Este estudo objetivou descrever o processo de elaboração de um protocolo de enfermagem para avaliação do risco de quedas para pacientes hospitalizados. Trata-se de um relato de experiência, desenvolvido em uma unidade de internação de um Hospital de médio porte, localizado na cidade de Santa Maria, durante os meses de julho e agosto de 2022. A construção do protocolo deu-se através de três etapas: revisão de literatura, idealização do conceito e elaboração do protocolo. Como resultado construiu-se a Escala de Morse, a qual foi adaptada por Urbanetto (2013). A escala de Morse possui seis domínios: histórico de quedas, diagnóstico secundário, ajuda para deambulação, uso de dispositivos intravenosos, marcha e estado mental, com duas ou três possibilidades de resposta para cada um dos itens. Cabe a cada domínio avaliado, atribuir uma pontuação que pode variar de zero a 30 pontos, sendo importante que a equipe de enfermagem aplique a escala nas primeiras 48 horas de internação dos pacientes. Conclui-se que a criação da escala é uma ferramenta facilitadora na realização das atividades rotineiras, a fim de auxiliar no trabalho da equipe de enfermagem e na sistematização do cuidado do paciente internado.Palavras-chave: Acidentes por quedas; Prática Clínica Baseada em Evidências; Segurança do Paciente.
