RELATO DE CASO: Paciente, M. S. C., 60 anos, gênero feminino, atendida em hospital particular da cidade de Jundiaí -SP, com queixa de dor abdominal em região de abdome superior, associado a náuseas e vômitos. O exame físico mostrou abdome doloroso à palpação em epigástrio e hipocôndrio direito. Nas análises laboratoriais, no período da internação (31/03/2016), a amilase = 1354 U/dl, gama GT = 820 U/l, TGO = 140 U/l, TGP = 251U/l. Na TC de abdome total, evidenciou se colecistopatia crônica calculosa e colelitíase com dilatação da árvore biliar intra e extra-hepática. Com a hipótese diagnóstica de pancreatite aguda secundária a coledocolitíase. Optado pela CPRE sendo realizado papilotomia e varredura da via biliar com extração de cálculos e grumos. A paciente cursou com instabilização hemodinâmica no terceiro pós-operatório sendo realizada nova TC de abdome total evidenciando grande área hipoecóica irregular ocupando quase todo o lobo direito hepático com pequena quantidade de gás de permeio, presença de líquido livre intra-abdominal, litíase vesicular, calcificação em cabeça pancreática em possível topografia do colédoco. Optado por realização de laparoscopia que evidenciou presença de sangue em grande quantidade livre em cavidade abdominal, sangramento hepático ativo e hematoma em lobo direito roto, sendo convertida para laparotomia. Realizado colecistectomia, hemostasia com eletro cautério e colocação de surgicel em leito hepático, não havendo sangramento ativo do hematoma hepático ao final do procedimento cirúrgico. Paciente evoluiu estável no pós-operatório recebendo alta no 17º dia de internação. DISCUSSÃO: O hematoma subcapsular após a CPRE é uma complicação rara, com poucos casos reportados na literatura médica. Pode ser explicado pela punção acidental do fio guia na árvore biliar intra hepática com subsequente ruptura dos vasos de pequeno calibre, passagem de ar para o canal biliar e hematoma. Comumente, adota-se o tratamento conservador, exceto nos casos em que há instabilidade hemodinâmica como no caso acima onde se fez necessario abordagem cirurgica. CONCLUSÃO: Pelo fato de a CPRE ser um exame complementar que cursa com pequena incidência de complicações, não é rotineiro a solicitação de exames de imagem complementares, sendo os mesmo necessários em alguns casos diante da persistência de quadros de dor abdominal ou descompensações clinicas importantes.
