La Guidance Interactive est une psychothérapie parent–enfant, dyadique, utilisant la voie d’entrée des interactions comportementales pour mobiliser progressivement les patterns dysfonctionnels de la relation parent–enfant. Nous décrirons cette technique validée par plusieurs études scientifiques. Nous présenterons ici son intérêt dans le champ des interventions précoces en périnatalité à travers des cas cliniques de situations de traumatisme ou de dépression maternelle périnatale.
Définie par des critères uniquement somatiques, la catégorie des nouveau-nés dits « à risque » ou « vulnérables », à tout le moins dans le cadre des protocoles de dépistage destinés (à juste titre) à cette population cible, nous paraît encore trop souvent faire l’impasse sur le fait suivant : ces nouveau-nés sont également à risque d’avoir des parents « exposés », grandement fragilisés, sinon parfois cliniquement traumatisés par le contexte ayant accompagné la grossesse et/ou la naissance de leur bébé. À partir de situations cliniques illustrant, dans la souffrance de premiers liens, l’intrication du « naître humain » et du « devenir parent » (Missonnier, 2009), le présent article montre l’intérêt des interventions précoces d’une unité de pédopsychiatrie périnatale et plaide en faveur d’échanges et de dispositifs créatifs entre nos équipes.
