Metabolic changes have been correlated with adverse pregnancy outcomes. The aim of the present study is to determine the TyG and TG/HDL-c indices in a cohort of healthy pregnant (n = 142), preeclamptic (n = 18), and healthy nonpregnant women (n = 56). Preeclamptic women were selected from the same cohort. Pregnant women were followed during three periods of pregnancy and postpartum. The results showed a significant increase in the values of TyG and TG/HDL-c (p < 0.01) as pregnancy progresses, without significant differences between healthy and preeclamptic women. TyG and TG/HDL-c indices are significantly low in nonpregnant and three months' postpartum women when compared with each gestational period studied. TyG and TG/HDL-c indices are positively correlated with HOMA-IR in the early and middle pregnancy (p < 0.05). Multiple linear regression using the TyG and TG/HDL-c indices as dependent variables showed that TyG index was significantly associated with HOMA-IR, gestational age, HDL-c, TC, LDL, fasting insulin, and mean BP (p < 0.001); meanwhile, TG/HDL-c index was only associated with HOMA-IR (p < 0.0242) and gestational age (p < 0.001). In conclusion, the TyG and TG/HDL-c indices could be useful in monitoring insulin resistance during pregnancy.
AntecedentesA partir del 2014 en Colombia se incorporó la Tamización primaria con prueba de Virus del Papiloma Humano (VPH) desde los 30 hasta los 65 años, cuando la prueba es positiva se hace triage con citología para remisión a colposcopia. Actualmente se discute la conveniencia de iniciar la tamización con prueba de ADN de VPH a partir de los 25 años. De esta manera, el objetivo de esta evaluación de tecnologías sanitarias es analizar la evidencia disponible en torno a la seguridad, efectividad, costoefectividad, valores y preferencias, dilemas éticos y aspectos relacionados con la implementación para el contexto colombiano de la prueba ADN-VPH como estrategia de tamización cervical en mujeres menores de 30 años. Métodos Evaluación de efectividad y seguridad clínicasSe realizó una búsqueda sistemática de la literatura en MEDLINE, Embase y CENTRAL de revisiones sistemáticas y ensayos clínicos. Se calificó el cuerpo de la evidencia con la aproximación GRADE. Posteriormente, se convocó a un grupo interdisciplinario a una mesa de trabajo en donde se presentó la evidencia recuperada, dando paso a la discusión y a la construcción de las conclusiones, siguiendo los lineamientos de un consenso formal acorde a la metodología RAND/UCLA. Estudio económicoSe hizo una búsqueda sistemática de la literatura de estudios que hubieran evaluado el costo-efectividad para Colombia. ResultadosDe 7.659 referencias recuperadas se incluyeron 8 estudios. Resultados clínicosSe realizó un análisis integrativo de 5 ensayos clínicos aleatorizados que cumplieron con los citerios de inclusión. Cuando se compara frente a la citología, la tamización primaria con ADN-VPH en mujeres menores de 30 años, podría asociarse con una mayor frecuencia de detección de lesiones NIC2+ durante la primera ronda de cribado (RR: 1.57; IC: 1,20 a 2,04; certeza en la evidencia baja), con una menor incidencia de NIC2+ (RR:0,67; IC: 0,48 a 0,92; certeza en la evidencia baja) y se asocia con una menor frecuencia de carcinoma invasor al término del seguimiento (RR: 0,19; IC: 0,07 a 0,53; certeza en la evidencia alta). Resultados económicosDesde el punto de vista económico, la alternativa de ADN-VPH y triage con citología desde los 25 años quizás representa la alternativa más costo-efectiva para Colombia (razón costo-efectividad incremental $8.820.980 COP año 2013). Otras implicacionesDos estudios sugieren que las barreras de implementación, atribuibles a circunstancias de intermediación, de orden público y de carácter geográfico, podrían ser solventadas por nuevas tecnologías o estrategias de cribado. Es importante considerar alternativas de forma de administración y de prestación de servicios para solventar algunas barreras de aceptabilidad y acceso. Todo programa de tamización cervical debe contemplar los principios éticos de no maleficencia, beneficencia, autonomía y equidad. Futuros estudios deben enfocarse en analizar nuevas tecnologías de cribado con énfasis en población menor de 30 años. ConclusionesEl uso de la prueba ADN-VPH como estrategia de tamización en mujeres menores de 30 años es una intervención probablemente efectiva y costoefectiva para Colombia. En la Antigua Grecia se creía que el cuerpo humano estaba constituido por cuatro tipos de humores y que el desbalance entre ellos llevaba a la enfermedad. La mujer era considerada más vulnerable de presentar dolencias, por tener una súper producción de sus fluidos y presentar un “útero errante” que podría irrumpir en sus diferentes órganos y llevarla aún a convulsionar durante el embarazo (1). Hipócrates el padre de la Medicina, pensaba que las convulsiones durante el embarazo se producían por congestión o por depleción sanguínea del cerebro (2).Durante la Edad Media dominaron las creencias religiosas. Se utilizaron amuletos, pociones mágicas y oraciones de sanidad (1). Para tratar de disminuir la congestión cerebral y prevenir la eclampsia, Mauriceau recomendó realizar de dos a tres flebotomías durante el embarazo y magnificó en sus escritos la naturaleza crítica de la enfermedad (3).Al final del siglo 19 y comienzo del 20, los médicos alemanes y de los países bajos realizaban un manejo agresivo de la eclampsia, desembarazando de manera inmediata por vía vaginal o abdominal. Esta conducta obstétrica agresiva incrementó de manera notable la mortalidad materna. Como resultado empezó a ganar popularidad el manejo conservador de la eclampsia (4). Los pioneros de este método fueron Tweedy de Dublin y Stroganoff de Rusia (1). El primero consideraba que el trabajo de parto incrementaba la ocurrencia de convulsiones por mecanismos reflejos. El médico debía abstenerse de realizar exámenes vaginales, palpación abdominal, dilatación del cérvix. La paciente debía sedarse con altas dosis de morfina. Si se entraba en trabajo de parto la aplicación del fórceps era aceptada (5).

