Introduction and hypothesis
Urinary incontinence (UI) in women is a frequent and invisible clinical situation that affects several aspects in the lives of patients. The aim of this study is to assess the impact of urinary incontinence on different dimensions of quality of life.
Methods
Cross-sectional observational study. The sample brings together 381 women living in the Metropolitan Region of Chile, who answered a survey between June 2020 and June 2021, with questions about the sociodemographic situation, obstetric history and the EQ5D-3L questionnaire Chilean version and urinary incontinence variables. For the analysis of association between variables were used a correlation coefficient and logistic regressions.
Results
The mean age of the sample was 39.9 (SD: 12.6) years, with 68.5% of multiparous women and 42.5% had a history of vaginal delivery. Fifty percent of the respondents belong to a medium-high socioeconomic level according to the health insurance proxy. The analysis adjusted for age, health insurance and obstetrics variables showed that women with a frequency of moderate/severe urine loss had a 72% greater probability of suffering from moderate/severe pain/discomfort compared to women with nothing/little urine loss

In turn, the self-perception of affectation of urine loss was significantly associated with problems in the dimensions of mobility, anxiety/depression and health status.
Conclusions
This research contributes with evidence that makes visible the affectation on physical and psychological dimensions in women with urinary incontinence, allowing decision-makers to prioritize resources and design health programs that include clinical management.
La salud percibida y el dolor son predictores de la morbilidad, la mortalidad y la utilización de servicios de salud. Existe evidencia de desigualdades socioeconómicas en estas medidas, pero en Chile no se ha abordado su percepción en población trabajadora. En este trabajo se analizan las desigualdades según nivel educacional (NEDU) en la salud percibida y el dolor en población trabajadora chilena, considerando variables demográficas y de condiciones de trabajo. Los datos provienen de la Encuesta Nacional de Condiciones de Empleo, Trabajo, Calidad de Vida y Salud (ENETS), realizada a una muestra probabilística de 9.507 trabajadores de 15 o más años. Como medida estructural de posición socioeconómica (PSE) se usó el NEDU. Se analizó la prevalencia de percepción menos que buena de la salud, de la condición física y del bienestar emocional, según género y edad. A continuación se construyó una variable de dolor en algún lugar del cuerpo y otra de exposición a riesgos físicos que impone el trabajo. Se estudió la asociación y magnitud de las variables mediante modelos de regresión logísticos. El riesgo de tener una percepción de salud menos que buena es dos veces mayor en los trabajadores que no han completado la educación básica frente a quienes tienen estudios universitarios, ajustando por género, edad y grado en que disfruta del trabajo. Predomina el dolor de espalda y zona lumbar; y el NEDU es un factor explicativo de la prevalencia de dolor (OR de 1,36 IC 95%: 1,11; 1,66 con independencia de otros factores. Como conclusión se confirma que en los trabajadores existen desigualdades socioeconómicas considerables en dolor corporal y en percepción de salud, no así en cuanto a bienestar emocional.
