Objectif Cette étude avait pour but d’examiner rétrospectivement les données de patients qui avaient suivi un traitement d’orthokératologie afin de quantifier l’accentuation de la courbure de la couronne périphérique de la cornée par topographie en mode tangentiel (CT-CP) et la modification de la puissance de réfraction totale (PRT). Méthodes Les cartes ont été examinées et les données de Pentacam ont été recueillies avant le traitement d’orthokératologie et après la dernière visite. Les variables enregistrées comprenaient l’aplatissement central (TC-C), l’eccentricité cornéenne, les courbures de la couronne périphérique en mode tangentiel (CT-CP), la puissance de réfraction totale (PRT) de la cornée, la Rx initiale (sphère) et la courbure cornéenne initiale (kératométrie plate). Les patients ont ensuite été subdivisés en groupes de myopie forte et faible à des fins de comparaison. Tous les participants avaient reçu des lentilles Paragon CRT bien ajustées. Les données ont été analysées pour déterminer les effets des lentilles sur les courbes tangentielles (CT) et la PRT. Résultats Au total, 40 patients (80 yeux), âgés de 13,95 ± 6,80 ans, 34 sujets de sexe masculin et 46 sujets de sexe féminin, ont reçu des lentilles Paragon CRT ajustées à leurs yeux. La sphère moyenne était de -4,23 ± 0,90 D pour le groupe à forte myopie et de -1,89 ± 0,62 D pour le groupe à faible myopie. Les changements de CT et de PRT par rapport au niveau de référence étaient significatifs (p < 0,0001 pour les deux). La différence entre la superficie de CT-C et la superficie maximale de CT-CP était d’environ +/- 4,00 D au méridien horizontal et d’environ +/- 3,00 D au méridien vertical. Le changement de CT-CP par rapport au niveau de référence était d’environ +/- 2,00 D pour les deux méridiens. Il y a eu un changement semblable de la PRT : la distance entre le centre et la portion périphérique était d’environ +/- 2,50 D. Pour les valeurs CT-C, PRT-C, CT-CP et PRT-CP, il n’y avait pas de différence significative entre les deux groupes, dans l’ensemble (p = 0,541 [TC] et p = 0,321 [PRT]). Conclusion Ces résultats devraient fournir de précieux renseignements sur les changements topographiques et réfractaires qui surviennent chez les patients qui ont suivi un traitement d’orthokératologie.
Purpose: The purpose of this study was to retrospectively examine data from patients who had undergone orthokeratology (OK) to quantify the amount of mid-perpheral steeping in tangential curvature (MP-TC) and total refractive power (TRP) changes. Methods: Charts were reviewed from participants and data from the Pentacam instrument were collected prior to OK and after the final visit. Variables included central flattening (C-TC), the e-value, the mid-peripheral tangential curvatures (MP-TC), total corneal refractive power (TRP), the initial Rx (sphere) and initial corneal curvature (flat K). The group was further subdivided into high and low myopia for comparison. All participants were fitted with the Paragon CRT lenses. Data analysis was conducted to analyse the effects of lenses on TC and TRP. Results: A total of 40 patients (80 eyes) age13.95±6.80, 34 M and 46 F, were successfully fitted with CRT lenses. The average sphere was -4.23±0.90D for Group 1 and -1.89±0.62D for Group 2 The changes in TC and TRP from baseline were significant (both P<0.0001). The difference between the amount of C-TC and the maximum area of MPTC was ≈4.00D in the horizontal meridian and ≈3.00 in the veritcal meridian. The amount of MP-TC change from baseline was ≈2.00D in both meridians. There was a similar change in TRP: the distance from the centre to mid-periphery ≈2.50D. For C-TC, C-TRP, MP-TC, MP-TRP there was no significant difference between the 2 groups, overall (P=0.541 (TC) and P=0.321(TRP)). Conclusion: Results from this study should provide valuable insights into the topographic and refractive changes occurring with orthokeratology.
