RESUMENEl síndrome de Turner tiene una prevalencia de 1 en 1800 a 5000 recién nacidos vivos femeninos y se caracteriza por la ausencia total o parcial del segundo cromosoma X. Actualmente se reconoce gran variedad en la presentación citogenética, siendo la más común la monosomia del X (45,X) y entre las menos frecuentes los mosaicismos que incluyen fragmentos o la totalidad del cromosoma Y. La presencia de este cromosoma confiere características fenotípicas de androgenización y obliga a la realización de estudios de extensión en las pacientes. Se reporta un caso de síndrome de Turner con mosaicismo 45,X/46,XY, se revisa la literatura y se propone un protocolo de estudio complementario y manejo de las pacientes con este síndrome.PALABRAS CLAVE: Síndrome de Turner, variación citogenética, fenotipo, mosaicismo SUMMARY Turner syndrome has an estimated prevalence of 1 in 1800 to 5000 female births and is characterized by total or partial absence of the second chromosome X. It's now recognized that there is variation in the cytogenetic presentation of the syndrome, being the most common monosomy X (45, X) and among the least frequent it's found mosaicism including fragments or the entire Y chromosome. The presence of Y chromosome confers phenotypic characteristics of androgenization and requires extension studies in patients. We report a case of Turner syndrome with mosaic 45,X/46,XY karyotype found by G-banding and aditionally, we propose a protocol for further studies and management of patients with this syndrome.Key words: Turner's syndrome, cytogenetic variation, phenotype, mosaicism INTRODUCCIÓNEl síndrome de Turner (ST) es una condición genética caracterizada por la ausencia total o parcial del segundo cromosoma X. Su prevalencia oscila entre 1 en 1800 a 5000 recién nacidos vivos femeninos, atribuyéndose las diferencias entre prevalencias estimadas, a la población estudiada y el momento del análisis cromosómico, sin embargo, se estima que representa hasta el 3% de todas las concepciones (1).La forma mas frecuente de presentación citogenética es 45X, sin embargo, alrededor del 50% de los pacientes presentan una formula cromosómica diferente. Marfan syndrome (SM) is a systemic disorder caused by mutations in the extracellular matrix protein fibrillin 1 (FBN1). With a dominant autosomal pattern, SM patients are characterized by ocular, cardiovascular and skeletal involvement, all within a variable clinical spectrum. It has been suggested that the intrafamilial and interfamilial phenotypic variability, characteristic of the syndrome, occurs by the association of other mutations called driver mutations. Even though there is a clear genetic causation, the recently described driver mutations are not yet fully elucidated. We present a SM case with a mutation not previously described in the fibrilin 1 gene, applying the revised Ghent nosology and analyzing the role of this new mutation and of the driver mutations in the genesis of the disease.
Introduction The study aim was to determine the frequency of prenatal ultrasound diagnosis of congenital anomalies in Newborns (NB) with birth defects hospitalized in two Neonatal Intensive Care Units (NICU) of Cali (Colombia) and to identify socio-demographic factors associated with lack of such diagnosis. Patients and methods It was an observational cross-sectional study. NB with congenital defects diagnosable by prenatal ultrasound (CDDPU), who were hospitalized in two neonatal intensive care units (NICU), were included in this study. A format of data collection for mothers, about prenatal ultra-sonographies, socio-demographic data and information on prenatal and definitive diagnosis of their conditions was applied. Multiple logistic and Cox regressions analyses were done. Results 173 NB were included, 42.8% of cases had no prenatal diagnosis of CDDPU; among them, 59.5% had no prenatal ultrasound (PNUS). Lack of PNUS was associated with maternal age, 25 to 34 years (Odds Ratio [OR]: 4.41) and 35 to 47 years (OR: 5.24), with low levels of maternal education (OR: 8.70) and with only a PNUS compared to having two or more PNUS (OR: 4.00). Mothers without health insurance tend to be delayed twice the time to access the first PNUS in comparison to mothers with payment health insurance (Hazard Ratio [HR]: 0.51). Among mothers who had PNUS, screening sensitivity of CDDPU after the 19thgestational week was 79.2%. Conclusions The frequency of prenatal diagnosis is low and is explained by lack of PNUS, or by lack of diagnostic in the PNUS. An association between lack of PNUS and late age pregnancy and low level of maternal education was found. In addition, uninsured mothers tend to delay twice in accessing to the first PNUS in comparison to mothers with health insurance. It is necessary to establish national policies which ensure access to appropriate, timely and good quality prenatal care for all pregnant women in Colombia.
