Decentralizing mental health care and integrating psychiatry in general hospitals requires new strategies for mental health work. The Austrian Structural Plan for Health 2006 identifies psychiatric departments in general hospitals as pacemakers in creating a needs-based service organization on a regional level. We describe psychiatric departments in general hospitals, how they specialize in responding to the needs of a particular region, and how they relate to community mental health services and general health care.

Dieses Bild, von einem Menschen gemalt, der nach Beginn seiner Erkrankung seine künstlerischen Fähigkeiten entdecken und entfalten konnte, hat durchaus Bezug zum Inhalt dieses Heftes: Lange Zeit war den psychisch Kranken die Integration in die Gesellschaft weitgehend verschlossen. Die strukturellen Bedingungen hierfür waren nicht gegeben. Dies hat sich in den letzten 20 Jahren durch den Ausbau flächendeckender psychosozialer Dienste entscheidend gebessert. Diese Strukturverbesserung bekam in den letzten Jahren durch die Gründung von österreichweit inzwischen 12 psychiatrischen Fachabteilungen an Allgemeinkrankenhäusern einen entscheidenden Schub. Dennoch bleibt noch viel zu tun. Ein wichtiger Markstein auf diesem Weg dürfte der derzeit noch als Entwurf vorliegende Österreichische Strukturplan Gesundheit 2006 des Öster-reichischen Bundesinstitutes für Gesundheitswesen (ÖBIG) werden, der einen grundsätzlichen Wandel in der Struktur der medizinischen Versorgung in Österreich vorsieht. Nicht mehr die Entwicklung und Ausstattung einzelner Institutionen, sondern der Versorgungsbedarf für eine bestimmte Versorgungsregion steht im Vordergrund der Planung. Die österreichische Gesundheitsplanung orientiert sich demnach an vier Versorgungszonen, die sich auf 32 Versorgungsregionen aufteilen. Die einzelnen Institutionen werden somit zu Leistungserbringern, die sich in einer bestimmten Versorgungsregion arbeitsteilig auf den festgestellten Bedarf hin abstimmen müssen.Für den Bereich Psychiatrie bekennt sich der Strukturplan zu dem Prinzip "ambulant vor stationär". Das System der psychiatrischen Versorgung soll in die bestehende allgemeinmedizinische und psychosoziale Grundversorgung integriert sein, an den allgemeinen Krankenanstalten sollen Fachabteilungen für Psychiatrie errichtet werden. Es sei eine Vernetzung der extra-und intramuralen Einrichtungen durch einen gemeindepsychiatrischen Verbund anzustreben. Der psychiatrischen Fachabteilung am Allgemeinkrankenhaus kommt bei dieser Entwicklung eine Schrittmacherfunktion zu.Diese Struktur stellt für den Bereich der psychiatrischen und psychosozialen Versorgung eine ganz besondere Herausforderung dar. Ich selber hatte das Glück, als Bereichsleiter der Akutpsychiatrie am Bezirkskrankenhaus in Kaufbeuren/Bayern eine Versorgungsregion kennen zu lernen, in der die Regionalisierung der Psychiatrie im Gegensatz zu einigen anderen Regionen Bayerns durch Gründungen von Abteilungen an Allgemeinkrankenhäusern nicht erst geplant oder begonnen, sondern bereits abgeschlossen war. Mich faszinierte, wie sich, ausgehend von der Mutterklinik in Kaufbeuren, für jedes der Kerngebiete der Psychiatrie eine eigene, ambulanzzentrierte Versorgungsstruktur etabliert hatte. Umso mehr hat es mich gefreut, in einer psychiatrisch und psychosozial gesehen sehr dynamischen Region Mitteleuropas, näm-lich in Oberösterreich, im Rahmen des dortigen Re...
