A case of an abnormal loop of the extracranial vertebral artery enlarging the intervertebral foramen at C5-C6 and the transverse foramen at C5 is reported. This occurrence is rare and was associated with cervicobrachial neuralgia caused by neurovascular compression of the C6 root. The patient was cured by microvascular decompression.
Lhermitte-Duclos disease (LDD), also known as dysplastic gangliocytoma of the cerebellum is a benign disease, extremely rare, characterized by loss of normal cerebellar cortical architecture and focal thickening of the folia. Lhermitte-Duclos disease as a major CNS manifestation of Cowden disease is a rare condition. The association between Lhermitte-Duclos disease and meningioma is even more rare, only few cases having been documented in the literature. We present a very rare case of a 53-year-old woman who was admitted in our department for recurrent episodes of headache and epilepsy. Magnetic resonance imaging revealed the presence of a well-defined lesion with an abnormal laminated pattern of cortical architecture involving most of the right cerebellar hemisphere, compressing the fourth ventricle and also a meningioma. Complete removal of the meningioma and partial removal of the cerebellar lesion was performed and histopathological exam confirmed Lhermitte-Duclos disease.
Deep brain stimulation is the most used neurosurgical procedure for advanced Parkinson’s disease. This procedure improves motor symptoms and some non-motor symptoms over a long period of time. In rare cases there are several complications which might lead to system removal. We report here a case were removal of the stimulation system was prompted by a scalp infection which extended to the subcutaneous fatty tissue and subcutaneous wire trajectories and did not resolved with antibiotics. The symptomatology worsened after device removal and reimplantation was decided. We followed this patient for 8 years after the first implant (6 years after the second implant) and he still has a good quality of live, being completely independent in most daily tasks.
