Cuando se produce una ruptura a través de todas las capas del intestino delgado se está en presencia de una perforación intestinal. La perforación es una lesión de espesor total de la pared del órgano. Dado que el peritoneo cubre completamente el estómago, la perforación de la pared crea una comunicación entre la luz gástrica y la cavidad peritoneal. El contenido intestinal puede liberarse hacia los órganos vecinos como el páncreas en el caso de la perforación de una úlcera específicamente a nivel del duodeno, lo cual puede producir una penetración al páncreas. Si la perforación ocurre de forma aguda, no hay tiempo para que una reacción inflamatoria bloquee la perforación y el contenido gástrico puede ingresar libremente a la cavidad peritoneal general, causando una peritonitis química. Las perforaciones que ocurren durante un período prolongado pueden ser contenidas localmente por la reacción inflamatoria. La perforación puede sospecharse en base a la presentación clínica del paciente, o el diagnóstico se vuelve obvio a través de un informe de aire libre extra luminal en las imágenes de diagnóstico realizadas para evaluar el dolor abdominal u otro síntoma. El tratamiento para este tipo de complicaciones es una reparación quirúrgica.
