Dietary supplement use has steadily increased over time since the 1970s; however, no current data exist for the U.S. population. Therefore, the purpose of this analysis was to estimate dietary supplement use using the NHANES 2003-2006, a nationally representative, cross-sectional survey. Dietary supplement use was analyzed for the U.S. population (≥1 y of age) by the DRI age groupings. Supplement use was measured through a questionnaire and was reported by 49% of the U.S. population (44% of males, 53% of females). Multivitamin-multimineral use was the most frequently reported dietary supplement (33%). The majority of people reported taking only 1 dietary supplement and did so on a daily basis. Dietary supplement use was lowest in obese adults and highest among non-Hispanic whites, older adults, and those with more than a high-school education. Between 28 and 30% reported using dietary supplements containing vitamins B-6, B-12, C, A, and E; 18-19% reported using iron, selenium, and chromium; and 26-27% reported using zinc- and magnesium-containing supplements. Botanical supplement use was more common in older than in younger age groups and was lowest in those aged 1-13 y but was reported by ~20% of adults. About one-half of the U.S. population and 70% of adults ≥ 71 y use dietary supplements; one-third use multivitamin-multimineral dietary supplements. Given the widespread use of supplements, data should be included with nutrient intakes from foods to correctly determine total nutrient exposure.

Objective To examine if children use supplements to fill gaps in nutritionally inadequate diets or whether supplements contribute to already adequate or excessive micronutrient intakes from foods. Study design Data were analyzed for children (2–18 y) from the NHANES 2003–2006, a nationally representative, cross-sectional survey (n=7,250). Diet was assessed using two 24-hour recalls, and dietary supplement use was assessed with a 30-day questionnaire. Results Prevalence of supplements use was 21% (< 2 y) and 42% (2–8 y). Supplement users had higher micronutrient intakes than nonusers. Calcium and vitamin D intakes were low for all children. Inadequate intakes of phosphorus, copper, selenium, folate, and vitamins B-6 and B-12 were minimal from foods alone among 2–8 y olds. However, among 9–18 y olds, a higher prevalence of inadequate intakes of magnesium, phosphorus, and vitamins A, C, and E were observed. Supplement use increased the likelihood of intakes above the Upper Tolerable Intake Level for iron, zinc, copper, selenium, folic acid, and vitamins A and C. Conclusions Even with the use of supplements, more than a one-third of children failed to meet calcium and vitamin D recommendations. Children 2–8 y had nutritionally adequate diets regardless of supplement use. However, in children older than 8 y dietary supplements added micronutrients to diets that would have otherwise been inadequate for magnesium, phosphorus, vitamins A,C, and E. Supplement use contributed to the potential for excess intakes of some nutrients. These findings may have implications for reformulating dietary supplements for children.

Background:An innovative strategy for helping people achieve recommended levels of daily physical activity is dog walking. We assessed differences in physical activity and risk indicators between dog owners who 1) walk their dog (n = 399) and 2) do not walk their dog (n = 137) and compared them with adults who do not own dogs (n = 380).Methods:Participants (39 ± 13 years) were recruited online and completed an electronic questionnaire. Healthy People 2010 risk indicators included physical activity, overweight status, tobacco use, nutrition behaviors, chronic conditions, depressive symptoms, and social support.Results:Compared with dog walkers, those who did not own or walk their dog reported less physical activity (MET-min·week−1) and a higher body mass index (P < .01). Moreover, after adjusting for age and moderate to high physical activity, those who did not own dogs had significantly greater odds of self-reported diabetes [OR = 2.53; 95%CI (1.17−5.48)], hypertension [OR = 1.71; 95%CI (1.03−2.83)], hypercholesterolemia [OR = 1.72; 95%CI (1.06−2.81)], and depression [OR = 1.49; 95%CI (1.09−2.05)] compared with participants who regularly walked their dogs.Conclusions:Because of the health benefits associated with dog walking, this activity should be encouraged within communities as a method of promoting and sustaining a healthy lifestyle.

Objective Approximately 55–76% of Service members use dietary supplements for various reasons, including pain and related outcomes. This work evaluates current research on dietary ingredients for chronic musculoskeletal pain to inform decisions for practice and self-care, specifically for Special Operations Forces personnel. Methods A steering committee convened to develop research questions and factors required for decision-making. Key databases were searched through August 2016. Eligible systematic reviews and randomized controlled trials were assessed for methodological quality. Meta-analysis was applied where feasible. GRADE was used to determine confidence in the effect estimates. The committee made evidence-informed judgments and recommendations for practice and self-care use. Results Nineteen eligible dietary ingredients were assessed for quality, efficacy, and safety. Avocado soybean unsaponifiables, capsaicin, curcuma, ginger (as a food source), glucosamine, melatonin, polyunsaturated fatty acids, and vitamin D were conditionally recommended as their benefits outweighed risks, but there was still some uncertainty about the trade-offs. No recommendations were made for boswellia, ginger (as a dietary supplement), rose hip, or s-adenosyl-L-methionine. Recommendations were made against the use of collagen, creatine, devil’s claw, l-carnitine, methylsulfonylmethane, pycnogenol, willow bark extract, and vitamin E. Research priorities were developed to address gaps precluding stronger recommendations. Conclusions Currently the scientific evidence is insufficiently robust to establish definitive clinical practice guidelines, but processes could be established to track the impact of these ingredients. Until then, providers have the evidence needed to make informed decisions about the safe use of these dietary ingredients, and future research can address existing gaps.

Dietary supplements (DS) have been purported to affect endothelial function and reduce cardiovascular disease risk. A systematic PubMed search (January 2000 to July 2011) identified 586 clinical trials that utilized DS, along with a standardized vascular function measure, in healthy and at risk subjects. 56 randomized controlled trials met the selection criteria for folic acid, omega‐3 fatty acids, cocoa, and isoflavones. The review utilized scoring criteria for design quality (Jadad, 1996), DS test material quality (CONSORT botanical guidelines; Gagnier, 2006), and nutrient reporting guidelines (Chung, 2009). Preliminary results: mean Jadad scores were 3.62, 3.12, 3.33, 3.60 (out of 5), for folic acid, omega‐3 fatty acids, cocoa, and isoflavones, respectively. For DS receiving a CONSORT score, cocoa and isoflavones, 92.6% of papers reported the DS proprietary name, 44.4% reported dosage, duration, and rationale for dose used, and only 14.8% reported chemical analysis prior to use. All four DS received a nutrient quality score. 73.2% of the papers reported the DS source or supplier, 60.7% reported dosage and duration, 53.6% provided baseline nutrient biomarker assessment, and 32.1% provided baseline nutrient assessment. While Jadad scores were comparable across DS, nutrient quality scores ranked higher for cocoa and isoflavones compared to folic acid and omega‐3 fatty acids (p<0.05).
