Revista Brasileira de Odontologia 150Manejo terapêutico e preventivo da osteorradionecrose: revisão integrativa da literatura Therapeutic and preventative management of osteoradionecrosis: integrative literature review IntroduçãoA s neoplasias malignas que acometem a região de cabeça e pescoço são compreendidas pelos seguintes sítios anatômicos: cavidade oral, faringe, cavidade nasal e seios paranasais, laringe e glândulas salivares. Em 2012, foram estimados cerca de 300 mil novos casos dessas neoplasias no mundo. No Brasil, em 2016, foram estimados 15.490 casos novos de câncer oral.¹ O tratamento antineoplásico desses tumores consiste nas modalidades de cirurgia, radioterapia e/ ou quimioterapia. A modalidade de tratamento escolhida ou suas associações irão depender da localização e do tipo do tumor, do estadiamento e da condição de saúde do indivíduo.2 A radioterapia (RT) é uma modalidade de tratamento eficaz, loco-regional, que provoca alterações visíveis nos tecidos sadios adjacentes às áreas irradiadase que tem um papel fundamental no tratamento do câncer de cabeça e pescoço. As doses de radiação empregadas para o tratamento desses tumores podem chegar a 7.000 cGy, podendo levar a alterações nos tecidos adjacentes, que são classificadas como injúrias agudas e tardias. 2Dentre as injúrias agudas produzidas pela RT destacam-se: mucosite, disgeusia, xerostomia e descamação da pele. E as injúrias tardias que são mais prováveis de acontecer são: ulceração da mucosa, lesões vasculares, atrofia dos tecidos, perda ou mudança do paladar, fibrose, edema, necrose dos tecidos moles, perda de dentes, diminuição do fluxo salivar, osteorradionecrose (ORN) e cárie de radiação. 3Dentre as complicações supracitadas a ORN é uma das mais graves sendo definida como uma exposição óssea através de uma abertura na pele ou mucosa, persistindo como uma ferida que não se cura por três meses ou mais.4,5 Na literatura, existe uma oscilação da frequência da ORN de 1% a 40% de casos, a mesma acomete sete vezes mais a mandíbula quando comparado com o osso maxilar, isso ocorre, devido à alta densidade óssea e menor vascularização desse osso. 6 A prevenção da ORN é fundamental, sendo indicado, antes da RT, exodontia ou restauração dos dentes comprometidos, com o intuito de eliminar focos de infecção. 7O tratamento da ORN, ainda, é um desafio para o clínico. Atualmente, parece consenso que a ORN deve ser manipulada, inicialmente, de maneira conservadora, por debridamento e limpeza das feridas cirúrgicas com soluções antimicrobianas, por antibioticoterapia e por sequestrectomia.8 Técnicas alternativas de tratamento para a ORN têm sido estudadas, uma vez que seu tratamento é extremamente difícil e de resultados imprevisíveis.O presente estudo tem como objetivo realizar uma revisão integrativa da literatura com o intuito de descrever sobre as terapias existentes para o manejo da ORN e sobre sua prevenção. Here is described a case of ameloblastic fibrosarcoma (AFS) affecting the posterior mandible of a woman who was treated surgically and recovered without signs of recurrence or metastasis after 12 years of follow-up. Tumor sections were immunostained for cell cycle, epithelial and mesenchymal markers. Immunohistochemical analysis evidenced high Ki-67 positivity in stromal cells (mean of 20.9 cells/High power field). Epithelial cells displayed strong positivity for p53, p63 and cytokeratin 19. In addition to the case report, a systematic review of current knowledge is presented on the AFS's clinical-demographic features and prognostic factors. Based on the review, 88/99 cases were diagnosed as AFS, 9/99 as ameloblastic fibro-odontosarcoma and 2/99 as ameloblastic fibrodentinosarcoma. All these lesions displayed very similar clinical-demographic and prognostic features. Moreover, the review provided evidence that first treatment, regional metastasis, distant metastasis and local recurrence were significant prognostic values for malignant odontogenic mesenchymal lesions. Based on the findings, segregation among ameloblastic fibrosarcoma, ameloblastic fibrodentinosarcoma and ameloblastic fibro-odontosarcoma seems illogical, considering all these lesions have similar predilections and outcomes.
