Psychoneuroimmunology-based interventions are used to attenuated disease progression and/or side effects of pharmacological treatment. This systematic review evaluates the different therapeutic and/or clinical psychoneuroimmunology-based interventions associated to both psychological, neuroendocrine and immunological variables. The review was conducted for all English, Portuguese and Spanish language articles published between 2005 and 2015. Independent investigators analyzed 42 studies concerning human psychoneuroimmunology-based interventions. Decreased levels of cortisol, epinephrine and norepinephrine (stress-related hormones) were associated to interventions like yoga, meditation, tai chi, acupuncture, mindfulness, religious/spiritual practices, cognitive behavior therapy, coping and physical exercises. Moreover, those interventions were also associated to reductions in inflammatory processes and levels of pro-inflammatory cytokines in cancer, HIV, depression, anxiety, wound healing, sleep disorder, cardiovascular diseases and fibromyalgia. Despite the associations between PNI variables and clinical/therapeutic interventions, only one study evidenced significant effects on a disease progression.
Resumo Fragilidade é um estado de vulnerabilidade fisiológica multissistêmica relacionada à idade e a um risco aumentado de desfechos adversos. O objetivo do presente estudo foi avaliar a prevalência e os fatores associados à fragilidade no estudo FIBRA em Minas Gerais, Brasil. Selecionou-se uma amostra aleatória, estratificada por unidade territorial, sexo e idade, de 461 indivíduos, com 65 anos ou mais. A fragilidade foi estabelecida pela presença de três ou mais de cinco itens: sensação de exaustão, baixa força de preensão manual, velocidade da marcha lenta, perda de peso e baixo gasto calórico. A média de idade foi de 74,4 anos (DP± 6,8), 69,6% eram mulheres e 71,9% brancos. A prevalência de fragilidade foi de 5,2%; 49,9% foram de indivíduos pré-frágeis. Idade avançada (OR: 6,4; IC 1,76-23,8), comprometimento das atividades básicas de vida diária (OR: 5,2; IC 1,1-23,1) e auto percepção de saúde ruim (OR: 0,13; IC 0,03-0,4), foram associados à fragilidade. No presente estudo, um número substancial de indivíduos apresentou-se frágil, enquanto que metade da amostra estava sob risco de progressão para esta condição, sugerindo que é urgente a adoção de medidas de saúde pública com objetivo de prevenção e redução de complicações.
RESUMO.Os portadores de hanseníase vivenciam situações de preconceito que, com o estigma e a discriminação, culminam para o isolamento social e a restrição dos relacionamentos sociais. Este estudo teve o objetivo de avaliar a percepção do estigma nos indivíduos com hanseníase e suas repercussões sociais. Caracterizou-se por um estudo qualitativo mediante aplicação de entrevistas semiestruturadas em 20 usuários cadastrados para tratamento poliquimioterápico nas unidades de referência na Zona da Mata Mineira, no primeiro semestre de 2014. A análise foi realizada por meio da análise de conteúdo e foram definidas as seguintes categorias de análise: Desconhecimento sobre a doença; Diagnóstico e cura; Discriminação e medo; Encobrimento da doença; Apoio social; e Vínculo e participação social. O desconhecimento sobre a doença interfere no enfrentamento por parte dos indivíduos e, aliado ao receio da discriminação, foi suficiente para que eles ocultassem seu diagnóstico para os outros. Dessa forma, não foi possível perceber nenhuma alteração em seu vínculo social. Destacaram -se as diversas reações emocionais no momento do diagnóstico além da ênfase dada à cura pelos entrevistados. Neste trabalho, ficou evidente que o encobrimento da doença e o suporte social atuaram como fatores de proteção que impediram momentos de discriminação e restrição de participação social. Palavras-chave: Hanseníase; estigma; problemas sociais. PERCEPTION OF STIGMA AND SOCIAL IMPACTS ON INDIVIDUALS WITH HANSEN'S DISEASEABSTRACT. The individuals with Hansen's disease experience situations of prejudice that, together with stigma and discrimination, culminate in social isolation and restrictions in social relationships. This study aimed to evaluate the perception of stigma in individuals with Hansen's disease and its so cial repercussions. It was configured as a qualitative study, by means of semi-structured interviews with 20 users registered to chemotherapy treatment in reference units of Zona da Mata Mineira, in the first half of 2014. The analysis was performed through content analysis and the following categories were defined: Lack of knowledge about the disease; Diagnosis and cure; Discrimination and fear; Concealment of the disease; Social support; and Bond and social participation. Lack of knowledge about the disease interferes with the individual's ability to cope with it, and coupled with fear of discrimination, it was enough for them to conceal their diagnosis from others. Thus, it was not possible to notice any changes in their social bonds. The different emotio nal reactions at the moment of diagnosis stood out, in addition to the emphasis on healing given by respondents. In this study, it became clear that the concealment of the disease and the social support acted as protective factors that prevented moments of discrimination and restriction in social participation.
