<p><strong>Introducción: </strong>La presión arterial elevada es una categoría de presión arterial, poco estudiado en adultos jóvenes y está asociado a otros factores de riesgo cardiovascular.<strong> Objetivos:</strong> Estimar la prevalencia de presión arterial elevada y otros factores de riesgo cardiovascular en universitarios de la Universidad Nacional de Asunción-Paraguay.</p><p><strong>Material y métodos: </strong>Estudio transversal en universitarios de primer curso de las carreras relacionadas a la salud de la Universidad Nacional de Asunción. Se consideró presión arterial elevada a una presión arterial sistólica de 120-129 mmHg y/o presión arterial diastólica de ?80 mmHg. Se midieron las variables antropométricas, lipídicas, metabólicas y se determinó el hábito tabáquico y de alcohol.</p><p><strong>Resultados: </strong>Se incluyeron en el análisis 284 universitarios (edad media [±DE] 19,4 ±2,1 años; 73,2% fueron mujeres). La prevalencia global de la presión arterial elevada fue del 25,5%; (IC del 95%: 20,5-30,5%) (hombres 40%; mujeres 29%). La presión arterial elevada se asoció con la circunferencia abdominal aumentada (OR: 1,031; IC del 95%: 1,005-1,058; p= 0,024); el c-HDL baja (OR: 1,355; IC del 95%: 1,054-1,743; p=0,018); la obesidad, OR: 2,124; IC del 95%: 1,334-3,941; p=0,007) y la DM2 (OR: 4,431; IC del 95%: 3,642-7,963; p=<0,001). La prevalencia de otros factores de riesgo cardiovascular más frecuente fueron el consumo de alcohol, el sobrepeso y la obesidad. Más del 70% de los universitarios tienen uno o más factor de riesgo cardiovascular.</p><p><strong>Conclusiones:</strong> alta prevalencia de la presión arterial elevada y está asociado con otros factores de riesgo cardiovascular.</p>
Introducción: el sobrepeso y la obesidad son factores de riesgo cardiovascular y están asociados a otros factores de riesgo cardiovascular. Objetivo: determinar la frecuencia de sobrepeso, obesidad y factores asociados en pacientes de primera consulta del consultorio de un Hospital Universitario de Paraguay. Materiales y métodos: se realizó un estudio observacional, retrospectivo, con componente analítico. Se estudió a pacientes entre 18 y 90 años, seleccionados por muestreo aleatorio simple. En el periodo de estudio se registraron 5146 historias clínicas en primera consulta y 4324 fueron considerados para la aleatorización. Se estudiaron las variables demográficas, antropométricas, bioquímicas, presencia de sobrepeso, obesidad, HTA, diabetes mellitus tipo 2, consumo de alcohol y tabaco. Resultados: se analizaron las historias clínicas de 515 pacientes con edad media de 49,68±15 años; 50,27±17 años en hombres y 49,38±15 años en mujeres, 66% eran mujeres y 49,5% eran solteros. La frecuencia de sobrepeso fue de 30,5% (IC 95%: 30,1-30,9%) sin diferencia estadística significativa entre mujeres y hombres (29,1% versus 33,1%) y la frecuencia de obesidad fue de 44,1% (IC 95%: 43,7-44,5%) con diferencia estadística significativa entre mujeres y hombres (50% versus 32,6%, p=0,002). La obesidad se asoció con la DM2 (OR: 4,62; IC 95%: 2,11-6,22), con la concentración de c-LDL ≥130 mg/dl (OR: 2,48; IC 95%: 2,26-4,46) y con la HTA (OR: 2,62; IC 95%: 1,62-4,08). Discusión: las altas frecuencias de sobrepeso y obesidad observadas en esta población coinciden con los datos reportados a nivel nacional e internacional y estuvo asociada significativamente a otros factores de riesgo cardiovascular.
Introducción: las infecciones urinarias constituyen una de las más frecuentes en el ser humano. Objetivos: describir los hallazgos semiológicos, laboratoriales y microbiológicos de las infecciones urinarias, a través de la presentación de una serie de casos. Material y métodos: estudio retrospectivo, observacional, de corte transversal de las historias clínicas y los urocultivos y análisis del sedimento urinario de pacientes adultos, de ambos sexos, que acudieron a la consulta ambulatoria y/o que permanecieron internados en la Primera Catedra de Clínica Médica del Hospital de Clínicas (San Lorenzo, Paraguay) con el diagnóstico de infección urinaria, en el periodo comprendido entre febrero de 2015 y febrero de 2017. Resultados: en los casos de pielonefritis aguda los síntomas más frecuentes fueron la fiebre (80%) y el dolor lumbar (46%). En los pacientes con cistitis aguda predominó la disuria (93%). En ambos grupos hubo antecedente de antibioticoterapia previa la mayoría de las veces. La diabetes mellitus fue la comorbilidad más prevalente en la pielonefritis aguda (53%). El sedimento urinario presentaba leucocituria menor a 100/campo (66%) y menor a 50/campo en las cistitis agudas (66%), siendo el urocultivo positivo en el 67% de los casos con prevalencia de (60%). En la cistitis Escherichia coli aguda la mayoría tuvo urocultivo negativo (87%). Conclusión: al momento del manejo diagnostico-terapéutico de ambas entidades se destaca el valor de la anamnesis y examen físico en el caso de las cistitis. En las pielonefritis aguda, a más de los datos de la anamnesis y la exploración semiológica, los estudios microbiológiocos y otros juegan un papel importante en el diagnóstico.
