RESUMENIntroducción: la automedicación es aquella situación en la que los pacientes consiguen y utilizan los medicamentos sin ninguna intervención por parte del médico. Objetivos: determinar la prevalencia de automedicación y condiciones asociadas a la misma en pobladores del Departamento de Itapúa (Paraguay). Material y método: estudio prospectivo, descriptivo, de corte trasverso. Se aplicó una encuesta cerrada y autocompletada en 9 distritos de zonas rurales y urbanas del Departamento de Itapúa. Además de las variables sociodemográficas se indagó problemas de salud relacionados, razones para no asistir al médico y las consecuencias. Resultados: de 2321 encuestados, 57,1% eran mujeres, la edad promedio 30,85 ±12,98 años, 81,6% provenía de zonas urbanas, 41% había culminado sus estudios secundarios. El 92,8% de la muestra se ha automedicado y ha acudido a la farmacia en 50%. El 40,8% de los mismo ha aconsejado alguna vez consumir algún medicamento a alguno de sus parientes cercanos. Valorando la permeabilidad a los métodos publicitarios, 50,8% respondió que nunca ha consumido un medicamento en base a los mismos. La cefalea y los síntomas gripales fueron la causa de la automedicación. La mayoría de los pacientes que se automedican (54,2%) no ha acudido al médico por falta de tiempo. El 33,9% de la población desconoce el efecto que la automedicación pueda tener sobre la salud de la comunidad frente a un 31,8% que sostiene el mal efecto que pudiera tener la misma. A mayor nivel de instrucción mayor porcentaje de automedicación (p<0,05). Conclusión: la automedicación es una práctica muy frecuente en nuestro Departamento, coincidiendo con series de investigaciones similares en Latinoamérica. Se halló una relación significativa entre el nivel de instrucción y el porcentaje de automedicación. Es un problema de salud pública importante y deberíamos optimizar estrategias a fin de poder reducirla. Creemos que esta investigación podría reflejar de alguna forma la realidad de un país con una política salud pública que aún presenta falencias y que resulta asistencialmente insuficiente. Entendemos por humanismo médico a todo el conjunto de valores, actitudes y prácticas que promueven una auténtica vocación de servicio y dan lugar a considerar al paciente como un semejante que sufre y solicita alivio. Los aspectos más significativos que promueven el humanismo en el trato con los pacientes son el afecto, el apoyo, el respeto y la solidaridad que, a la vez, son los que nos procuran la mayor cooperación posible del paciente para conocerlo mejor y ayudarlo más (1,2) .El humanismo ha sido definido de diversas formas. El término humanista a mediados del siglo XV se utilizaba para señalar a quienes enseñaban y cultivaban la gramática, la retórica, la poesía, la historia y la filosofía moral. Humanitas significaba lo que los griegos habían expresado en términos de paideia, es decir, educación y formación del hombre (3) .La idea del humanismo renacentista en el ámbito médico se resume en lo expresado a fines del siglo XIX por José Letamendi "el que sólo sabe de medicina, ni medicina sabe", "el médico que a la vez no es filósofo, no es ni siquiera médico", ideas que a muchos hace considerar que el médico humanista es aquel que cultiva las artes, la pintura, la literatura, "que cultiva las ciencias que enriquecen el espíritu".La idea del humanismo médico se encuentra ya expresada en el juramento y en otros libros del Corpus Hipocrático. RESUMENSe presenta caso de mujer joven portadora de lupus eritematoso sistémico que consulta por síntomas gastrointestinales y fiebre. Por laboratorio y tomografía abdominal se llega a diagnóstico de pancreatitis aguda que mejora con corticoides. Keywords: acute pancreatitis, autoimmune pancreatitis, systemic lupus erythematosus CASO CLÍNICO Mujer de 20 años de edad, conocida portadora de lupus eritematoso sistémico (LES) hace 5 años en tratamiento regular con hidroxicloroquina, consulta por historia de 13 días antes con epigastralgia, dolor corporal generalizado, dispepsia y náuseas, sensación febril precedida de escalofríos y astenia. Fue internada en centro médico privado con diagnóstico de infección de vías urinarias por presentar puño percusión (+) y leucocituria. Fue tratada con antibiótico para foco urinario. Al tercer día de tratamiento la paciente continuaba febril, con náuseas y vómitos, se solicitó amilasa, que resultó normal, y lipasa, que resultó aumentada más de 2 veces su valor normal. La tomografía de abdomen mostró páncreas con porción cefálica y proceso uncinado con densidad disminuida, heterogénea y aumentada de tamaño, aumento de densidad de tejido peripancreático ( fig. 1-4).
