RESUMO: Foram analisados 545 pacientes submetidos a 560 plásticas da valva mitral no período de março de 1980 a dezembro de 1997. A idade variou de 3 meses a 86 anos com média de idade de 42,2 ± 21,4 anos. Duzentos e setenta e quatro (50,3%) pacientes eram do sexo masculino. A etiologia foi reumática em 234 (42,9%) pacientes. As técnicas utilizadas foram: ressecção quadrangular da cúspide posterior em 204 (36,5%) pacientes, anuloplastia com tira posterior de pericárdio bovino em 139 (24,5%) pacientes, anel de Carpentier em 102 (18,2%) pacientes, anuloplastia posterior segmentar em 37 (6,6%) pacientes e outras técnicas utilizadas em menor freqüência. Técnicas associadas foram empregadas em 199 (35,6%) pacientes, sendo a mais freqüente o encurtamento de cordas em 72 (12,8%). Operações associadas foram realizadas em 267 (47,8%) pacientes. A mortalidade hospitalar foi de 3,7% (21 pacientes). As taxas linearizadas dos eventos no pós-operatório foram 2,9% pacientes/ano para reoperação, 0,6% pacientes/ano para tromboembolismo, 0,3% pacientes/ano para endocardite e 0,1% pacientes/ano para hemólise. A sobrevida actuarial foi de 76,8 ± 10,8 em 17 anos e sobrevida livre de endocardite, tromboembolismo, reoperação e hemólise no mesmo período foi respectivamente 98,9 ± 0,6%, 93,9 ± 3,7%, 61,0 ± 7,9% e 99,7 ± 0,2%. Podemos concluir que os pacientes submetidos à plástica da valva mitral apresentaram evolução satisfatória. INTRODUÇÃOAs técnicas de plastia valvar são cada vez mais utilizadas no manuseio cirúrgico das lesões valvares e são preferíveis à troca valvar quando tecnicamente possíveis. Alguns obstáculos à utilização das diversas técnicas existentes incluem a incerteza do cirurgião quanto ao sucesso da reconstrução, bem como em relação à durabilidade do procedimento a longo prazo (1) .O sucesso da operação conservadora depende de um profundo conhecimento da estrutura dinâmi-ca tridimensional da valva e do seu funcionamento mecânico normal através do ciclo cardíaco. Objetivo: Analisar os resultados tardios da plástica da valva mitral em pacientes reumáticos. Casuística e Métodos: Durante o período de março de 1980 a dezembro de 1997, 201 pacientes portadores de insuficiência mitral secundária a febre reumática foram submetidos à plástica da valva mitral, no Instituto do Coração do HCFMUSP. A idade média foi de 26,9 ± 15,4 anos e 59,7% dos pacientes eram do sexo feminino. Outros diagnósticos estavam presentes em 67,7% dos pacientes, sendo o mais freqüente a insuficiência tricúspide (31,3%). As técnicas de plástica da valva mitral foram: anel de Carpentier em 75 (37,3%), anuloplastia com tira posterior de pericárdio bovino em 68 (33,8%), anuloplastia posterior segmentar em 16 (7,9%), ressecção quadrangular da cúspide posterior com plicatura do anel correspondente em 11 (5,5%), ressecção de segmento da cúspide anterior em 6 (3%), anuloplastia tipo De Vega em 6 (3%), Kay em 5 (2,5%), Reed em 4 (2%) e outras em 10 pacientes. Técnicas associadas de plástica foram empregadas em 94 (46,8%) pacientes, sendo a mais freqüente o encurtamento de cordas (48 pacientes - 23,9%). Cirurgias associadas foram realizadas em 113 pacientes (56,2%). As curvas actuariais de Kaplan-Meier foram comparadas através de análise de regressão linear. Resultados: A mortalidade hospitalar foi de 4 (2,0%) pacientes, e as causas de mortalidade foram a falência de múltiplos órgãos em 2 (50%) pacientes e o baixo débito cardíaco em 2 (50%). No pós-operatório tardio, 83,9% dos pacientes se encontravam em classe funcional I (NYHA). A sobrevida actuarial foi de 93,9 ± 1,9% em 125 meses. Vinte e três pacientes foram reoperados no pós-operatório tardio, com um intervalo médio de 35,7 meses. A sobrevida livre de reoperação foi 43,3 ± 13,7% em 125 meses. Analisando-se separadamente os pacientes segundo a faixa etária, no grupo menor de 16 anos (Grupo 1) a sobrevida actuarial foi de 91,3 ± 3,8%, contra uma sobrevida de 95,6 ± 2,7% no grupo maior de 16 anos (Grupo 2), com uma diferença estatisticamente significativa (p < 0,0001). Com relação à sobrevida livre de reoperação, também houve uma diferença significativa (p < 0,0001), sendo 50,8 ± 16,9% para o Grupo 1 e 47,0 ± 14,9% para o Grupo 2. Conclusão: Os resultados tardios com a plástica da valva mitral na insuficiência mitral reumática são satisfatórios.

