BackgroundTreatment of patients with newly diagnosed HPV‐associated oropharyngeal squamous cell carcinoma (OPSCC) with neoadjuvant chemotherapy (NAC) results in a high rate of 5‐year recurrence free survival with few patients requiring adjuvant treatment. We hypothesized that NAC enhances primary tumor HPV‐specific T cell responses.MethodsHPV‐specific responses in tumor infiltrating lymphocytes (TILs) before and after NAC were determined using autologous co‐culture assays.ResultsGreater HPV16‐specific TIL responses, sometimes polyclonal, were observed after NAC compared to before in 8 of 10 patients (80%) with PCR‐verified HPV16‐positive tumors. A significant association was observed between net‐negative change in HPV‐specific TIL response and disease relapse (p = 0.04, Mann–Whitney test), whereas pathologic complete response at time of surgery did not correlate with recurrence.ConclusionsNAC induces HPV‐specific tumor T cell responses in patients with newly diagnosed HPV‐associated OPSCC; whereas lack of an increase following NAC may associate with risk of relapse.
En la denominación general de osteodistrofia renal se incluyen las complejas anomalías óseas asociadas con la enfermedad renal. Aunque los síntomas clínicos de enfermedad ósea son tardíos e inespecíficos, estudios recientes indican que las alteraciones esqueléticas comienzan en etapas tempranas de la insuficiencia renal crónica (IRC) (1-3). Si bien, el diagnóstico del tipo de osteodistrofia requiere una biopsia ósea, se ha tratado de hallar distintos marcadores bioquímicos capaces de valorar pre-cozmente, de una manera no invasiva, el alcance de las lesiones. De los marcadores disponibles, la PTH sérica junto con la fosfatasa alcalina ósea son los que mayor información aportan en el estudio de la función osteoblástica (4-8). Otra causa de morbilidad en el enfermo renal son las alteraciones gastrointestinales. Los síntomas digestivos son frecuentes y precoces en la uremia y se sabe que la enfermedad ulcerosa péptica se presenta hasta en el 25% de los sujetos urémicos, aunque se desconoce si esta alta incidencia está relacionada con el hiperparatiroidismo secundario (9-11).
The records of 13 adult patients operated on for a choledochal cyst were reviewed. The cyst was saccular in 6 patients and fusiform in 7. A fusiform enlargement of the main intrahepatic ducts was associated in 5 cases. In 5 patients the pancreatico-biliary ductal union occurred at a high position, forming an abnormally long common terminal duct. 11 patients underwent an excision of the cyst with Roux-en-Y hepaticojejunostomy. Microscopically, the wall of the cyst consisted of fibrous tissue usually devoid of epithelium and muscle layer. Recovery was uneventfull with good late results in most patients. A precise knowledge of its radiological features is necessary to recognize this anomaly and manage it adequately.