A noção de desenvolvimento está atrelada a um contínuo de evolução ao longo de todo o ciclo vital que, segundo a proposição de Vigotski, grande estudioso do tema, o desenvolvimento e a transformação dos indivíduos acontecem a partir dos planos genéticos do desenvolvimento humano (filogênese, ontogênese, sociogênese e microgênese). Com isso, o objetivo deste estudo foi abordar os planos genéticos como uma matriz de entendimento dos fenômenos que compõem as etapas da evolução humana. A filogênese estuda a evolução das espécies, por meio da adaptação progressiva desde os seus primórdios, abordando tanto as predisposições biológicas quanto as características gerais do comportamento humano. A ontogênese refere-se à evolução humana, iniciada na concepção, seguida de transformações sequenciadas até a morte, de tal forma que cada estágio apresenta um determinado nível de maturidade. A sociogênese estuda as interações sociais como sendo as raízes das funções mentais superiores, que só passam a existir no indivíduo na relação mediada com o mundo externo. A microgênese é caracterizada pela emergência do psiquismo individual no cruzamento dos fatores biológico, histórico e cultural, sendo crucial na questão da afetividade e no conceito de personalidade. Portanto, é de suma importância adensar estudos acerca de conhecimentos sobre esta perspectiva do desenvolvimento, os planos genéticos, que, uma vez juntos, vão caracterizar a gênese dos processos psicológicos no ser humano.
ResumoO contexto de isolamento social e de restrição dos relacionamentos sociais gerado pela hanseníase promove a diminuição na participação na comunidade, destacando a importância do suporte social. Desta forma, esse estudo teve como objetivo avaliar o grau de restrição de participação e o suporte social recebido pelos indivíduos em tratamento para hanseníase em dois centros de referência para hanseníase no município de Juiz de Fora-MG, Brasil. Foram aplicadas as Escalas de Participação e de Avaliação do Suporte Social, no primeiro semestre de 2014, nos indivíduos inseridos nesses centros. Para análise estatística foram utilizados teste de Wilcoxon, correlação de Spearman e teste exato de Fisher, com nível de significância de p<0,05. A partir dos resultados, observa-se que 60% dos sujeitos não apresentaram restrição de participação, e altos valores de disponibilidade e de satisfação dos suportes oferecidos. Pode-se ressaltar a família e os profissionais de saúde como fonte desse suporte. Destaca-se o suporte social tipo emocional, visto que os indivíduos com hanseníase que apresentavam incapacidades e os que não tiveram restrição de participação apontaram uma associação com a satisfação desse suporte. A partir deste estudo, conclui-se que o suporte social pode ser apontado como um fator protetor, tornando-se essencial a escuta ao indivíduo, não perdendo de vista a noção de que o cuidado não se remete apenas aos sinais visíveis da doença, mas antes a todo o contexto social ao qual o portador encontra-se inserido. AbstractThe context of social isolation and restriction of social relationships created by leprosy promotes decreased participation in the community, highlighting the importance of social support. Thus, this study aimed to assess the degree of participation restriction and social support received by individuals in treatment for leprosy in two reference centers for leprosy in the city of Juiz de Fora. The Participation and Evaluation of Social Support Scales were applied in the first half of 2014 to individuals inserted in these centers. Statistical analyses were performed using Wilcoxon test, Spearman correlation and Fisher's exact test, with a significance level of p<0.05. According to the results, 60% of the subjects did not present participation restriction, and high values of availability and satisfaction of the support offered were observed. The family and health professionals can be highlighted as this source of support. The emotional type of social support is noteworthy, as individuals with leprosy who had disabilities and those without participation restriction showed an association with the satisfaction of that support. It was concluded that social support can be seen as a protective factor, making it essential to listen to the individual, not losing sight of the notion that care does not refer only to the visible signs of the disease, but rather the whole social context to which the carrier is inserted.
Patients and the general public are under insurmountable psychological pressure which may lead to various psychological problems, such as anxiety, fear, depression, and insomnia, causing, consequently, the impaired quality of life. Psychological crisis intervention plays a pivotal role in the overall deployment of health-related quality of life and disease control. A novel Severe Acute Respiratory Syndrome Coronavirus-2 (SARSCoV-2), a pathogen of the new coronavirus disease (COVID-19), has affected several sector activities, including people's health. To enhance infection control methods, appropriate interventions, and public health policies, the present study aims to assess the fear and peri-traumatic stress during the Covid-19 in Brazil. Method: A cross-sectional survey has been conducted from April 12th to 18th using the Peri-Traumatic Distress Scale (CPDI) and the Fear Scale (FCV-19S) aiming to measure the peri-traumatic stress and fear as psychological reactions during the COVID-19 pandemic. For that purpose, an online spreadsheet was used to send the questionnaire and scales to a sample of 1844 participants as a collecting information tool. After the data analysis, the individuals were separated into 4 groups: Group 1 (1232) population without chronic health conditions; group 2 (298) patients with previous psychological suffering, group 3 (229) patients with cardiovascular diseases, group 4 (71) patients with diabetes. For analysis, G1 and were considered control for comparison with groups 2, 3 and 4 in accordance with One-Way Anova followed by Bonferroni test. Results: All the groups showed the CPDI and FCV-19S increased in comparison with the G1 group. Concerning CPDI, the G 3 was increased when compared to G1, G2 and G4. The G3 had the FCV-19S higher in comparison with G1, G2 and G4. The Wilcoxon-Mann-Whitney test showed a statistical difference between the control group in comparison with 2 and 3 groups (Mann-Whitney p< 0.05). Conclusion: The COVID-19 pandemic had a significant impact on the Brazilian population, with patients with heart disease and hypertension presenting the highest numbers of stress and fear, with numbers comparable and even higher than those who reported previous psychological distress.
