Pulmonary actinomycosis presented as a mass-like consolidation. The diagnosis was not established until clinical suspicion prompted special staining of the pathology material. Response to antibiotics was facilitated by a removal of the airway plug through bronchoscopy. Airway obstruction may contribute to the prolonged medical treatment of pulmonary actinomycosis.
Hintergrund: Die klinischen Folgen von asbestassoziierten diffusen Pleuraverdickungen (diffuse pleural thickening, DPT) sind bisher ungeklärt.Ziele: Klärung der Zusammenhänge zwischen dem Bildgebungsbefund DPT und der Lungenfunktion.Methoden: Von Patienten mit asbestassoziierter DPT wurden medizinische Daten zu folgenden Aspekten gesammelt: Asbestexposition am Arbeitsplatz oder im Wohnumfeld in der Vorgeschichte; anfängliche Symptome; Schwere der Dyspnoe laut modifizierter Skala des Medical Research Council; Raucheranamnese; radiologische Befunde; Ergebnisse von Lungenfunktionstests.Ergebnisse: Zwischen 2005 und 2010 gab es in den Institutionen der Autoren 106 DPT-Patienten (103 Männer (97,2%) und 3 Frauen (2,8%)). Der Altersmedian bei Diagnosestellung lag bei 69 Jahren (Bereich 46-88 Jahre). Die Asbestexposition am Arbeitsplatz erlebten die Patienten in folgenden Formen: Herstellung von Asbestprodukten (n = 17); Schiffbau (n = 14); Bau (n = 13); Wärmedämmungsarbeiten (n = 12); Sanitärarbeiten; Aufbringung von Asbestspray; Elektrikerarbeiten (jeweils n = 7) sowie Transport und Abrissarbeiten (jeweils n = 4). Die mediane Dauer der Asbestexposition betrug 25 Jahre (Bereich 2-54 Jahre), und die mittlere Latenzzeit bis zum Einsetzen der DPT betrug 46 Jahre (Bereich 25-66 Jahre). Die Beteiligung des Zwerchfell-Rippen-Winkels (costophrenic angle; CPA) korrelierte negativ mit der prozentualen Vitalkapazität (%VC; r = -0,448, p < 0,01). Die Dicke der Pleura und die Ausdehnung der Pleuraverdickung in kraniokaudaler und horizontaler Richtung laut Thorax-Computertomographie (Thorax-CT) korrelierten ebenfalls negativ mit %VC (r = -0,226, p < 0,05; r = -0,409, p < 0,01 bzw. r = -0,408, p < 0,01).Schlussfolgerungen: DPT entwickeln sich nach einer langen Latenzzeit in der Folge von Asbestexposition am Arbeitsplatz und verursachen ausgeprägte Störungen der Atemfunktion. Das Ausmaß der DPT sollte durch eine Thorax-CT beurteilt werden; das konventionelle Röntgen des Thorax ist bedeutsam, um die Beteiligung des CPA zu beurteilen.Übersetzung aus Respiration 2014;88:277-284 (DOI: 10.1159/000364948)
