Women with a history of severe preeclampsia exhibit early structural and functional preatherosclerotic vascular impairment, which might explain their higher risk of future cardiovascular disease.

Vitamin D supplementation increased insulin sensitivity and decreased androgen levels in vitamin-D-deficient women with PCOS but did not have any effect in vitamin-D-deficient non-PCOS women. These results may indicate the possible role of vitamin D in the complex pathogenesis of PCOS.

Despite the positive effects of hyperandrogenemia and hyperinsulinemia, PCOS patients have lower bone mineral density due to hypoestrogenism. Insulin resistance, BMI, estrogen, and androgen levels are the determinants of BMD in PCOS.

Aim: To determine the obstetric and neonatal outcomes of pregnant patients having undergone laparoscopic sleeve gastrectomy (LSG) in the previous 12 months. Methods: This retrospective and observational study included 144 pregnant women: 48 had pregnancies within 12 months after LSG (Group A), 42 became pregnant more than 1 year after surgery (Group B) and 54 obese pregnant women who had a body mass index (BMI) >30 kg/m 2 , were categorized as the control group because they did not undergo surgery (Group C). The participants' early gestational BMI, predelivery BMI and gestational weight gain were determined and the obstetric and neonatal outcomes of the groups were compared. Results: The time interval from surgery to conception was 7.8 AE 3.4 months and 25.8 AE 13.4 months for Groups A and B, respectively (P < 0.01). There were no significant differences in the rates of gestational hypertension, pre-eclampsia, preterm delivery and cesarean section between the groups. The number of patients with gestational diabetes mellitus was significantly higher in Group C than in Groups A and B (P = 0.004). The number of large-for-gestational-age infants was higher in Group C than in Groups A and B (P = 0.046). The number of small-for-gestational-age infants was significantly higher in Group A than in Groups B and C (P = 0.025). Conclusion: Pregnancy within one year after LSG is related to an increased risk of small-for-gestational-age infants. LSG decreases the risk of gestational diabetes mellitus independent of the time interval between surgery and conception.

Amaç: Laparoskopik kistektominin ovaryan rezerv üzerine etkisini endometriomalı ve dermoid kistli hastalarda karıştırmaktır. Gereç ve Yöntemler: Ultrasonografik olarak preoperatif tanı konulmuş 36 endometriomalı (grup A) ve 32 dermoid kistli (grup B) hasta çalışmaya alındı. Preoperatif anti-mullerian hormon (AMH) ve kistin olduğu overdeki antral folikül sayıları (AFC) ölçüldü. Tüm hastalara laparoskopik kistektomi stripping tekniği uygulanarak yapıldı. Ameliyattan 3 ay sonra tüm hastaların AMH ve AFC ölçümleri siklusun 3. ve 6. günleri arası tekrar yapıldı. Bulgular: Preoperatif serum AMH seviyeleri ve opere olan taraftaki AFC değerleri grup A'da grup B'de göre anlamlı derecede düşük saptandı [(p=0,001),(p=0,002)], sırasıyla. Ameliyattan 3 ay sonra serum AMH seviyesi grup A'da 2,04±0,68'den 1,47±0,55'e (p=0,001), grup B'de 2,60±0,57'den 2,17±0,56'ya (p=0,001) düştü. AFC seviyesi grup A'da ameliyattan 3 ay sonra 4,05±1,24'ten 2,16±0,94'e, grup B'de 4,93±0,94'den 3,40±0,87'ye (p=0,001) düşmüştür. AMH seviyesindeki düşüş grup A'da grup B'ye göre anlamlı derecede daha fazla olarak görüldü (p=0,033). AFC değerindeki düşüş grup A'da grup B'ye göre anlamlı oranda daha fazla idi (p=0,044). Sonuç: Laparoskopik stripping tekniği hem endometriomalı hem de dermoid kistli hastalarda serum AMH ve opere olan taraftaki AFC seviyeleri üstünde yıkıcı etkilere sahiptir. Endometriomaların laparoskopik olarak çıkartılması ovaryan rezerv üzerine dermoid kistlere göre daha yıkıcı etkilere sahiptir.

The RPL patients diagnosed with inherited thrombophilia and who were given LMWH with low-dose aspirin had higher live-birth rates and lower miscarriage rates than those in the unexplained RPL patients. Increased risk of preeclampsia is seen in RPL patients with inherited thrombophilia despite thrombophilia prophylaxis.
