Santa Catarina is a Brazilian State that has reported the lowest prevalence of human T-cell lymphotropic viruses (HTLV-1/2) in blood donors (0.04%). Although it presents ports, airports and roads that facilitate the entrance and dissemination of new infectious agents, no information exists concerning the HTLV-1/2 infections in HIV/AIDS patients. This study searched for HTLV-1/2 antibodies in plasma samples of 625 HIV/AIDS patients from the municipality of Tubarao (Southern Santa Catarina), and disclosed 1.1% of positivity (0.48% HTLV-1, 0.48% HTLV-2 and 0.16% untypeable HTLV), and a positive correlation with the male sex (OR 4.16) and intravenous drug use (OR 35.18). Although the percentage of 1.1% appears to be low, it is 27.5 times higher than the percentage detected in blood donors. Since HTLV-1 and HTLV-2 are circulating in HIV-infected individuals in Southern Santa Catarina, and these retroviruses could cause a differently impact on the HIV/AIDS outcomes, the surveillance of HTLV-1/2 is necessary, and it could support public health policies in preventing the transmission and dissemination of these viruses in this State.
Introduction: Coronavirus disease 2019 (COVID-19), a potentially fatal disease, is caused by the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). The number of cases has increased rapidly, but information on the clinical characteristics remains limited. Methods: Cohort study. We collected and analyzed epidemiological, demographic, and clinical data of critically and noncritically ill patients and compared the outcomes. Results: The mean age of hospitalized patients with COVID-19 was 54 years (standard deviation 16.9; 53.8% men), 29% required ICU admission, and 18.6% died. Conclusions: The main risk factors for ICU admission were age over 60 years, obesity, and preexisting chronic lung diseases.
Introdução: O envelhecimento da população mundial traz consigo o aumento de doenças crônicas, sem ser acompanhado de melhoria na qualidade de vida ou do enfrentamento das enfermidades. Nesse contexto, os cuidados paliativos se enquadram como um modelo interdisciplinar que visa garantir o cuidado integral ao ser humano. Em contrapartida, a busca incessante pela cura pode resultar em um sentimento de negação e derrota frente à finitude da vida. Durante o processo de graduação médica, é necessário abordar essa temática para que os futuros profissionais se sintam adequadamente preparados e seguros, proporcionando o melhor para seus pacientes. Objetivo: Avaliar o conhecimento sobre cuidados paliativos dos acadêmicos do internato do curso de graduação em Medicina da Universidade do Sul de Santa Catarina (Unisul), campus Tubarão. Método: Realizou-se um estudo transversal por meio de questionário autoaplicável com 39 questões, agrupadas em 12 questões sociodemográficas e 27 questões objetivas sobre cuidados paliativos. A coleta de dados ocorreu de março a junho de 2018. Foram utilizados os testes de qui-quadrado (X 2 ), exato de Fisher, razão de verossimilhança, Anova e teste de Tukey. O nível de significância estatística adotado foi de 5%. Resultados: Participaram do estudo 188 acadêmicos, 56,9% do sexo feminino, com faixa etária prevalente entre 21 e 25 anos. Caso fossem pacientes oncológicos, 3,2% (6) dos entrevistados prefeririam a decisão de tratamento tomada somente pelo médico, sendo que, destes, 83,3% (5) eram do sexo masculino (p = 0,04); 49,5% (93) dos entrevistados se declararam preparados para enfrentar o processo de morte e luto, sendo que, destes, 54,8% (51) eram do sexo masculino (p = 0,005); 80,3% (151) dos acadêmicos negaram ter adquirido habilidades para comunicar más notícias, sendo que, destes, 62,9% (95) eram do sexo feminino (p < 0,001). O décimo semestre do curso apresentou a melhor média de acertos em questões relacionadas à farmacologia do tratamento da dor. A maioria dos acadêmicos considerou importante incorporar conteúdos sobre cuidados paliativos ao currículo, porém 68,1% não tinham interesse em atuar nessa área. Conclusão: O ensino sobre a temática de cuidados paliativos durante a graduação de Medicina da Unisul resultou em um conhecimento adequado dos acadêmicos do internato, porém se observa dificuldade frente ao processo de morte e insegurança na abordagem de comunicações e na atitude médica. É necessário aprimorar o ensino de competências e habilidades na área, enfatizando o cuidado universal centrado no paciente e não somente na cura de doenças, bem como mobilizar esforços a fim de incentivar a autoestima dos acadêmicos. Objectives: to analyze both frequency and risk factors for seroconversion among newborns of HIV-positive mothers to HIV. Methods: a cohort study was conducted with children residing in Southern Region of Santa Catarina. Secondary data from the notification files and medical records of newborn's mothers of infected infants were used. The participants were all the newborns from 2005 to 2015 that were exposed to HIV through vertical transmission and attended a municipal health care center. Results: there were 104 cases of infant exposure to HIV. Seroconversion was confirmed in three cases, two of them died of AIDS during the study period. Breastfeeding (PR= 32.7; CI95%= 10.7-99.5; p= 0.002) and non-use of antiretroviral drugs during pregnancy (PR=18.2; CI95%= 2.0-163.0; p= 0.008) were risk factors for HIV seroconversion. Conclusions: seroconversion rates among neonates in Southern Region of Santa Catarina were similar to the national average. Seroconversion was associated with non-use of antiretroviral therapy during pregnancy and breastfeeding.
