Ziel der Studie Untersuchung der Lärmexposition der Notärzte an einem Luftrettungszentrum und Ableitung von Arbeitsschutzmaßnahmen.
Methodik Dosimetrische Messung der Lärmexposition durch diensthabende Notärzte während 24-h-Schichten auf RTH und NEF über 7 Wochen.
Ergebnisse Bei den Messungen ergaben sich Überschreitungen der in Verordnungen definierten Auslösewerte der Lärmexposition.
Schlussfolgerung Medizinische Crews sind in der Luftrettung potenziell gesundheitsschädlichem Lärm ausgesetzt. Arbeitsschutzmaßnahmen müssen ergriffen werden.

Intoxikationen mit Schn?ffelstoffen sind im Notarztdienst selten. Butan kommt dabei aufgrund seiner leichten Verf?gbarkeit gerade bei Jugendlichen eine besondere Bedeutung zu. Bei missbr?uchlichem Einsatz wurden wiederholt akzidentelle Todesf?lle infolge maligner Arrhythmien beschrieben (?Sudden Sniffing Death Syndrom?), suizidaler Einsatz von Butan ist bislang eine selten beschriebene Rarit?t. Bei Vorliegen einer Butanintoxikation ist der Notarzt mit pathophysiologischen Besonderheiten konfrontiert, die sich durch die physikochemischen Eigenschaften der Substanz erkl?ren lassen. Die Behandlung eines solchen Patienten muss diesen Ver?nderungen Rechnung tragen. Die folgende Kasuistik schildert die Symptomatik und pr?klinische Therapie einer Intoxikation mit Butangas in suizidaler Absicht.
In the emergency medicine field cases of intoxication by sniffing agents do not occur very often. Nevertheless, considering the easy availability of butane the option of abuse especially by adolescent persons cannot be ignored. Although many cases of accidental death caused by malignant arrhythmia are described ("Sudden sniffing death syndrome"), suicide attempts using butane are a rarity. In this case the emergency treatment has to allow for special pathophysiological changes explained by physicochemical characteristics of butane. The following case report describes the symptomatology and pre-hospital treatment of an intoxication by butane with a suicidal intention.
