Le cure palliative pediatriche (CPP) rappresentano un ambito multidisciplinare della pediatria che si prefigge l’obiettivo di garantire la qualità di vita a bambini affetti da malattie inguaribili ad alta complessità assistenziale e di rispondere ai loro bisogni e a quelli delle loro famiglie. A livello clinico, molteplici sono i sintomi disturbanti con ampia ricaduta sulla qualità della vita, che devono essere gestiti e trattati. Alcuni di questi in modo particolare rappresentano una sfida importante per i caregiver e i sanitari che si occupano dei bambini eleggibili alle CPP. Il prurito e i disturbi del sonno sono frequenti e invaldanti in CPP e possono determinare un impatto fortemente negativo sulla qualità di vita del bambino e possono risultare difficili da gestire per i clinici.
Pediatric palliative care is a multidisciplinary area of pediatrics that aims to ensure a quality of life for children suffering from incurable diseases with high care complexity and also respond to their needs and those of their families. At the clinical level, there are many disturbing symptoms with a wide impact on the quality of life, which must be managed and treated. In particular some of them represent a major challenge for caregivers and health professionals looking after children eligible for PPC. This work aims to present, with a practical and immediately useful cut for the pediatrician, the management strategies of frequent symptoms, starting with drooling and mucus plugs; the latter are very frequent in children with non-oncological disease, followed by PPC. If not treated, they limit the child’s quality of life and have a significant impact on the concerns and management of family members / caregivers.

A livello clinico, molteplici sono i sintomi disturbanti con ampia ricaduta sulla qualità della vita, che devono essere gestiti e trattati. Alcuni di questi, in modo particolare, rappresentano una sfida importante per i caregiver e i sanitari che si occupano dei bambini eleggibili alle CPP. Questo secondo lavoro si propone di presentare, con un taglio pratico e di immediata utilità per il pediatra, le strategie di gestione di tosse e astenia. Nell’ambito delle CPP questi due sintomi sono frequenti. La tosse e l’astenia, assieme agli atri sintomi respiratori, al dolore, alla difficoltà di alimentazione, rappresentano un’importante determinante della qualità di vita nei pazienti, impattando significativamente sulla capacità di svolgere le attività della vita quotidiana e sulla loro sfera psicologica e sociale. Per entrambi i sintomi ci sono ancora pochi studi in letteratura che guidino il clinico nella loro gestione, che a oggi si basa ancora troppo sull’esperienza individuale piuttosto che sull’evidenza scientifica.