Resultados da implementação de modelo organizacional de um serviço de cirurgia cardiovascularResults of the establishment of an organizational model in a cardiovascular surgery service Abstract Objective: Increasing complexity of patients referred to cardiac surgery demands more effective heart centers, in order to maintain the same quality. The aim of this study is to examine the short-term effect of adoption of an organizational model on surgical outcomes.Methods: From January 2006 to June 2007, 367 consecutive adult patients underwent cardiovascular surgery. Pre-, intra-and postoperative data were prospectively collected and transferred to an institutional database. Organizational model was established in August 2006, and based on integrated multiprofessional team work patient-centered, evidence-based medicine with standardized patient care and personal conflict management. The outcomes studied were hospital mortality and combined adverse events (death, stroke, acute myocardial infarction and acute renal failure), by using multivariate logistic regression analysis. Results:After establishment of such model, there was reduction of hospital mortality (from 12% to 3.6%, relative risk= 0.3; P=0.003) and combined events (from 22% to 15%, relative risk=0.68; P=0.11). Operations performed previously to the model were independently associated with higher mortality (OR=2.5; P=0.04), adjusted to preoperative characteristics and Euroscore risk stratification system. Quality control programs aim at adequacy of hospitals units and professionals involved in order to provide the best possible medical care. With this, the influence of organizational factors in the surgical outcomes can be minimized, with only the team human error [6] and individual factors linked to the patient, such as, socioeconomic status, severity of the disease and its comorbidities [7,8].Although the literature is extremely important for the development of medical practice in various specialties, there are few publications aiming at hospital organizational aspect and its quality control. Little evidences correlates integrated measures for care to outcomes after heart surgery [9-11], which encourages broad discussion on the subject, considering the increasing transparency of the outcomes of medical procedures to the lay public [12] and the consequent tendency of payment based on parameters such as quality and performance [13].The aim of this study is to assess the short-term impact of the adoption of an organizational model of cardiovascular surgery service on the surgical outcomes.

Background: Cardiopathies are high prevalence conditions. Among them, rheumatic carditis is of high relevance in developing countries. Left cardiac chamber changes are associated to endothelial dysfunction and ET-1 levels increase. Pulmonary circulation is then affected, and not seldom leading to pulmonary hypertension (PH). However, the presence of ET-1 and its receptors in the mitral valve itself -promoting pulmonary vascular changes, with increased rheumatic valvular deformation -has not been discussed in the literature.

We compared the canine left ventricle (LV) response to sudden dilation when the LV was supported by the arterial blood of a support dog with when it was supported by an oxygenator + haemodialyser system. 3. A slow inotropic response (SIR) after dilation was seen in all six hearts supported by the donor dog, attaining 87 +/- 6% of immediate increase, whereas a mere 10% SIR occurred in only one out of seven hearts maintained by the oxygenator + haemodialyser. 4. These results indicate that SIR genesis involves one or more renewable components essential to the intracellular calcium gain elicited by stretch.

Objectives: To determine indications and results of axillary artery cannulation for cardiopulmonary bypass.Methods: From January 2005 through December 2008, axillary artery cannulation was used in 48 patients. Mean age was 62 ±11 years and 33 patients (69%) were males. Axillary artery was approached by infraclavicular incision and the cannula introduced in a 8 millimeter Dacron side graft.Results: Indications were calcified aorta (N=18, 38%), aortic dissection (N=15, 31%), ascending and/or aortic arch aneurysm (N=11, 23%) and prior to reoperative median sternotomy (N=4, 8%). Changes in intraoperative planning occurred most often in patients with calcified aorta (100% versus 10%, P<.0001) than in patients with other indications, which follow their preoperative plan. Cardiopulmonary bypass (deep hypothermic circulatory arrest in 55% and conventional in the remaining) was uneventfully conducted in all patients but one (success rate 98%) due to undiagnosed inominate artery stenosis.
