Se presentan cuatro pacientes con leishmaniasis difusa, procedentes de cuatro regiones de Colombia, diagnosticados inicialmente mediante el estudio histopatológico. Dos casos tenian lesiones diseminadas, en una nifia de 10 afios y en u n hombre de 80 aííos.Los otros dos casos, hombres de 33 y 62 afios, presentaron lesiones nodulares iniciales que curaron con calor local y Glucantime. Todos tuvieron leishmaninas negativas. Se confirma que la entidad puede curar cuando no está diseminada. El calor local fue muy útil en un caso con lesión única inicial. La inoculación al hamster con macerados de biopsias de dos de los pacientes les originó prominentes "histiocitomas" en el hocico y las almohadillas plantares. En un caso no diseminado se identificó el parasito como L. m. amazonensis mediante isoenzimas. En el paciente de 80 aííos, el agente causal fue una cepa de L. mexicana. llustramos las lesiones cllnicas, enfatisamos el aspecto tlpico de la imagen histopatológica e ilustramos y discutimos la relación leishmaniamacrófago mediante imágenes de microscopla electrónica. Las caracterlsticas de la enfermedad y la literatura sobre la misma se revisa ampliamente. SINONIMOSLeishmaniasis anérgica tegumentaria difusa, leishmaniasis difusa (LD), leishmaniasis anérgica leproide, leishmaniasis cutánea pseudolepromatoide, leishmaniasis leproide, leishmaniasis nodular, leishmaniasis queloidea, leishmaniasis anérgica hansenoide. INTRODUCCIONLa LD se describió inicialmente en el Brasil en 1945 (1). La presentación e ilustración de un paciente boliviano (2) en 1948 y los trabajos realizados en Venezuela desde esta misma época (3-8), así como los estudios sobre la enfermedad en Etiopía 19-13), han contribuído a definir que sus características principales son:1. Lesiones nodulares numerosas, usualmente sin ulceración, que pueden afectar cualquier parte del tegumento, con excepción del cuero cabelludo.2. Discreta infiltración y exulceración de la parte anterior de la mucosa nasal, sin perforación del tabique pero con compromiso laringofaringeo tardío (8).
OBJETIVOS: comparar la calidad de los servicios de salud materno perinatales (SMP) de los establecimientos participantes en un programa de mejora de calidad (PMC) y compararlos con un grupo control; evaluar comparativamente los conocimientos y actitudes de usuarias de servicios de salud materna; y determinar la contribución del PMC en los niveles de calidad. MÉTODOS: fue implementado un PMC en 74 establecimientos de salud seleccionados para mejorar la calidad. Se desarrolló un cuasiexperimento controlado en 74 hospitales, centros de salud y establecimientos periféricos. El análisis de la calidad de los SMP se realizó mediante comparación de promedios de escalas centesimales; los conocimientos y actitudes de las usuarias mediante análisis bivariado; y la predicción del nivel de calidad a través de análisis multivariado. RESULTADOS: al final de la intervención, la puntuación de los establecimientos con PMC tuvo un promedio de 61,8 puntos y de 37,5 en el grupo de comparación, (p <0,001). El promedio de conocimientos de signos de alarma obstétricos fue mayor en las usuarias PMC (3,6 signos/entrevistada) versus 2,5 en usuarias sin PMC, (p <0,05). La intención de parto institucional fue dos veces mayor en las usuarias de los servicios con PMC, (p = 0,038). Los predictores significativos de calidad fueron la mejora continua de la calidad y la disponibilidad de profesionales de salud. CONCLUSIONES: el nivel de calidad es mayor en los servicios que desarrollaron PMC. Las usuarias de tales servicios tienen más conocimientos de los signos de alarma obstétricos y mejor actitud hacia el parto institucional. El PMC ha contribuido para tales logros.
Los inventarios entomológicos de insectos de la subfamilia Phlebotominae, relacionados con la transmisión de patógenos como Leishmania spp., y Bartonella bacilliformis, son de gran importancia debido a que su presencia simboliza una potencial reaparición de infección de estas dos enfermedades tropicales en áreas con antecedentes epidemiológicos. Entre los años 2011 y 2012 se realizó la búsqueda de flebotomíneos en los municipios de Sandoná, Consacá, La Unión, Linares, Guaitarilla y Samaniego, en el departamento de Nariño, Colombia. Se colectaron 710 ejemplares del género Lutzomyia, mediante el uso de trampas de luz tipo CDC, Shannon y búsqueda activa en sitios de reposo. La especie Lutzomyia columbiana fue colectada en mayor abundancia (n= 699; 98,45%) en los valles interandinos de Nariño, mientras Lu. rosabali y Lu. osornoi tienen una menor representación. Los resultados reafirman la circulación y presencia de Lu.Columbiana, potencialmente asociada con la transmisión histórica de bartonellosis y la esporádica aparición de casos de leishmaniasis en Nariño. Estudios de infección natural son requeridos para avanzar en profundidad sobre la importancia vectorial de Lu. columbiana.
La constitucionalización de las distintas áreas del Derecho es un fenómeno cada vez más arraigado en nuestra cultura y contexto jurídicos. Tal vez la forma más evidente en que se haya manifestado sea el creciente protagonismo del Tribunal Constitucional (TC), supremo intérprete de la Constitución, en la significación y resignificación de conceptos, derechos y principios jurídicos que, a través de sus sentencias, han afectado desde el derecho tributario hasta los derechos humanos. Sea que esto se valore positiva o negativamente, su relevancia para comprender el derecho y sus efectos en la actualidad es innegable.
