BackgroundBurnout syndrome can be defined as long-term work stress resulting from the interaction between constant emotional pressure associated with intense interpersonal involvement for long periods of time and personal characteristics. We investigated the prevalence/propensity of Burnout syndrome in clinical nurses, and the factors related to Burnout syndrome-associated such as socio-demographic characteristics, work load, social and family life, leisure activities, extra work activities, physical activities, and work-related health problems.MethodWe conducted a cross-sectional, quantitative, prospective epidemiological study with 188 surgical clinic nurses. We used the Maslach Burnout Inventory (MBI), which is a socio-demographic questionnaire and the most widely used instrument to assess Burnout syndrome (three basic dimensions: emotional exhaustion, despersonalization and professional underachievement). The socio-demographic profile questionnaire wascomposed of questions regarding identification, training, time at work, work characteristics and personal circumstances.ResultsThe prevalence of Burnout syndrome was higher (10.1%) and 55, 4% of subjects had a propensity to develop this syndrome. The analysis of the socio-demographic profile of the nurse sample studied showed that most nurses were childless married women, over 35 years of age, working the day shift for 36 hours weekly on average, with 2-6 years of post-graduation experience, and without extra employments. Factors such as marital status, work load, emotion and work related stress aggravated the onset of the syndrome.ConclusionThe prevalence and propensity of Burnout syndrome were high. Some factors identified can be useful for the adoption of preventive actions in order to decrease the prevalence of the clinical nurses Burnout syndrome.
Anomalias do calibre e/ou do comprimento da artéria basilar têm sido infrequentemente registradas na literatura, não obstante a grande variedade de manifestações neurológicas a elas atribuídas. A terminologia adotada para designar tais anomalias é diversa, não existindo expressão alguma que as defina de maneira perfeita. Dandy (cit. por Adams e col. *) descreveu-as como artérias em S; o termo ectasia tem sido utilizado nos casos em que ocorre hidrocéfalo por compressão do terceiro venttculo 4 , 6 , 7; outras expressões também encontradas na literatura são aneurisma e dilataçüo fusiforme da basilar n. O termo megadolicobasilar foi pela primeira vez empregado por Boeri e PasseriniB, visando a descrever artéria basilar anormalmente dilatada, alongada e sinuosa, sendo desde então adotado pela maioria dos autores. Segundo alguns, esta anomalia está associada com freqüência a alterações da mesma natureza das artérias carótidas ou de outros vasos 5 , 14 .O presente trabalho tem por finalidade a discussão de 5 casos de megadolicobasilar, que tivemos ocasião de diagnosticar em nosso Serviço nos últimos três anos, no qual analisaremos alguns dos aspectos clínicos relacionados a estas anomalias vasculares, salientando a importância do estudo angiográfico encefá-lico, único exame capaz de estabelecer seu diagnóstico, que se presta à confusão com várias outras afecções, inclusive de caráter expansivo. OBSERVAÇÕESCaso 1 -A.D.E., sexo masculino, cor branca. 60 anos de idade, registro 41.936. Sensação de tontura, acompanhada de queda da pálpebra direita e. três meses depois, perda súbita de força nos membros esquerdos. Paciente com hipertensão arterial de longa duração, não tomando drogas hipotensoras de maneira constante.Exame clinico--neurológico -Pressão arterial 220 χ 120 mm de Hg; pulso regular e rítmico. 90 batimentos/min. Consciente e orientado: paralisia completa do nervo motor oculacomum direito; hemiplegia esquerda total e proporcionada: sinais piramidais de lib?-
A grande frequência do acidente vascular cerebral (AVC), assim como sua alta mortalidade e morbilidade, fazem com que esta sindrome se destaque entre as afecções neurológicas.Nos últimos anos os neurocirurgiões vêm, cada vez mais, se interessando pelo tratamento cirúrgico de doentes acometidos de acidente vascular cerebral hemorrágico (AVCH), relacionado na maioria das vezes à doença vascular hiperten¬ siva. Este interesse se justifica em face do prognóstico sombrio na quase totalidade dos casos e pela falta de medidas de natureza conservadora que possam melhorá-lo.A indicação cada vez mais frequente da angiografia cerebral tem contribuído notavelmente para o esclarecimento fisiopatogênico e etiológico desses quadros, demonstrando ser de validade muito relativa o diagnóstico de AVC baseado simplesmente nos dados de anamnese, exame clínico-neurológico e caracterís-ticas do líquido cefalorraqueano. De fato, afecções hemorrágicas, como o hematoma intraparenquimatoso (HIP), podem, por estes critérios, ser rotulados como acidente vascular cerebral isquêmico (AVCI). Além disso, a elevada incidência de hemorragia intracraniana causada por aneurismas ou malformações artério--venosas e, menos frequentemente, por várias outras afecções, tais como neopla¬ plasias, arterites, flebites e tromboflebites, tornam quase obrigatório o exame angiográfico cerebral em casos dessa natureza.O presente trabalho, partindo de casuística relativamente numerosa, visa a demonstrar a importância do estudo angiográfico cerebral de doentes acometidos de AVC, no sentido de possibilitar o diagnóstico de afecções de tratamento cirúr-gico, destacando os resultados obtidos em 11 casos de HIP.
In mid-1997, the new board of directors of SBC/ FUNCOR established the Committee of Medical Residency and Training to assist with the evaluation of training in cardiology. Besides providing subsidies and criteria for accreditation of the courses, this committee was encouraged to address the problem of cardiologists' training in our country. During six months of work, the committee concluded that, despite the apparent dichotomy between the official programs of Medical Residency (MR) approved by the Brazilian Department of Education and Culture and those endorsed by SBC/FUNCOR, the establishment of a link between the two institutions was a priority. The following proposal resulted from this recognition of the need for greater cooperation and was developed from contributions of participants from the main cardiology MR program in São Paulo state, during the seminar "Medical Residency and Brazilian Society of Cardiology", held on February 7, 1998 at the SBC/FUNCOR headquarters. Subsequently with the approval of the board of directors of SBC, this project was presented to the National Committee of Medical Residency (NCMR) in April 1998, and was well received by its members. Both groups recognized that a proposal was needed. In conjunction with NCMR, a national seminar would be held to carefully examine and define the main consensual proposals. We present here the proposal in its preliminary form in the hopes of inspiring discussion of this significant topic affecting the evolution of cardiological education in our country.
