Severe trauma to soft tissue and the shafts of the tibia and fibula are usually caused by traffic accidents (46.7 %). More than half of these (56.4%) involve the middle third of the leg; in a third we have to deal with open fractures, 20 % of which belong to the third category of severity according to the classification of Tscherne and Oestern (1982). In a retrospective study, a third of 120 closed fractures showed signs of significant soft tissue lesions that belonged to the second and third categories of severity. The development of a special strategy of treatment is based on knowledge of the pathophysiology involved and equal respect for the principles of wound healing and biomechanics. It depends upon an extensive diagnosis and strictly distinguishes between open and closed fractures. The main complication of open fractures is infection; the main complication of closed fractures is the compartment syndrome.Zusammenfassung. Schwere Weichteil-und Knochenverletzungen des Unterschenkels werden im wesentlichen dutch StraBenverkehrsunf'fille (46,7 %) verursacht. Obcr die Hfilfte (56,4 %) sind im mittleren Drittel lokalisiert und in etwa einem Drittel handelt es sich urn offene Frakturen, von denen ca. 20 % dem Schweregrad III zuzuordnen sind. Von 120 geschlossenen Frakturen des Unterschenkels weisen in einer retrospektiven Studie ein Drittel Zeichen relevanter Weichteilsch/iden auf, die in der Klassifikation nach Tscherne und Oestern (1982) dcm Schweregrad II und III angeh6ren. Die Entwicklung einer speziellen Behandlungstaktik beruht auf Kenntnissen der Pathophysiologie und berticksichtigt biologische Vorg/inge der Wundheilung und der Biomechanik gleichermagen. Sic ist auf eine umfassende Diagnose angewiesen und unterscheidet streng offene yon geschlossenen Frakturen. Hauptkomplikation der offenen Fraktur ist die Infektion, der geschlossenen das Kompartmentsyndrom.
