RESUMEN Introducción: la epicondilitis lateral crónica es una de las patologías más frecuentes en la consulta de traumatología de miembro superior. El tratamiento quirúrgico está indicado en pacientes con fracaso del tratamiento conservador, que suponen el 1-16%, según las series. El porcentaje de reintervenciones es aún menor, por lo que la literatura acerca de la cirugía de rescate es escasa. Caso clínico: se presentan 2 casos clínicos de pacientes trabajadores manuales de fuerza reintervenidos por epicondilitis lateral rebelde al tratamiento. En ambos casos se había empleado la técnica de Nirschl como cirugía primaria y se realizó una plastia de rotación del ancóneo como tratamiento de rescate. Resultados: los 2 pacientes refirieron mejoría del dolor según la escala visual analógica (EVA), lograron un arco de movilidad funcional del codo, aunque con disminución de la fuerza para el desarrollo de su actividad laboral previa. Discusión: la plastia de rotación del ancóneo se considera una opción válida para el tratamiento de pacientes con fracaso del tratamiento quirúrgico de epicondilitis lateral cuando refieren persistencia del dolor al nivel de la inserción humeral del grupo extensor tanto a la palpación como con la actividad.
