism", which involves high self-demand. This is explained by the specificity of the work of anesthesiologists (surgeons work in a team, an anesthesiologist - alone), who must first of all rely on themselves, and only then on the help of others. 4. Hypoperfectionism of the self-critical and narcissistic types in anesthesiologists with a certain length of work is associated with psychoprotective deviation of professional behavior, which excludes setting extremely high demands for the results of one's work, since the self-critical type implies a high probability of psycho-emotional burnout syndrome, and the narcissistic type sometimes leads to medical errors.
Любая травма как причина критического состо-яния вызывает стрессовое напряжение или, наобо-рот, торможение нервной системы, что проявляется различным по выраженности снижением эффек-тивности ее работы и прежде всего когнитивных функций.Развитие методов интенсивной терапии постра-давших с критическими состояниями, вызванны-ми травмой, как связанной непосредственно с воз-действием поражающих факторов того или иного оружия (боевой), так и не связанной с ними (так называемая травма мирного времени), привело к значительному снижению летальности.
