OBJECTIVES:The aims of this study were to evaluate the correlation between oropharyngeal examination and objective palatine tonsil volume in snoring adults and verify the influence of the oropharyngeal anatomy, body mass index, age, and severity of obstructive sleep apnea on actual tonsil volume. In addition, we aimed to assess the influence of tonsil size on obstructive sleep apnea in adults.INTRODUCTION:Pharyngeal wall geometry is often altered in adults who have obstructive sleep apnea, and this might influence the findings of the oropharyngeal examination that, in turn, are the key factors when considering surgical management for this condition. Furthermore, the correlation between the actual tonsil volume and the severity of obstructive sleep apnea in adults is currently unknown.METHODS:We prospectively studied 130 patients with obstructive sleep apnea or primary snoring who underwent pharyngeal surgery with intraoperative measurement of tonsil volume. We compared tonsil volume with preoperative polysomnography, oropharyngeal examination, and anthropometric data.RESULTS:We found a significant correlation between actual tonsil volume and subjective tonsil grade. We also found a significant correlation between tonsil volume and the apnea-hypopnea index. Using a multivariate linear regression model, tonsil volume was found to be significantly correlated with age, body mass index, and oropharyngeal examination, but not with polysomnography. Clinically, only the rare tonsil grade IV was indicative of more severe obstructive sleep apnea.CONCLUSIONS:There is a strong correlation between clinical tonsil grade and objective tonsil volume in snoring adults, and this correlation exists regardless of the presence or severity of obstructive sleep apnea. Pharyngeal tissue volume likely reflects the body mass index rather than obstructive sleep apnea severity.
In this small case series, lateral pharyngoplasty reduced the values obtained in the 24-hour ABPM due to a significant reduction of blood pressures during sleep in patients with OSA 6 months after surgery. Although the patients presented with reductions in AHI, arousals, and desaturation time, this was not correlated with the improvement in arterial blood pressure.
Objective:To analyze the effectiveness of myofunctional therapy (MT) in the treatment of habitual snoring in obese patients. Material and Methods: This randomized clinical trial consisted of an experimental group (n=14) that underwent MT and a control group (n=26) that performed nonspecific exercises for the treatment of snoring. The Epworth sleepiness scale (ESS), Pittsburgh sleep quality index (PSQI), and short-form health survey (SF-36) were applied before and after treatment. Snoring was assessed subjectively by asking the partner about improvement after treatment. The SnoreLab app was used for objective assessment. Results: There was no significant effect of MT on any of the SnoreLab variables analyzed when groups, time points or covariates (adherence, age, body mass index [BMI], neck circumference, and sex) were compared. Neck circumference (cm) and the Pittsburgh sleep quality index score were significantly higher after treatment. There was no change in the Epworth sleepiness scale score after treatment. A correlation was found between BMI and the Pittsburgh sleep quality index and between BMI and the functional capacity component of the SF-36. Patient adherence was similar between groups. Discussion: Apps for recording snoring are a useful tool to be explored. MT exerted no significant effect on habitual snoring in obese patients despite the reduction of the snore score in the experimental group. Therapy applied without exclusion criteria based on the severity of sleep breathing disorders and pharyngeal characteristics fails to achieve the results necessary to treat habitual snoring in obese patients.
