A asma constitui-se como uma enfermidade crônica, causada pelo contato com um alérgeno indutor de uma resposta inflamatória, que acomete parte do trato respiratório, em um indivíduo predisposto geneticamente. Apesar de seus conceitos serem bem conhecidos pela maioria da comunidade científica atual, a divulgação de novos estudos ocorre de maneira constante, o que contribui para a atualização da temática, ao passo em que ocorre o acréscimo de novas informações. Portanto, o objetivo deste trabalho foi revisar na literatura acerca das principais características da asma, destacando seus aspectos fisiopatológicos. Para isso, foi realizada uma revisão integrativa com uso de dados secundários, obtidos por meio das bases bibliográficas digitais PubMed, STM (Science Translational Medicine) e SciELO (Scientific Electronic Library Online), durante o período de setembro a outubro de 2022. Para a exposição dos resultados obtidos, foi construído o Quadro 1, que expõe, de forma resumida, os principais trabalhos usados para este estudo. Da mesma forma, o objetivo deste trabalho foi melhor aprofundado no tópico de “Discussão”, organizado com base nas fontes selecionadas. Dessa forma, as informações apresentadas por esta revisão podem ser levadas em consideração por futuros pesquisadores que desejem se atualizar acerca da temática da asma, com enfoque em seus aspectos fisiopatológicos.
A trombose venosa cerebral (TVC) constitui-se como uma enfermidade cerebrovascular que, apesar de relativamente rara, é uma importante causa de morbimortalidade quando presente. Embora esteja relacionada a uma série de fatores de risco com alta prevalência dentro da população mundial, tais como hipertensão, obesidade, diabetes mellitus e hipertrigliceridemia, a TVC possui cura, especialmente quando o diagnóstico é estabelecido precocemente. Portanto, o objetivo deste estudo foi estabelecer uma descrição clínico-topográfica do quadro dos pacientes acometidos por tal patologia, em um hospital de referência em Neurologia em Salvador, Bahia. Para isso, foi elaborada a construção de um modelo de projeto de pesquisa, de forma a esclarecer etapas para replicação pela comunidade científica, adaptado conforme a localidade dos pesquisadores. Dentre os resultados apresentados, configura-se a proposta de um método observacional, retrospectivo e descritivo, com a finalidade de instituir um estudo clínico-topográfico acerca do quadro sintomatológico dos pacientes com TVC em um hospital de Salvador, Bahia. Conclui-se que, a TVC é uma relevante causa da elevação das taxas de morbimortalidade em meio atual, embora apresente baixa incidência. Consequentemente, este estudo possui importância significativa, ao passo em que estabelece um projeto de pesquisa referente à análise descritiva dos quadros de pacientes acometidos por TVC, de forma a ser replicado pela comunidade científica.
This article presents a comparative analysis between the human rights described in the Universal Declaration of Human Rights and Organic Laws 8,080/1990 and 8,142/1990, which regulate the Brazilian Unified Health System. The three documents were related and compared, the first, an international one and the other two of national character, to identify humanistic and ethical values in the legal framework of Brazilian public health. Discussions on this topic are fundamental so health professionals can identify the gaps between theory and hospital practice, thus ensuring the rights of patients.
Introduction: Obstructive sleep apnea (OSA) is highly prevalent after stroke and associated with stroke recurrence. Several clinical scales have been used to predict OSA, but their relevance for functional outcomes are unknown. Objective: We aimed to determine if a sleep apnea risk score is associated with functional outcome in stroke patients. Methods: Consecutive patients with imaging-confirmed ischemic stroke were recruited from a public hospital in Salvador, Brazil. Demographic and cerebrovascular risk factor data were collected prospectively. We used NIH stroke scale (NIHSS) to quantify stroke severity and STOP-BANG questionnaire to quantify risk of OSA on admission (0 to 8, higher scores indicating higher risk). Patients were followed for 90 days and functional outcome was assessed by modified Rankin Scale (mRS). Poor outcome was defined as a mRS = 3 to 6. Results: We recruited 382 patients with ischemic stroke between October, 2018 and November, 2019, mean age 64 +/- 13 years, 189 (49.5%) female, median NIHSS 8 (interquartile range - IQR - 6 to 14) and median STOP-BANG score 5 (IQR 4 to 5). After 90 days, 216 (56.5%) had poor outcomes. Median STOP-BANG score was higher in patients with poor, when compared to good outcomes (4 vs 3, respectively, p=0.001). In multivariable logistic regression, STOP-BANG score was independently associated with poor outcome (odds ratio 1.56, 95% confidence interval 1.02 to 2.63, p=0.003), after adjustment for age, sex, NIHSS, cerebrovascular risk factors and stroke etiology. When STOP-BANG score was stratified by OSA risk, patients with mild, moderate and high OSA risk reached poor outcomes in 38.2%, 58.7% and 62.1%, respectively (p=0.004). Conclusions: A simple sleep apnea risk score (STOP-BANG) is independently associated with poor functional outcome in ischemic stroke patients.
