Graft versus Host Disease is an entity that occurs after bone marrow transplant, in immunocompromised patients, characterized by dermatologic, hepatic and gastrointestinal manifestations. Morbimortality can be reduced significatively by an accurate and prompt diagnosis. The differential diagnosis includes neutropenic enterocolitis and C. difficile Colitis. Computed tomography can help in the diagnosis, with a constellation of findings as focal dilatation of bowel loops, and significant mucosal enhancement. Key words: Enterocolitis, Graft, Host.Resumen: La enfermedad de injerto contra huésped es un síndrome que ocurre en pacientes inmunodeprimidos generalmente post-trasplante de médula ósea, caracterizado en su etapa aguda por manifestaciones dermatológicas, hepáticas y gastrointestinales. El diagnóstico oportuno es fundamental, ya que puede disminuir significativamente la morbimortalidad. Dentro del diagnóstico diferencial de las manifestaciones gastrointestinales se incluye enterocolitis neutropénica y colitis pseudomembranosa. Mediante tomografía computada, hay signos que apuntan a su diagnóstico, como dilatación focal de asas, y refuerzo significativo de la mucosa enterocólica. IntroducciónLa enfermedad injerto versus huésped (EI-H), es un síndrome que ocurre en forma secundaria a la introducción de células inmunológicamente competentes en un huésped inmunodeprimido (1) . En el caso de los seres humanos, esto ocurre típicamente después de trasplante alogénico de médula ósea (TMO), siendo la primera causa de morbimor talidad en estos pacientes. Sus manifestaciones principales son a nivel de piel, hígado y tracto gastrointestinal.A pesar de la profilaxis, 15-50% de los pacientes con TMO desarrollaran esta enfermedad (2) , por lo que su diagnóstico precoz es muy importante, ya que el tratamiento consiste en producir mayor inmunosupresión y se deben descartar otras patologías, tales como infecciones, casos en los cuales el tratamiento inmunosupresor está contraindicado. Existen dos tipos de EI-H: agudo y crónico.El tipo agudo se caracteriza por dermatitis, elevación de pruebas hepáticas con un patrón colestásico, y enteritis en un plazo menor a 100 días después del T M O ( 3 ) como promedio, se inicia aproximadamente a los 20 días. Para que se produzca la EI-H, el injerto debe contener células inmunocompetentes, el huésped debe poseer aloantígenos que no posee el injerto, y finalmente el huésped debe estar inmunocomprometido.Clínicamente, el paciente presenta un rash doloroso o pruriginoso seguido de elevación de las pruebas hepáticas con un patrón colestásico caracterizado por elevación de bilirrubina, SGOT, SGPT y fosfatasas alcalinas. El tercer órgano comprometido es el intestino -en especial íleon distal y colon-y menos frecuentemente esófago y estómago. El compromiso puede ser difuso o segmentario, refiriendo el paciente diarrea, nauseas, vómitos, fiebre, dolor abdominal tipo cólico y hemorragia digestiva baja. Abstract: Intussusception is an uncommon event in adults, with different clinical presentations, which range from bowel obstruction symptoms to nonspecific abdominal pain. Can occur in the small or large bowel. In adults there is usually an underlying cause, usually benign or malignant neoplasms, being the latter the main cause of this disease in the colon. The diagnosis with CT is usually straightforward, with a very typical appearance. We report two cases of patients with bowel obstruction and diagnosis of intussusception caused by a lipoma and colon cancer respectively.
A case report of a patient with treated Non-Hodgkin Lymphoma is presented. In his usual tomographic control patient was requested a PET-CT scan to supplement prior study that showed a metabolically active focus on the left adductor muscle without evident tomographic correlation. Lesion underwent both a soft tissue ultrasound study and a directed biopsy, the latter being positive for secondary infiltration by lymphoma. This case has demonstrated the usefulness of applying complementary techniques in the management of these lesions, mainly of PET-CT scans in the study of unusual sites of spread. This combined medical imaging technique allows accurate lesion localization, which in turn permits performance of a subsequent directed study.
