En diciembre de 2019 se reportó a la oficina de la Organización Mundial de la Salud (OMS) los primeros casos de neumonía de etiología desconocida en Wuhan, Provincia Hubei, China. Posteriormente, se identificó como agente causal un nuevo virus de la familia coronavirus (CoV). La enfermedad fue nombrada por la OMS como enfermedad coronavirus 2019 (COVID-19, por su sigla en inglés). Por su rápida expansión global y alta infectividad, se declaró la enfermedad como una emergencia sanitaria mundial. La mortalidad se ha visto que varía dependiendo de la región. Múltiples países, incluyendo Colombia, han logrado desarrollar medidas de contención fundamentadas en el aislamiento social con un gran impacto socioeconómico secundario. El siguiente artículo describe el cuadro clínico del virus, su comportamiento epidemiológico en Colombia y qué medidas públicas se han adoptado en Colombia y otros países latinoamericanos frente a la pandemia.
Introducción: Los quistes de retención mucoso de los senos paranasales son un hallazgo imagenológico incidental muy frecuente. Usualmente comprometen el seno maxilar. Metodología: Se realizó una amplia búsqueda de la literatura, sobre quistes de retención mucoso nasosinusales, en enero de 2021 en múltiples bases de datos.Se seleccionaron 21 artículos sobre el tema para la realización de la revisión. Discusión: En la mayoría de los casos el tratamiento de los quistes de retención mucoso nasosinusales debe ser expectante debido a su tendencia a la involución y cursoasintomático. Sin embargo, en la práctica clínica se observa con frecuencia un manejoquirúrgico sin una indicación clara. Conclusiones: Las indicaciones quirúrgicas de los quistes de retención mucoso son: obstrucción del ostium del seno paranasal, compromiso del complejo ostiomeatal o compromiso del nervio infraorbitario consíntomas secundarios. Es importante que el cirujano reconozca la naturaleza y comportamiento de esta patología para evitar procedimientos quirúrgicos innecesarios.
El quiste dermoide es una lesión congénita poco frecuente, secundario a regresión incompleta de tejido ectodérmico. Su incidencia es de 1/20,000 – 40,000 nacidos vivos. Debe realizarse una resección quirúrgica oportuna para prevenir compromiso intracraneal y/o complicaciones. El estudio imagenológico determina su extensión. Se han recomendado diferentes enfoques quirúrgicos, todos deben cumplir con los 4 principios de Pollock. El siguiente estudio presenta el caso de un paciente pediátrico, 9 meses de edad, con un quiste dermoide en base del cráneo asociado a un trayecto fistuloso a punta nasal. La resonancia magnética nuclear contrastada evidenció una masa quística en región anterior a la crista galli comunicada con tracto fistuloso a la punta nasal. Fue llevado a cirugía para resección del quiste mediante rinoplastia externa combinada con abordaje endoscópico transnasal con resultados funcionales y estéticos satisfactorios, sin recurrencia. Se evidenció una fístula de líquido cefaloraquídeo intraoperatoria corregida en el mismo tiempo quirúrgico.
