Objective: To build and validate algorithms for prevention and treatment of incontinence-associated dermatitis. Method: A methodological study. An integrative review of articles published from 2009 to 2019 was carried out after a literature search in the main Health Sciences databases for the construction of the algorithms. The algorithms were evaluated by 27 nurses using the Delphi technique. The content validity index was used for data analysis. Results: The judges evaluated the algorithms from inadequate to adequate in the first round of consultation, and from adequate to totally adequate in the second round of consultation. The global content validity index was 0.923 in the first consultation and 1.0 in the second consultation. Conclusion: After reviewing the literature, the algorithms were constructed and validated by professionals with experience in the area, reaching agreement among the judges in the second round of consultation.

Para o desenvolvimento dos algoritmos, foi realizada revisão integrativa de artigos publicados de 2009 a 2019 encontrados nas principais bases de dados em ciências da saúde. A avaliação dos algoritmos foi realizada com 27 enfermeiros, utilizando a técnica de Delphi. Para a análise de dados, foi adotado o índice de validade de conteúdo. Resultados: Os juízes consideraram os algoritmos entre inadequados e adequados na primeira avaliação e entre adequados e totalmente adequados na segunda avaliação. O índice de validade de conteúdo geral foi de 0,923 na primeira avaliação e 1,0 na segunda avaliação. Conclusão: Após revisão da literatura, os algoritmos foram construídos e validados por profissionais com experiência na área, obtendo concordância entre os juízes na segunda avaliação.
Irrigating wounds with tap water does not increase colonisation, but controlled studies are required for further evidence. Microbial colonisation was assessed in skin wounds, before and after irrigation with tap water, and was compared with irrigation using 0·9% sodium chloride sterile solution. The study included 120 subjects with chronic, traumatic, vascular, pressure or neuropathic wounds. A total of 60 wounds were randomly assigned to be irrigated with tap water (tap water group) and another 60 to be irrigated with 0·9% sodium chloride sterile solution (saline group), at a pressure of 0·46-0·54 PSI. Samples were collected from the centre of each wound using Levine's technique, before and after irrigation, and cultivated in thioglycollate, hypertonic mannitol agar, eosin methylene blue (EMB) agar, blood agar and Sabouraud agar at 37°C for 72 hours. There was concordance (kappa test) and discordance (McNemar test) regarding the count of positive and/or negative samples before and after irrigation in each group. The proportion of reduction of positive samples was similar for both groups in all cultures. Colony-forming unit count before and after irrigation was similar in both groups and in all cultures, except for the culture in hypertonic mannitol agar from the tap water group, for which the count was lower after irrigation (Wilcoxon z = 2·05, P = 0·041). It is concluded that skin wound irrigation with tap water leads to further reduction of Gram-positive bacteria compared with 0·9% sodium chloride sterile solution, with no difference in colonisation of haemolytic bacteria, Gram-negative bacteria and fungi.
OBJETIVOS: Desenvolver um aplicativo para auxiliar na avaliação, prevenção e tratamento da dermatite associada à incontinência. MÉTODO: Estudo aplicado à produção tecnológica na área da saúde. Para o design do aplicativo, foram elaborados: conteúdo didático, definição dos itens relacionados à avaliação, prevenção e tratamento da dermatite associada à incontinência e foi feita a seleção das mídias. Foram escolhidos os dispositivos do aplicativo. Na fase de implementação, os dispositivos e os recursos tecnológicos foram instalados, e foi construído um ambiente para baixar o aplicativo na Internet e implementar a funcionalidade do aplicativo móvel. RESULTADOS: Após revisão integrativa da literatura, foi desenvolvido o DIAPERSKIN com 22 telas e 8 figuras. Ele foi registrado no Instituto Nacional da Propriedade Industrial sob o número BR-51-2018-000-720-1 e está disponível gratuitamente no Google Play Store. CONCLUSÃO: Foram descritos o planejamento e desenvolvimento, com base em evidências, de um aplicativo móvel para a avaliação, prevenção e tratamento de DAI, o qual poderá ser útil durante a assistência, na avaliação de pacientes, seleção de ações preventivas e na indicação do tratamento, assim como para a educação continuada de profissionais da enfermagem por meio do uso da tecnologia de fácil utilização.
