Amaç: Antibiyotikler, dünyada en fazla kullanılan ilaçlar arasındadır. Antibiyotiklerin endikasyon dışı ve bilinçsizce kullanımı hem mikroorganizmaların direnç geliştirmesine hem de ekonomik yüke neden olabilir. Bu çalışmada Kırıkkale İlindeki iki hastanede yatan hastaların antibiyotik kullanımları ve uygunsuz antibiyotik kullanım oranları araştırılmıştır. Gereç ve Yöntem: Çalışma; Kırıkkale ilinde, biri devlet, diğeri üniversiteye bağlı iki büyük hastanede nokta prevalans metoduyla yapılmıştır. Çalışmanın yapıldığı gün hastanelerin doluluk oranı, yatan hasta sayısı, kullanılan antibiyotikler kayıt altına alındı. Hastaların antibiyotik kullanımlarının uygun endikasyonda olup olmadığı çalışma grubunda yer alan enfeksiyon hastalıkları uzmanlarınca değerlendirildi. İstatistiksel ölçümler için SPSS 15.0 programı kullanıldı. Bulgular: İki hastanede toplam 443 hasta yatıyordu ve bunların 221’i antibiyotik tedavisi alıyordu. Hastaların, %92.8’sine antibiyotik tedavisi ampirik olarak başlanmışken, %7.2’si kültür sonuçlarına göre başlanmıştı. Antibiyotik kullanan hastaların 137’sinde (%61.9) antibiyotik kullanımı uygundu. Antibiyotikler, çoğunlukla tedavi amaçlı başlanmıştı. Profilaktik amaçlı tedavi alan hastaların %19.1’ine uygun proflaksi yapılmışken, tedavi amaçlı antibiyotik alan hastaların %73.5’i uygun antibiyotik tedavisi almıştı ve fark istatistiksel olarak anlamlıydı (p<0.001). Enfeksiyon hastalıkları konsültasyonu yalnızca 53 hastadan istenmişti ve konsültasyon istenmesi antibiyotik kullanımında uygunluk oranını arttırmıştı. Cerrahi klinikler en çok uygunsuz antibiyotik kullanan kliniklerdi.Sonuç: Antibiyotik tedavisine başlanılmadan önce, enfeksiyon hastalıkları uzmanından görüş alınmasının önemli olduğunu düşünüyoruz.
