A Psiquiatria está sendo atingida, de certo modo tardiamente, pelas profundas modificações operadas nas idéias e interpretações do comportamento humano baseadas apenas no indivíduo isolado de seu grupo.As raízes metafísicas da psicopatologia muito contribuiram para retardar a evolução que hoje se processa, colocando a ciência do comportamento em flagrante atraso em relação às outras ciências do homem. A abertura freudiana inegavelmente representou rotura de certos tabus, liber¬ tando-nos de entraves incômodos nessa investigação. Entretanto, dogmatismo sectário determinou nova estagnação num campo que se prenunciava auspicioso para conquistas cientificamente bem fundamentadas e contribuiu para retardar a influência do enfoque social sobre a concepção do homem e de seu papel. É que, apenas aparentemente, o psiquiatra vienense teria levado em conta os fatores sociais nas suas concepções sobre o comportamento humano: a evolução do libido e o complexo de Édipo envolvem a constelação familiar, núcleo social por excelência. Também noutros trabalhos, muito ousados, por um processo de extrapolação de concepções ideadas para o indivíduo, Freud levou a conseqüências extremas a análise de fenômenos antropológico-culturais e religiosos, de nível social portanto, numa imperdoável falta de perspectiva filosófica o que, aliás, lhe custou justas e severas críticas. Assim, o espírito da obra freudiana é eminentemente individualista e o dogmatismo de que se reveste decorre precisamente de interpretação do mundo anímico do adulto, isolado do resto do universo e sujeito a leis e esquemas que já não mais sofrem as influências do mundo externo. Uma vez processada a marcha do indivíduo do nascimento aos 5 anos de idade, todo seu destino está traçado e o resto de sua vida é inteira e fatalmente sujeito às injunções que esses primeiros anos predeterminaram. O ego, mera instância de compromisso, é escravo do id, cujo poder desmesurado acabará por esmagá-lo nos casos de desobediência extrema. E o superego, forjado naquela fase pretérita, apesar de representar a introjeção de valores culturais, não vai pautar-se pelas normas atuais impostas pela existên-cia, mas sim pelo que do passado trouxe.Nos trabalhos em que analisa fenômenos sociais, o criador da psicaná-lise tomou às avessas o caso, e o comportamento grupai, mais complexo, foi por ele interpretado apenas como dimensão maior do fenômeno individual, psicológico. As an introduction, some doctrinary and technical problems dealing with the complexity of the human mind and the possible approaches to investigate and treat the personality are discussed and, among them, the "medical model" is emphasized. A brief description of some cases in the author's experience are presented; they, at first, had been very difficult to diagnose. However, after some time, they were found to be of indisputable neuro-psychogenetic pathogenesis. The psychosomatic aspects of each case are analyzed, with emphasis on the different levels of pathogenic action and the means of elaboration to defend and readapt the affected personality, resulting in its final clinical expression. There are four possible levels where the primary dysfunction may act; (a) physiopathologic neuronal level; (b) cerebral subsystemic level; (c) psychodynamic level; (d) psychosocial level. The primary dysfunction, acting as a new stimulus on each of these levels, may determine in the personality pathologic responses that may assume very complex clinical situations. Many of these responses are quite different from the precipitating primary dysfunction, although there are still many evident links among them.
