A fim de descrever a clientela atendida nos Centros de Atenção Psicossocial (CAPS) da Cidade de Santos foi realizado um censo de pacientes atendidos nos CAPS no período 1º de junho de 2001 a 30 de junho de 2001. O primeiro contato do paciente com o serviço foi registrado e seus dados foram preenchidos em ficha padronizada utilizando o prontuário médico como fonte de informação. Foram atendidos 1.673 usuários, com idade média de 46 anos, mulheres (59,0%), morando com a família (78,0%) e tempo médio de três anos em tratamento no serviço. Atenderam 38,0% de esquizofrênicos, esquizotípicos e delirantes, 31,0% com transtorno de humor e 17,0% de neuróticos, nas modalidades de ambulatório médico-psiquiátrico (81,0%), individual (23,0%), grupal (13,0%) e reabilitação psicossocial (4,3%). Os CAPS de Santos atendem todas as categorias de transtornos mentais, sendo os mais freqüentes aqueles considerados mais graves, o que é esperado para este tipo de serviço. O modelo de tratamento oferecido tem características ambulatorial e médico-psiquiátrica.
A síndrome de Ekbom, conhecida também como delírio de infestação parasitária, acarofobia, "delusional parasitosis", parasitose psicogênica, é doença de rara ocorrência. Caracteriza-se pela firme convicção dos pacientes de que estão infectados por vermes que saem pela pele, em geral do couro cabeludo ou até mesmo da boca, dos olhos e da região genital. A maioria dos pacientes é idosa e do sexo feminino, freqüentemente com isolamento social. Alguns casos estão associados a doenças orgânicas como hipertireoidismo, diabetes, lesões corticais, intoxicações medicamentosas. A comorbidade com torcicolo espasmódico, até onde vai nosso conhecimento, é um achado inédito na literatura. Relatamos caso de uma senhora de 72 anos de idade que se apresentou com torcicolo espasmódico associado ao quadro psiquiátrico.

É conhecido mais comumente como dismorfofobia. Embora TDC tenha sido descrito na literatura ao longo de vários anos, é surpreendente o pouco conhecimento a respeito dessa condição.TDC em geral produz morbidade significativa. Muitos pacientes evoluem com perdas ocupacionais e sociais importantes e muitos permanecem isolados dentro de casa durante anos. 1 A proximidade entre TDC e transtorno obsessivo-compulsivo (TOC) tem sido referida em alguns trabalhos. [1][2][3][4][5] Há dados na literatura sugerindo que o TDC é relativamente comum em pacientes com transtorno depressivo, 5 fobia social, 6 TOC, [7][8][9] e naqueles que procuram tratamento dermatológico e cirurgia plástica. 1,5,8 A maioria dos pacientes com TDC busca tratamento não-psiquiátrico, como cirúrgico ou dermatológico, que, no caso dessa patologia, estão fadados ao insucesso. 5Dados oriundos de estudos abertos sugerem que os inibidores da recaptação da serotonina são geralmente e, talvez, preferencialmente, efetivos no tratamento do TDC. 9,10 No presente trabalho, os autores descrevem uma paciente com TDC que apresentou remissão dos sintomas pelo tratamento com venlafaxina e discutem a possível associação dessa patologia com o TOC. Apresentação do casoC., 39 anos, sexo feminino, solteira, de nível universitário.Refere que há dez anos apresentou, sem motivo aparente, vários episódios seqüenciais nos quais tinha tontura, opressão no peito, insônia, medo de ficar sozinha, insegurança para dirigir e sensação de que lhe faltava o ar. Nessa época, foi medicada com clomipramina obtendo melhora dos sintomas.Havia cerca de dois meses, ao achar que a musculatura da face se tornara flácida, iniciou uma seqüência de exercícios físicos para reforçá-la. Colocava na boca um dispositivo metálico, como se fosse uma mola que distendia suas bochechas, e fazia diariamente uma série de exercícios durante mais ou menos 15 minutos.
