OBJECTIVES: to identify factors involved in not stopping smoking in spite of being pregnant. METHODS: standardized interviews were applied to 486 pregnant women in the pre-natal clinics of four health centers in the city of Rio de Janeiro, Brazil, between April 2003 and February 2004. Every time a smoker was identified, an additional interview, which included the Edinburgh Postnatal Depression Scale, the Fagerströn scale for nicotine dependence, and the Screening Questionnaire for Adult Mental Disorders, was carried out. RESULTS: the prevalence of smoking, in the initial stages of pregnancy was 21.1%. Most smokers presented a low level of nicotine dependence. Thirty-six percent of them stopped smoking by the first trimester of the present pregnancy without any specific medical intervention. Important differences between those who were able to stop and those who were not were alcohol intake and number of previous attempts at abstinence. Women who stopped smoking drank less during gestation. CONCLUSIONS: stopping smoking during pregnancy seems to be linked to a non-specific drive towards the well-being of the fetus. The number of previous attempts at abstinence was positively related to stopping at the beginning of pregnancy. In spite of the prevalence of the problem, there is still inadequate support for smokers in the prenatal services.
Método de coleta de evidências: As condições de abuso e dependência de substâncias químicas mais encontradas na população foram distribuídas entre os especialistas para elaboração do texto inicial. A elaboração destes textos foi fundamentada na experiência pessoal do especialista; nas recomendações de entidades nacionais e internacionais e na literatura científica disponível. Nove textos relacionados à abordagem geral, álcool, nicotina, benzodiazepínico, anfetamina, maconha, cocaína, opiáceo e solventes foram apresentados para avaliação dos elementos do grupo assessor. IntroduçãoA Cannabis sativa é um arbusto da família das Moraceae, conhecido pelo nome de "cânhamo" da Índia, que cresce livremente nas regiões tropicais e temperadas. Os seus efeitos medicinais e euforizantes são conhecidos há mais de 4 mil anos. Na China, existem registros históricos das suas ações medicinais desde o século III a. C.1 (D). No início do século passado, passou a ser considerada um "problema social", sendo banida legalmente na década de 30. O seu uso médico declinou lentamente, pois pesquisadores não conseguiram isolar os seus princípios ativos em função da rápida deterioração da planta. Alguns países começaram a relacionar o abuso da maconha à degeneração psíquica, ao crime e à marginalização do indivíduo. (1987, 1989, 1993 e 1997), a maconha foi a droga que mais teve seu "uso na vida" aumentado, passando de 2,8% em 1987 para 7,6% em 1997. Também o uso freqüente e o pesado aumentaram estatisticamente ao longo dos quatro levantamentos. O uso freqüente (seis vezes ou mais no mês) passou de 0,4% em 1987 para 1,7% em 1997 6 (C). Em levantamento domiciliar feito na cidade de São Paulo em 1999, com uma população acima de 12 anos, a maconha foi a droga que teve maior uso na vida (6,6%), seguida de longe pelos solventes (2,7%) e pela cocaína (2,1%) 7 (C). A Cannabis sativa contém aproximadamente 400 substân-cias químicas, entre as quais se destacam pelo menos 60 alcalóides conhecidos como canabinóides. Este artigo descreve o estado atual da Terapia Cognitiva, Comportamental, Prevenção de Recaída e Treinamento de Habilidades no tratamento de usuários de drogas. O objetivo é apresentar uma revisão sobre teorias e técnicas da Terapia Cognitiva e outras abordagens que dela derivam. Terapias Cognitiva e Comportamental, bem como Prevenção da Recaída e Treinamento de Habilidades, são tratamentos limitados no tempo, orientados em uma meta, e que utilizam sessões estruturadas, assumindo, assim, uma postura diretiva e ativa. Nós salientamos algumas diferenças entre Terapia Cognitiva, Prevenção de Recaída e Treinamento de Habilidades. A Terapia Cognitiva tem seu foco prioritariamente nos pensamentos, crenças, sentimentos e circunstâncias, como base do comportamento disfuncional. A Prevenção de Recaída e o Treinamento de Habilidades baseiam-se nas teorias comportamentais, além da teoria Cognitiva. Esperamos apresentar os últimos achados científicos para ajudar o psiquiatra geral a melhorar o tratamento da dependência química.
