É cada vez maior a importância atribuída aos fatores psíquicos na patologia humana. Graças, sobretudo, à escola psicanalítica de Chicago, estamos assistindo ao desenvolvimento da medicina psicossomática, que tende a demonstrar a existência de componentes psicológicos na gênese de grande número de doenças. A originalidade desta escola reside menos nos fatos, muitos dos quais já tinham sido assinalados por mé-dicos de várias épocas, que na rigorosa interpretação de fenômenos que vão sendo concatenados paulatinamente, dentro de um sistema de idéias perfeitamente coerente.A escola de Chicago tem dado o melhor de sua atenção às perturbações nas quais é evidenciável a ação do sistema nervoso vagossimpático. Como é sabido, encontram-se os centros superiores autônomos ao nível da região hipotálamo-infundibular e são controlados pelo córtex cerebral através de fibras descendentes córtico-hipotalâmicas. Esta mesma região do diencéfalo atua sobre a hipófise, a qual, por sua vez, regula a secreção de todas as glândulas endocrinas. Por esta forma, rege o córtex, de maneira indireta, o funcionamento de todos os órgãos e aparelhos do corpo, e compreende-se com facilidade o mecanismo fisiológico da formação dos sintomas psicossomáticos, devidos a alterações endocrinas ou circulatórias ou a perturbações tróficas dos tecidos.Há, entretanto, outras importantes correlações psicos somáticas que se podem chamar indiretas em oposição às correlações diretas, antes enumeradas. Referimo-nos aos doentes que, por sua conduta, provocam em si mesmos lesões, mutilações, ferimentos ou outras perturbações. Merece ser recordado o caso famoso narrado por Dieulafoy 1 . Conta êle a história de um rapaz que, por meio da potassa, provocou lesões gangrenosas nos dedos, levando os cirurgiões a amputá-los.
