Dentes imaturos necrosados apresentam dificuldade ao tratamento endodôntico por suas paredes finas, canais largos, ápice aberto e divergente. Além de dificuldades do preparo químico mecânico e obturação, com prognóstico duvidoso pelo risco de fraturas. O objetivo desse estudo é relatar sobre o tratamento de paciente adulta com dente 21 imaturo necrosado e abcedado. Esse é um estudo observacional descritivo sobre os procedimentos realizados de dezembro de 2021 a maio de 2022. A queixa principal foi o escurecimento da coroa, abcesso e leve sensibilidade. A coleta de dados foi realizada por radiografias, tomografias e fotografias intraorais. Imagens radiográficas foram compatíveis com granuloma apical e rizogênese incompleta. O diagnóstico periodontite periapical crônica. Indicação de necropulpectomia, apicificação com plug apical(Bio C Repair- Angelus), obturação com guta percha e BioC Sealer(Angelus). A sintomatologia retornou após três mêses da obturação. Indicada abordagem cirúrgica e confirmada presença de trincas oblíquas radiculares. Foi realizada curetagem cística periapical, apicectomia, retroobturação com dissilicato de cálcio CIMMO, enxerto ósseo/membrana com Lumina Bone(Criteria). Dentes imaturos com histórico de trauma exigem estratégias avançadas de tratamento. Plug apical com biocerâmicos possibilita a vedação e obturação segura do ápice. A ressecção apical, curetagem e enxerto ósseo possibilitam a cicatrização dos tecidos e manutenção da função dentária. A dificuldade na identificação de trincas radiculares por exames radiográficos/tomográficos torna o momento cirúrgico ímpar. Consideramos as estratégias empregadas as mais coerentes com o desejo do paciente e com as condições tecnológicas existentes.