Purpose: This study evaluated the effects of 12 weeks of karate training on cardiometabolic parameters, oxidative stress, and inflammation in adolescents with overweight and obesity. Method: Seventy adolescents were randomized into 2 groups: control received nutritional and psychological interventions once a week for 12 weeks, and treatment received nutritional and psychological interventions once a week, plus 3 karate sessions per week, for 12 weeks. The main outcome measure was improvement in cardiometabolic parameters, oxidative stress, and inflammation. Results: After the intervention period, the treatment group showed a reduction in resting heart rate (77.86 [10.89]), high-density lipoprotein cholesterol (40.86 [8.31]), and triglycerides (75.18 [32.29]) and an increase in low-density lipoprotein cholesterol (95.64 [42.53]) in relation to pretraining. Regarding oxidative stress markers, there was a reduction in protein carbonylation (0.07 [0.06]) and nitric oxide (1.39 [1.11]) and an increase in superoxide dismutase (0.68 [0.31]) and glutathione (0.11 [0.08]) compared with pretraining. With respect to inflammation, adiponectin increased (14.54 [5.36]) after the intervention when compared with preintervention. Conclusion: The study concluded that the intervention may improve cardiometabolic parameters, oxidative stress, and inflammation in adolescents with overweight and obesity. Long-term effects need to be evaluated.
Hepatitis B is a serious public health problem. The state of Santa Catarina presents areas of high endemicity. The aim of this study was to describe temporal trends in detection rates of hepatitis B in the period from 2002 to 2009 in Santa Catarina and in its regions. A time series study was carried out. Crude rates were calculated and standardized by age using the direct method. Annual variation percentages were estimated by Joinpoint regression. There were two distinct and significant trends in Santa Catarina. From 2002 to 2006 a significant increase of 5.9% per year was observed. From 2006, there was a significant decrease of 6.4% per year. In this same period the southern and far-western regions had significant increases of 15.9% and 4.6% and significant decreases of 7.5% and 4.8%, respectively. Greater Florianópolis and Northeast also showed significant increases until 2006, of 15.4% and 17.4%, respectively. In the following period, non-significant decreases of 5.8% and 9.8% respectively were observed. Foz do Rio Itajaí and Planalto Serrano showed non-significant increases up to half of the studied period of 21.1% and 12.0%, respectively and after, significant decreases of 21.5% and 18.0%, respectively. Vale do Itajaí showed a significant decrease of 9.7%; Planalto Norte showed a non-significant decrease of 0.6% and Midwest a non-significant increase of 2.7% per year, in the period from 2002 to 2009.
Realizamos a leitura do estudo de Melo et al. 1 , e gostaríamos de parabenizar os autores por esse excelente artigo e fazer algumas contribuições. O artigo teve como objetivo avaliar as reações adversas a medicamentos (RAMs) nos pacientes com COVID-19 e os fatores associados ao surgimento de reações graves. Nesse contexto, considero pertinente trazer à discussão a questão da automedicação. A automedicação é um hábito no Brasil, sendo comum em aproximadamente 77% dos brasileiros, segundo dados do Conselho Federal de Farmácia (CFF) de 2019. O pânico instalado pela pandemia do COVID-19 contribuiu negativamente para o aumento desse hábito 1 .As redes sociais disponíveis por intermédio da Internet, no cenário da pandemia, são um importante canal de comunicação, contudo, mostrou-se também ser um veículo de compartilhamento de fake news. Há propagação em massa de informações contrárias às recomendações da Organização Mundial da Saúde (OMS). Dessa forma, a desinformação, bem como a negação da ciência, tem contribuído para que as pessoas se automediquem 2 .De acordo com um levantamento realizado pela consultoria IQVIA (Instituto de Pesquisa e Pósgraduação para o Mercado Farmacêutico), houve um aumento de 180% nas vendas de vitamina C. Fato ocorrido também com a hidroxicloroquina, que teve suas vendas aumentadas em cerca de 68% entre janeiro e março, depois de ser defendida como suposta cura para a COVID-19. É importante lembrar à população que mesmo medicamentos isentos de prescrição, como é o caso da vitamina D, podem causar danos quando usados sem indicação e orientação profissional 1 .A automedicação não coloca apenas a saúde individual em risco, como é o caso nas reações adversas. Há também prejuízo coletivo, visto que as pessoas que se automedicam podem ter a falsa sensação de segurança contra a infecção causada pelo novo coronavírus, tendendo a desrespeitar o isolamento social e as orientações da OMS. Além disso, o uso indiscriminado de antibiótico, como é o caso da azitromicina, contribui para o desenvolvimento da resistência bacteriana, que também gera um problema coletivo 3,4 .Em última análise, o aumento da automedicação é explicado pela insegurança e pânico trazidos pela situação de pandemia, aliado à desinformação e à negação da ciência. As consequências vão além do âmbito individual, contribuindo para o colapso do sistema de saúde como um todo 5 . Portanto, a automedicação é um tema que merece atenção da comunidade científica, principalmente no que diz respeito ao combate a notícias falsas com informação.
