RESUMO:Avaliou-se a qualidade de vida relacionada à saúde e sobrecarga de trabalho de 126 cuidadores de idosos atendidos em dois ambulatórios geriátricos de um hospital universitário, bem como a associação entre sobrecarga e variáveis sociodemográficas e clínicas. Estudo exploratório-descritivo, de corte transversal e abordagem quantitativa. Foram utilizados um instrumento de caracterização sociodemográfica e clínica, o Zarit Burden Interview, para avaliação da sobrecarga percebida, e o Medical Outcomes Study , para avaliação da qualidade de vida relacionada à saúde. Realizaram-se análises descritivas, de comparação, correlação e multivariadas dos dados. Houve diferença significativa entre a sobrecarga de cuidadores dos dois ambulatórios; todos os aspectos sociodemográficos e de cuidado estiveram correlacionados à piora de pelo menos uma dimensão do SF-36 e doenças frequentes nos cuidadores estiveram relacionadas à piora de sua qualidade de vida relacionada à saúde. QUALITY OF LIFE AND WORK OVERLOAD AMONG ELDERLY OUTPATIENT CAREGIVERS ABSTRACT:We evaluated the quality of life related to health and work overloads among 126 caregivers to patients at two geriatric outpatient clinics of a university hospital, as well as the association between overloads and demographic and clinical variables, using an exploratory, descriptive, cross sectional and quantitative approach. The Zarit Burden Interview, a socio-demographic and clinical characterization instrument, was used to assess perceived workloads and the Medical Outcomes Study Short-Form Health Survey (SF-36) was used to assess quality of life related of health. Descriptive, comparative, correlative, and multivariate data analyses were carried out. There was significant difference between two outpatient caregiver workloads; all socio-demographic aspects and variables related to care were associated to worsening at least one dimension of the SF-36; frequent illnesses among caregivers were related to a worsening of their quality of life related to health, demonstrating the wear experienced by caregivers to the elderly in these health care units. Objective. To assess immediate and two-year treatment results in smokers seen at the outpatient clinic of a university hospital. MethOds. One hundred seventy one smokers were assessed for inclusion in a Treatment Group at the Outpatient Clinic for Psychoactive Substance Use at Hospital das Clínicas da Universidade Estadual de Campinas. Sociodemographic variables, history of smoking, presence of comorbidities and psychiatric symptoms, immediate and long-term outcomes (median: 25 months) were evaluated by telephone contact. Frequency analyses and multiple logistic regression analysis were carried out, at a significance level of 5%. Results. Most patients were female (73.4%), married (48%), had elementary education (74.6%), and were working (57%); 65.2% started to smoke before the age of 15; 63.8% had been smoking for more than 30 years; 76% had already tried to stop smoking; 46.2% presented severe dependence; 72.1% presented medical comorbidities; and 36% presented psychiatric symptoms. The service was sought spontaneously by 51% of the patients, who were mainly concerned with their own health. Of the total number of patients, 79.1% stopped smoking during treatment; after 25 months, 62% remained abstinent. Presence of psychiatric symptoms was the variable associated with failure in smoking cessation. Variables related to relapse were psychiatric symptoms and low attendance to motivation sessions. There was an association between presence of clinical comorbidities, years of smoking, and professional inactivity. cOnclusiOn. This study reinforces the importance of understanding the profile of subjects attending smoking treatment groups in order to evaluate the strategies employed and the adequacy of treatment regimens for smokers, with the aims of improving smoking cessation rates and reducing relapse rates. The life expectancy of a smoker is 25% lower than that of a non-smoker. When compared to people who continue smoking, those who stop smoking before the age of 50 show a 50% reduction in the risk of death for tobacco-related diseases. 4 Although 70% of smokers state they would like to stop smoking, less than 10% reach that objective on their own account. A total of 3,419 smokers received minimum individual intervention and 982 had group intervention. Of all, 9.2% quit smoking. The main difficulties for smoking cessation reported by health providers were: low patient compliance (11%); lack of medications (38%); and lack of health unit infrastructure/support (29%). The training allowed to reaching out a larger population through smoking cessation approaches at health units.
RESUMOEste trabalho tem por objetivo relatar a experiência da implantação de um grupo de apoio a cuidadores familiares de idosos com transtornos mentais em um ambulatório de psicogeriatria. Esse grupo foi elaborado a partir das dificuldades dos cuidadores no cuidado com os idosos (realização de atividades físicas, sono e repouso, comunicação, alimentação, higiene corporal e higiene oral), incluindo temas que pudessem melhorar a qualidade de vida desses cuidadores e facilitar-lhes o cuidado com os idosos. A cada encontro o tema foi abordado com o auxílio de materiais audiovisuais e posteriormente foi entregue a cada participante um folheto explicativo contendo o resumo do conteúdo discutido. Durante as apresentações eram fomentadas discussões e realizadas atividades práticas relacionadas ao tema abordado. As atividades realizadas foram avaliadas pelos cuidadores como de importância significativa para seu cotidiano, destarte este grupo pode ser um exemplo de estratégia para instrumentalizar o cuidador familiar do idoso com Alzheimer e depressão no tocante às informações e técnicas necessárias para o cuidado.Palavras-chave: Enfermagem. The aim of this report was to verify in-patients and their relative's opinion and knowledge of the use of electroconvulsive therapy. By means of a quantitative analysis, the use of this treatment was shown to be accepted despite the individuals knew little about it. It was observed that the knowledge of this treatment had influence over individual's opinion about it. Thus nurses play an important role on the instruction of these individuals regarding this treatment.
