Purpose To present in a single source the relevant information needed to assess spinopelvic balance and alignment, and to estimate the amount of correction needed in a patient during surgical treatment. Methods Narrative literature review Results Sagittal balance can be evaluated by global balance estimates (sagittal vertical axis and T1 tilt). Other important parameters are the relationship between pelvic incidence and lumbar lordosis (spinopelvic harmony), between pelvic incidence and difference of thoracic kyphosis and lumbar lordosis (spinopelvic balance), excess of pelvic tilt, knee flexion and thoracic compensatory hypokyphosis. Different methods to calculate the amount of surgical correction needed in patients with sagittal imbalance have been based on combinations of these parameters. Conclusions Relevant parameters of sagittal imbalance have been identified and correlated with clinical outcomes. Methods for calculation of surgical correction of imbalance have been proposed, but not validated in patients with midterm follow-up.

Introduction In adult spinal deformity (ASD), sagittal imbalance and sagittal malalignment have been extensively described in the literature during the past decade, whereas coronal imbalance and coronal malalignment (CM) have been given little attention. CM can cause severe impairment in adult scoliosis and ASD patients, as compensatory mechanisms are limited. The aim of this paper is to develop a comprehensive classification of coronal spinopelvic malalignment and to suggest a treatment algorithm for this condition. Methods This is an expert's opinion consensus based on a retrospective review of CM cases where different patterns of CM were identified, in addition to treatment modifiers. After the identification of the subgroups for each category, surgical planning for each subgroup could be specified. Results Two main CM patterns were defined: concave CM (type 1) and convex CM (type 2), and the following modifiers were identified as potentially influencing the choice of surgical strategy: stiffness of the main coronal curve, coronal mobility of the lumbosacral junction and degeneration of the lumbosacral junction. A surgical algorithm was proposed to deal with each situation combining the different patterns and their modifiers. Conclusion Coronal malalignment is a frequent condition, usually associated to sagittal malalignment, but it is often misunderstood. Its classification should help the spine surgeon to better understand the full spinal alignment of ASD patients. In concave CM, the correction should be obtained at the apex of the main curve. Introduction Sagittal balance is an independent predictor of clinical outcomes in spinal care. Surgical treatment is challenging and jeopardized by frequent complications. Guidelines for surgical treatment are currently not based on a classification of the disease. A comprehensive classification of sagittal balance based on regional deformities and compensatory mechanisms combined in deformity patterns is proposed. Though the sagittal shape of the spine can change due to degeneration or trauma, correlations between sagittal shape parameters and pelvic incidence (PI) have been described. Pelvic incidence is not changed by degeneration, thus representing a permanent source of information on the original sagittal shape of the spine. Methods One hundred and twenty-eight full-spine lateral standing radiographs of patients with different spinal conditions were evaluated and classified by one rater. One random subseries of 35 patients was evaluated by two raters for calculation of inter-rater agreement. Spinopelvic parameters were measured in all the radiographs. Internal validity of the classification system was evaluated comparing the values of regional sagittal parameters that distinguish one category from the others. Results Eight different patterns were identified regarding the site of the deformity and the presence of compensatory mechanisms: cervical, thoracic, thoracolumbar junction, lumbar, lower lumbar, global and pelvic kyphosis and normal sagittal alignment. Inter-rater agreement was almost perfect (j = 0.963). Statistically significant differences were found comparing the means of selected sagittal spinopelvic parameters that conceptually divide pairs or groups of categories: C2-C7 SVA for cervical kyphosis vs all other patients, TK-PI mismatch for thoracic kyphosis vs all other patients, T11-L2 kyphosis for thoracolumbar kyphosis vs all other patients, global alignment (LL?TK-PI) and SVA for lumbar kyphosis vs global kyphosis and pelvic tilt for pelvic kyphosis vs lumbar, lower lumbar and global kyphosis. Conclusion A comprehensive classification of sagittal imbalance is presented. This classification permits a better interpretation of the deformity and muscle forces acting on the spine, and helps surgical planning. Preliminary validation has been provided.

XLIF is a promising MIS option for adult deformity. Specific surgical strategies are needed to avoid imbalance and define ideal fusion levels and methods. An XLIF-based MIS strategy with a reduced number of levels of lumbar scoliosis can lead to significant advantages. Evaluation of the incidence, complications, their avoidance and real impact on patients' outcomes is necessary to better understand the advantages of this approach. Studies comparing effectiveness and safety of traditional versus XLIF approaches are needed to assist evidence-based decision making.

The classification system of spondylolisthesis proposed by Marchetti and Bartolozzi is the most practical regarding prognosis and treatment and includes the description of both low-and high-dysplastic developmental spondylolisthesis (HDDS). Unfortunately, it does not provide strict criteria on how to differentiate between these two subtypes. The accepted treatment for HDDS is surgical. However, there is no consensus on how to surgically stabilize this subtype of spondylolisthesis, and although the concept of reducing spinal deformity before fusion is attractive, the issue of surgical reduction versus in situ fusion remains controversial, especially for HDDS (Meyerding Grades III and IV). The purpose of this study was (1) to describe the severity index (SI) as a simple method that can be used in the identification of low-dysplastic developmental spondylolisthesis from HDDS allowing earlier surgical stabilization to prevent slip progression, (2) to provide guidelines for using the unstable zone for the inclusion of L4 in stabilization, and (3) to describe a surgical technique in the reduction and stabilization of this challenging surgical entity in an attempt to decrease the risk of iatrogenic L5 neurologic injury. The concepts of SI and unstable zone in the evaluation and treatment of HDDS are relatively new. In our study, patients with an SI value [20% were classified as having HDDS and surgical stabilization was offered. In addition, all vertebrae that were contained in the defined unstable zone were surgically instrumented and fused with attempts at anatomic reduction. This case series involved the retrospective radiological review of 25 consecutive patients surgically treated for HDDS between April 2000 and September 2004 by two senior surgeons. All 25 patients had a minimum 3-year follow-up. Reduction of slip, lumbosacral kyphosis, sacral inclination, fusion rate, maintenance of reduction, and iatrogenic L5 neurologic injury were evaluated. Twentytwo patients underwent a single-level L5-S1 fusion. Three patients had extension of the L5-S1 fusion to include L4 because it fell into the unstable zone. Slip improved from 67.2 to 13.6%, focal L5-S1 kyphosis improved from ?17.5°to -6.4°. There were no pseudoarthroses and all patients had radiographic evidence of solid bony fusion at latest follow-up. To date, there have been no re-operations secondary to progression of deformity or loss of fixation. Two re-operations were performed, one for a superficial wound infection, the other for further laparoscopic decompression for continued L5 nerve root symptoms after the index surgery. One patient developed an iatrogenic L5 radiculopathy with dysaesthesiae 3 days postoperatively which completely resolved over 6 weeks. HDDS is best treated surgically. Early identification and stabilization of this challenging surgical entity could prevent the progression of slip and deformity making the index surgery less technically demanding. Reports of XLIF fusion rate in the literature vary from 85 to 93 % at 1-year follow-up. Fusion rate in our series corroborates data from previous publications. The results of this series confirm that anterior inter body fusion by means of XLIF approach is a technique that achieves high fusion rate and satisfactory clinical outcomes.

Patients with concomitant unilateral HOA and LBP showed a marked anteverted FNA in the arthritic hip and a spinopelvic misalignment. After THR, a relief of both hip and low back pain and a change in spinopelvic parameters is observed.