Leptin inhibits signaling of dopamine in the nucleus accumbens, suggesting its role in regulating stress and its possible involvement in the neurobiology of reward system. The aim of this study was to review of the literature on the influence of leptin in the craving for alcohol and tobacco and whether there is already evidence that leptin may be a biomarker to indicate risk for craving and relapse. The review used as data bases Medline, LILACS and SciElo in the period between 2000 and 2012. Keywords were leptin, substance use disorders, craving and withdrawal, in Portuguese and English. Only 12 articles were met the inclusion criteria, relating leptin with craving in alcoholics (n = 10) and smokers (n = 2). No studies were found in the LILACS database. Leptin levels increase during abstinence and this may be related to a reduction of dopaminergic action in mesolimbic system, resulting in a greater intensity of craving and maintenance of addictive behavior. Although there are few studies, the most recent results indicate the usefulness of leptin as a marker of risk for relapse among smokers and alcoholics in abstinence.

RESUMOObjetivo: Avaliar a atitude frente ao paciente alcoolista e o conhecimento sobre uso do álcool do estudante de Medicina em função do seu padrão de beber. Métodos: Estudo transversal realizado com estudantes matriculados no terceiro, sexto e décimo primeiro período dos cursos de Medicina de três universidades federais da Região Sudeste do Brasil (UFF, UFJF e Unifesp). Foram coletados dados sociodemográficos e aplicadas escalas de avaliação da atitude médica diante do paciente alcoolista. Os dados foram complementados por uma avaliação do conteúdo científico sobre alcoolismo e pelo Audit para avaliar o padrão de ingestão de álcool dos alunos. Resultados: Participaram do estudo 371 estudantes. Os consumidores de bebidas alcoólicas no padrão binge representaram 60,4% (n = 223), e 113 estudantes (30,7%) apresentaram comportamento de risco para o consumo de álcool (Audit ≥ 8). O padrão de consumo alcoólico não diferiu entre as universidades e nem entre os períodos avaliados (Kruskal Wallis, p > 0,05). Os alunos das três universidades aumentaram a pontuação na escala de conhecimentos com o progredir do curso. Entretanto, não houve correlação significativa entre conhecimento e padrão de beber (Spearman > 0,05). Quanto à atitude, foi observado que já é satisfatória no terceiro período e melhora no sexto período. Exceção foi observada entre os alunos da Unifesp, que já apresentaram atitude positiva quase máxima no terceiro período e que foi mantida até o final do curso.Conclusão: Embora a atitude dos alunos melhore ao longo do curso, assim como seu conhecimento sobre adição a substâncias, o comportamento de beber permanece o mesmo, sem correlação com os conhecimentos transmitidos a respeito dos riscos de beber. ABSTRACT Objective: To evaluate the attitude toward the alcoholic patient and the knowledge about the use of alcohol by the medical student according to his drinking pattern. Method: It was a cross-sectional study involving at random selected students enrolled on the third, sixth and eleventh semester in three federal universities of southeastern Brazil (UFF, UFJF and Unifesp). Method: Sociodemographic data were collected and the following instruments were applied: attitude evaluation scale, knowledge of scientific information scale on alcohol and management of the alcoholic patient and Audit for evaluation of alcohol problems. Results: The study included 371 students. Alcohol users in the binge pattern accounted for 60.4% (n = 223) and 113 students (30.7%) presented a risk behavior for alcohol consumption (Audit ≥ 8). The alcohol consumption pattern did not differ between the universities nor between the evaluated periods (Kruskal Wallis, p > 0.05). Students from the three universities increased the score on the knowledge scale as the course progressed.
