The purposes of this study were to assess intra- and interrater reliabilities by novice users of the Myotonometer (Neurogenic Technologies, Inc., Missoula, MT), a portable electronic device that quantifies muscle tone (stiffness) and paresis, in assessing children with cerebral palsy. Two raters used the Myotonometer to assess the biceps brachii and medial gastrocnemius muscles of 10 children with spastic-type cerebral palsy. Muscles were measured in a relaxed state and during a voluntary isometric contraction. Intraclass correlation coefficients and repeatability coefficients were calculated for each muscle and for each condition (relaxed and contracted). Intrarater reliabilities ranged from 0.82 to 0.99 (biceps brachii muscles) and 0.88 to 0.99 (medial gastrocnemius muscles). Interrater reliabilities ranged from 0.74 to 0.99 (biceps brachii muscles) and 0.84 to 0.99 (medial gastrocnemius muscles). Repeatability coefficients indicated a 98% level of agreement between raters across all conditions. Novice users of the Myotonometer, with few exceptions, had high to very high intra- and interrater reliabilities for measurements of the biceps brachii and medial gastrocnemius muscles of children with spastic-type cerebral palsy.
Myotonometer and sEMG measurements were highly correlated. Tissue stiffness, as measured by the Myotonometer, appears capable of assessing changes in muscle activation levels.
SUMMARY
Experiments were performed to determine whether spinal and supraspinal components of reciprocal inhibition (a neural mechanism responsible for the prevention of muscular co‐ordination during voluntary movement) were present in groups of non‐disabled children and children with cerebral palsy. Changes in the gastrocnemius‐soleus H reflex were examined during voluntary dorsiflexion and plantarflexion of the ankle and during a vibration applied to the anterior tibial tendon. The results indicate that children with cerebral palsy have impairments in reciprocal inhibition, both before and during voluntary movement. These deficits, which involve damage to supraspinal centres, contribute to their inability to perform smooth, co‐ordinated movements.

Une expérimentation a été réalisée pour déterminer si les composantes spinales et supra‐spinales de l'inhibition réciproque (mécanisme neuronique responsable de la prévention d'une co‐contraction musculaire durant le mouvement volontaire) étaient présentes chez les adolescents et les enfants avec infirmité motrice cérébrale. Les changements dans le reflexe H du triceps sural ont été examinés durant la flexion volontaire dorsale et planaire de la cheville et durant une vibration appliquée au tendon du jambier antérieur. Les résultats indiquent que les enfants porteurs d'IMC présentent des troubles de l'inhibition réciproque à la fois avant et durant le mouvement volontaire. Ces déficits liés à des lésions des centres supra‐spinaux, contribuent à l'incapacité de réaliser des mouvements souples et coordonnés.

Störungen der reziproken Inhibition bei Kindern mit Cerebralparese, festgestellt durch H‐Reflex Testung
Es war das Ziel dieser Untersuchungen, festzustellen, ob bei Jugendlichen und Kindern mit Cerebralparese spinale und supraspinale Komponenten der reziproken Inhibition (ein neuraler Mechanismus, der die muskuläre Kokontraktion bei der willkürlichen Bewegung verhindert) vorhanden sind. Die Veränderungen beim Gastrocnemius‐Soleus H Reflex wurden bei der willkürlichen Dorsal‐ und Plantarflexion im Knöchelgelenk und bei Vibrationen auf die Sehne des M. tibiales anterior untersucht. Die Ergebnisse zeigen, daß die reziproke Inhibition bei Kindern mit Cerebralparese vor und während einer willkürkürlichen Bewegung gestört ist. Diese Störungen, die durch eine Schädigung der supraspinalen Zentren bedingt sind, sind dafür verantwortlich, daß die Kinder nicht in der Lage sind, weiche koordinierte Bewegungen auszuführen.

Electromyographic and kinematic data were collected during treadmill locomotion by normal infants and infants with cerebral palsy. Locomotor patterns of the infants with cerebral palsy were similar to those of normal infants during the stage of supported locomotion, but as they matured, some of the characteristics of the infant stepping pattern, such as synchronous muscle activity with excessive muscular co‐contraction and short‐latency reflexes at foot contact, were retained. The normal plantigrade features of adult gait did not develop in these children.

Im Stadium der unterstützten Bewegung waren die Bewegungsmuster der cerebralparetischen und der gesunden Kinder ähnlich. Im späteren Entwicklungsverlauf blieben bei den Kindern mit Cerebralparese einige Charakteristika des kindlichen Laufschemas, wie synchrone Muskelaktivität mit exzessiver muskulärer Kokontraktion erhalten. Der normale Sohlengang des Erwachsenen entwickelte sich bei diesen Kindern nicht.

Los patrones locomotores de los niños con parálisis cerebral eran similares a los de los niños normales durante el estadio de locomoción sostenida. A medida que el niño con parálisis cerebral maduraba, algunas de las caracteristicas del patrón de andadura, tales como la actividad muscular sincrónica con una co‐contracción muscular excesiva, persistieron.
