. PIODERMA GANGRENOSO ASSOCIADO A RETOCOLITE ULCERATIVA: RELATO DE CASO. ABCDExpress. 2017;1(2):1094.Instituição: HUB Introdução: O pioderma gangrenoso (PG) é uma dermatose rara, crônica, de difícil diagnóstico e tratamento. Está relacionado com doenças sistêmicas, dentre elas a doença inflamatória intestinal (DII), neoplasias malignas, artrites e doenças hematológicas. Manifesta-se classicamente como um nódulo ou pústula estéril que progride para uma úlcera necrótica mucopurulenta com borda indeterminada, edematosa, violácea e com expansão serpiginosa. Não existe tratamento padronizado ou algoritmo simples para escolha da sua terapia. Objetivo: Relatar o caso de um paciente com Retocolite Ulcerativa (RCU) e pioderma gangrenoso, com boa resposta a azatioprina e prednisona. Método: Revisão de prontuário, entrevista com a paciente e revisão da literatura. Relato de Caso: Paciente do sexo masculino, 32 anos, há 2 meses com quadro de diarreia mucossanguinolenta, febre e lesões hiperemiadas em face anterior de ambos os MMII. Fez uso de antibioticoterapia endovenosa (amoxicilina-clavulanato e clindamicina) evoluindo com persistência da diarreia e ulcerações extensas em MMII, de difícil cicatrização. Procedeu-se, então, biópsia da lesão de pele e colonoscopia, a qual evidenciou edema, enantema, friabilidade e áreas cicatriciais em toda a mucosa do colon e pseudopólipos, achados compatíveis com RCU. Iniciado pulsoterapia com metilprednisolona por 3 dias, substituída por prednisona em associação com azatioprina. O paciente evoluiu satisfatoriamente, com formação de tecido de granulação e rápida reepitelização das lesões. Conclusão: O PG é uma doença cutânea rara que se associa mais frequentemente à RCU. Apesar de muitos artigos indicarem uma melhor resposta com o uso de infliximab, observamos que houve uma resposta adequada com o tratamento empregado neste caso. Sendo assim, mais estudos e ensaios clínicos deverão ser realizados para obtermos mais dados para definição de protocolos de tratamento para o PG.
Instituição: HOSPITAL UNIVERSITÁRIO DE BRASILIA Paciente W.C.S, sexo masculino, 17 anos, estudante, natural da cidade de Rio Branco, Acre, com história de anemia ferropriva desde a infância, inclusive com necessidade de hemotransfusões periodicamente, apresentando há uma semana hemorragia digestiva alta na forma de melena. Durante investigação, foram evidenciadas múltiplas malformações vasculares hemangiomatosas em orofaringe, esôfago, estômago e intestino. Negava comorbidades. Havia relato de cirurgia prévia para correção de anormalidade congênita em membro inferior direito (pé torto congênito),além de ressecção de hemangioma em joelho direito e enterectomia segmentar devido a intussuscepção intestinal, como uma das complicações de um dos hemangiomas. Ao exame, paciente em bom estado geral, orientado em tempo e espaço, hipocorado, hidratado. Recebeu, portanto, o diagnóstico de Síndrome de Blue Rubber Bleb Nevus. Foi optado por iniciar tratamento medicamentoso com o Sirulimus, visto que a paciente apresentava múltiplos hemangiomas em todo o trato gastrointestinal. A Síndrome Blue Rubber Bled Nevus, diagnosticada pela primeira vez em 1958 por Willian Bennet Bean, também denominada de "Síndrome de Bean", é uma doença rara e hereditária,com uma incidência em torno de 200 casos descritos na literatura, comumente presente desde o nascimento em 30% dos pacientes, ou desde a infância em 9% ou ainda durante a primeira infância em 48%. É uma entidade que não apresenta predileção por sexo. Consiste na formação de múltiplas lesões hemangiomatosas, afetando a pele e tecido subcutâneo numa proporção de até 93%, além de vísceras e trato gastrointestinal em 76% dos casos e menos comum o sistema nervoso central. Trata-se, portanto, de uma enfermidade rara, que se não tratada devidamente, evolui com complicações crônicas, como anemia por perda sanguínea, com necessidade de hemotransfusões periodicamente. Os estudos mostram algumas possibilidades de tratamento para esta síndrome; uma das modalidades seria o tratamento endoscópico com a escleroterapia, plasma de argônio e fotocoagulação a laser, dentre outros, além do tratamento sistêmico com alguns medicamentos descritos na literatura, como o Octreotide, o qual pode estar relacionado a alguns eventos adversos, como a osteonecrose de quadril, além do Sirulimus. Alguns trabalhos mostram a segurança e a eficácia do Sirulimus para o tratamento desta Síndrome, mesmo que as outras terapias se mostrem eficazes, principalmente nos pacientes com múltiplos hemangiomas.