Introducción: con base en el impacto que genera el hallazgo de un feto o recién nacido con alguna alteración cromosómica, desde el punto de vista familiar, social y para las entidades de salud, se analizan los resultados de los estudios genéticos realizados por el Servicio de Perinatología de la Clínica Rafael Uribe Uribe del Instituto de los Seguros Sociales en la ciudad de Cali, durante los últimos seis años.Objetivo: describir la frecuencia de las alteraciones cromosómicas encontradas en la Clínica Rafael Uribe Uribe de Cali durante los estudios genéticos realizados por el Servicio de Perinatología de este centro desde enero 1 de 1997 hasta diciembre 31 de 2002.Materiales y métodos: se realizó un estudio retrospectivo, en el cual se revisaron las historias clínicas de todos los procedimientos genéticos practicados a las gestantes que aceptaron amniocentesis o cordocentesis según la indicación.Resultados: se encontraron 615 procedimientos de diagnóstico prenatal: 604 amniocentesis (98,21%) y 11 cordocentesis (1,79%). Se evidenciaron 19 estudios anormales (3,09%); de ellos, 17 correspondieron a aneuploidias (89,47%): seis trisomías 21 (31,57%), 4 trisomías 18 (21,05%), dos trisomías 13 (10,52%), dos síndromes de Klinefelter (10,52%) y tres monosomías 45XO (15,78%); se presentaron dos alteraciones estructurales (10,53), de las cuales una correspondió a una translocación reciproca (46XX, t (6,15) der 15) (5,26%) y una delección (46XX, 9 q (-)) (5,26%).Las indicaciones más frecuentes del estudio genético fueron:Edad materna (>35 años).Hallazgos ecográficos.Deseo de la pareja.Conclusiones: los resultados obtenidos no distan de los publicados en la literatura mundial; la alteración cromosómica más frecuente fue la trisomía 21. Puesto que para diagnosticar una alteración cromosómica se requirieron 32 estudios de diagnóstico prenatal, debemos afinar (con la ayuda de las directivas de la institución) en la determinación de marcadores ecográficos y bioquímicos con el propósito de someter al menor número de pacientes al riesgo que implican los procedimientos invasivos.
RESUMENEl síndrome de duplicación 7q11.23 es una patología causada por la duplicación de una región del cromosoma 7 que comprende 26 genes. El primer caso descrito en la literatura fue reportado por Somerville et al., en el año 2005, quienes describieron un paciente con dolicocefalia, frente alta y estrecha, pestañas largas, nariz alta y ancha, filtrum corto, paladar ojival, maloclusión dental, retrognatia y retardo grave en el lenguaje. Presentamos una paciente colombiana con hallazgo de duplicación 7q11.23 mediante técnicas de hibridación genómica comparativa y hallazgos clínicos clásicos. Este es el primer caso comunicado en Colombia y en América Latina. ABSTRACT 7q11.23 duplication syndrome is a disease caused by duplication of a region of chromosome 7 comprising 26 genes. The first case described in the literature was reported by Somerville et al. in 2005, who described a patient with dolichocephaly, high and narrow forehead, long eyelashes, high and wide nose, short philtrum, high arched palate, dental malocclusion, retrognathia, and severe language delay. We report the case of a Colombian patient with 7q11.23 duplication by comparative genomic hybridization techniques, and classical clinical findings, this being the first reported case in Colombia and Latin America.
