Mucopolysaccharidosis IVA (MPS IVA), also known as Morquio-Brailsford or Morquio A syndrome, is a lysosomal storage disorder caused by a deficiency of the enzyme N-acetyl-galactosamine-6-sulphate sulphatase (GALNS). MPS IVA is multisystemic but manifests primarily as a progressive skeletal dysplasia. Spinal involvement is a major cause of morbidity and mortality in MPS IVA. Early diagnosis and timely treatment of problems involving the spine are critical in preventing or arresting neurological deterioration and loss of function. This review details the spinal manifestations of MPS IVA and describes the tools used to diagnose and monitor spinal involvement. The relative utility of radiography, computed tomography (CT) and magnetic resonance imaging (MRI) for the evaluation of cervical spine instability, stenosis, and cord compression is discussed. Surgical interventions, anaesthetic considerations, and the use of neurophysiological monitoring during procedures performed under general anaesthesia are reviewed. Recommendations for regular radiological imaging and neurologic assessments are presented, and the need for a more standardized approach for evaluating and managing spinal involvement in MPS IVA is addressed.
Etwa 50-75 % aller seltenen Erkrankungen beginnen in der Kindheit. Doch durch verbesserte Diagnostik und die zunehmenden Therapieoptionen erreichen viele Kinder mit einer seltenen Erkrankung das Erwachsenenalter, sodass sie nach Erreichen des 18. Lebensjahres in die Obhut der Erwachsenenmediziner übergehen. Bislang gibt es keine Leitlinien oder Finanzierungsmodelle für diese Transition, die sowohl für die Patienten und deren Familien als auch die behandelnden Ärzte keine leichte Aufgabe ist. Erwachsenenmediziner werden somit immer öfter mit pädiatrischen seltenen Erkrankungen konfrontiert. In den letzten 14 Jahren hat sich die Erforschung seltener Erkrankungen und deren Therapien rasant entwickelt: während es 1999 lediglich 5 Arzneimittel gegen seltene Erkrankungen gab, sind es heute 106, aber 1200 Medikamente befinden sich in der Testphase bzw. warten auf eine Zulassung. Um den Betroffenen einen frühzeitigen Therapiestart zu ermöglichen, ist eine frühe Diagnosestellung essentiell. Die lysosomalen Speichererkrankungen -insbesondere die Mukopolysaccharidosen -sind typische Modellerkrankungen für seltene neurologische Erkrankungen im Kindesalter. Ein weiteres Beispiel für eine seltene Erkrankung im Kindesalter ist die tuberöse Sklerose. Für beide dieser multisystemischen Erkrankungsgruppen sind Therapien verfügbar. Die milden Verlaufsformen werden jedoch oft erst im jungen Erwachsenenalter diagnostiziert. Im nationalen Aktionsplan für Menschen mit seltenen Erkrankungen werden Kompetenzzentren empfohlen, die sich Patienten jeglichen Alters mit seltenen Erkrankungen in einem interdisziplinären Team annehmen. Ziel ist es, die Lebensqualität dieser besonderen Patienten zu verbessern. Etwa 80 % aller Kinder mit chronischen Erkran kungen -auch Kinder mit seltenen Erkrankun gen -erreichen das Erwachsenenalter. Der wis senschaftliche Fortschritt und die zunehmende Aufmerksamkeit für seltene Erkrankungen ha ben dazu geführt, dass deutlich verbesserte diag nostische und therapeutische Möglichkeiten zur Verfügung stehen. Somit können viele seltene Erkrankungen tatsächlich früher diagnostiziert und behandelt werden. Seltene Erkrankungen zeichnen sich oft -auch innerhalb einer Erkran kung -durch eine enorme Variabilität und Hete rogenität in ihrer Krankheitsschwere und ihrem Krankheitsverlauf aus. Insbesondere milde Krankheitsverläufe werden oft spät erkannt. Für Pädiater ist es in der Regel sehr schwierig, ihre Patienten über das Kindesalter hinaus wei ter zu betreuen. Eine Erlaubnis muss individuell bei der Krankenkasse beantragt werden. Im günstigsten Falle wird diese dann per Einzelfall entscheidung für einen limitierten Zeitraum (maximal bis zum 25. Lebensjahr) genehmigt. Erwachsenenmediziner sind also zunehmend mit Erkrankungen konfrontiert, die eigentlich als "pädiatrische Erkrankungen" galten.