A apneia obstrutiva do sono (AOS) é uma doença global que se associa à síndrome metabólica, especialmente à obesidade. A hipoxemia intermitente decorrente da AOS pode levar a um aumento na resistência à insulina e a uma alteração no metabolismo lipídico, podendo precipitar o desenvolvimento de esteatose hepática, caracterizada pelo acúmulo de lipídeos no interior dos hepatócitos, considerada provável causa principal de morbidade e mortalidade ligada a doenças do fígado. Dessa forma, este estudo retrospectivo buscou avaliar uma possível relação entre a AOS e a esteatose hepática em pacientes obesos no pré-operatório para cirurgia bariátrica. A partir da coleta de dados de 1.416 prontuários de pacientes em avaliação pré-operatória, obteve-se dados dos exames de ultrassonografia (USG) e polissonografia (PSG), resultando em uma amostra de 87 indivíduos avaliados. Desta amostra, 61% dos indivíduos eram do sexo feminino, com idade média de 39 anos de idade, com desvio padrão (d.p.) de 11,4 e IMC de 39.3 (d.p. 4,4). A ultrassonografia classificou como portadores de esteatose hepática 74 indivíduos (85%). O valor médio do Índice de apneia-hipopneia (IAH) foi de 33.5 (d.p. 28.9) e do Índice de dessaturação da oxihemoglobina (IDO) foi de 41.4 (d.p. 34,1). Os pacientes foram divididos em 2 grupos: um grupo com IAH menor ou igual 15 eventos/hora e outro com IAH maior ou igual 16 eventos/hora. O grau de esteatose diferiu entre os grupos pelo IAH (p <0,001) e sexo (p <0,001). Após o ajuste de várias variáveis por meio de regressão múltipla, apenas o sexo masculino foi um preditor de graus mais elevados de esteatose. Concluímos que há maior prevalência de esteatose hepática entre portadores de AOS, sendo que os principais fatores associados a esteatose foram o IAH e sexo masculino, porém após regressão apenas o sexo feminino seria preditor de proteção à esteatose na amostra estudada.
Introduction: Tuberculosis (TB) is a global infectious disease that has plagued human beings for centuries. It currently ranks as one of the primary causes of death by a single agent. Treatment of latent TB (LTBI) is one of the cornerstones used to prevent the activation of TB. The current regimens are lengthy, associated with poor treatment adherence and several side effects. The need for shorter regimens is essential to control TB infection and accomplish the "End TB Strategy," which has been proclaimed by the World Health Organization (WHO). Our study aims to provide a study design that assesses the efficacy of one month of Isoniazid (IHN) and Rifapentine (RFT) in non-immunosuppressed LTBI patients. Methods: The proposed study will be a phase III, single-center, randomized, two-arms (1:1 ratio), double-blinded, placebo-controlled non-inferiority trial. We will include immunocompetent patients above the age of 18 with a new diagnosis of LTBI. Participants will be randomized, either receiving one month of INH with RFT or 6 months of INH alone with respective placebo to maintain blinding. The primary study outcome is the cumulative incidence of active TB in the studied treatment arms. The secondary outcome is the cumulative incidence and severity of adverse effects of the studied regimens. Discussion: Given the scope and spread of LTBI around the globe, shorter and safer treatment options are of the utmost importance to eradicate TB. The proposed trial can contribute to this objective by improving the current standard of care. Potential disadvantages of the trial design include the need for many participants, trial duration and costs.
Introdução: A paralisia do sono é um transtorno que ocorre logo após acordar ou no momento em que se está tentando adormecer e que impede o corpo de mexer, mesmo quando a mente está acordada. Assim, a pessoa acorda, mas não consegue se movimentar, causando angústia, medo e terror. Fundamentação teórica: A paralisia do sono acontece quando a atonia de movimento rápido dos olhos (REM) é mantida na vigília. Os episódios ocorrem durante o limiar entre o sono e estado de vigília física, estar acordado, a pessoa sente medo de dormir, percebe o cérebro acordar, mas o corpo não, consegue abrir os olhos, ouvir tudo, mas não se mexer, tem a sensação de estar acompanhado, sendo observado e de estar paralisado. Os episódios de paralisia do sono no geral são autolimitados, não deixam sequelas e são rápidos, tem duração de segundos a minutos. Metodologia: Será analisado a partir de formulário online a incidência desses episódios nos estudantes do curso de medicina do Centro Universitário Assis Gurgacz (FAG) e também a relação do aparecimento dessa condição com a qualidade do sono. Visto que distúrbios da qualidade do sono foram encontrados em vários estudos como estando associados a maiores chances de ocorrência de paralisia e acredita-se que se trata de um evento multifatorial. Discussão e análise: foi possível verificar com esse estudo que existe uma significativa porcentagem de acadêmicos de medicina da instituição que já vivenciaram episódios de paralisia do sono e conseguimos também analisar as diferentes formas de manifestações, além disso, as condições de sono dessas pessoas tem impacto direto sobre a qualidade de vida e também no aparecimento desses episódios. Conclusão: Ainda são necessários muitos estudos que possam identificar as causas e as formas de evitar essa condição, que pode muitas vezes causar medo e pânico.