Agradecemos los comenatrios realizados en la carta al editor enviada por los doctores Campos-Ponce, Castro-Mamani y Ocaña-Fernández, sin embargo, quisiéramos hacer algunas consideraciones a lo allí expresado.El cáncer de cuello uterino (CCU) ocupa el cuarto lugar en incidencia y mortalidad por cáncer en mujeres menores de 30 años en el mundo. Para el año 2020, según la Agencia Internacional de Investigación sobre el Cáncer (IARC, por sus siglas en inglés), se reportaron 22.884 casos nuevos y 5.297 muertes en este grupo etario (tasas de incidencia ajustadas, TIa) de 1,1 y 0,26 por 100.000 mujeres, respectivamente). Para el mismo año, las tasas de incidencia y mortalidad reportadas fueron mayores para Latinoamérica y el Caribe, (2,0 por 100.000 y 0,42 por 100.000 respectivamente) (1).Para la detección del CCU se dispone, actualmente, de estrategias basadas en la citología, la prueba de ADN del VPH o la inspección visual con acético (VIA). Objetivo: reportar el caso de una paciente con carcinosarcoma del cuello uterino con sangrado intraabdominal, y revisar la literatura disponible sobre el tratamiento y pronóstico de la entidad.Materiales y métodos: se reporta el caso de una paciente de 84 años, que consultó por masa abdominal y obstrucción del tracto urinario. Durante la estancia hospitalaria presentó sangrado intraabdominal con signos de choque, por lo que fue necesario realizar histerectomía abdominal total con salpingo-oforectomía bilateral, ligadura de hipogástricas y taponamiento de cavidad pélvica como intervenciones para controlar el sangrado. El estudio histológico reportó como diagnóstico carcinosarcoma del cérvix. La paciente evolucionó de forma adecuada y fue remitida para continuar manejo por oncología. Para la búsqueda de la literatura se realizó una pesquisa en las bases de datos Medline vía PubMed, SciELO y Ovid, utilizando los términos “uterinecarcinosarcoma” “treatment “cáncer treatment” “treatmentreview” y “treatmentoutcome”. La búsqueda se limitó por tipo de idioma, pero no por año de publicación.Resultados: 19 estudios cumplieron con los criterios de inclusión y de exclusión, estas fueron predominantemente reportes de caso. El estadio clínico reportado con mayor frecuencia fue FIGO IB en cerca del 53 % de los casos, y la presentación más frecuente fue el sangrado genital acompañado de masa pélvica. Con un seguimiento promedio de 15 meses, la sobrevida para este tumor es del 17 % para las pacientes que recibieron radioterapia. El 63 % de las pacientes que recibieron cirugía como tratamiento primario permanecieron libres de enfermedad durante los dos primeros años de seguimiento, con una frecuencia cercana al 100% para este mismo periodo cuando se administró radioterapia posterior a la cirugía.Conclusiones: el carcinosarcoma de cérvix es una entidad poco frecuente, cuya manifestación clínica más común suele ser la presencia de sangrado genital acompañado de masa pélvica. Dentro de las opciones terapéuticas disponibles para tratar esta entidad se encuentran la cirugía, la radio y la quimioterapia. No obstante, independientemente del tratamiento proporcionado, el pronóstico de sobrevida para las mujeres con esta patología es inferior al de las mujeres con carcinomas escamosos o adenocarcinomas. Se requieren estudios de alta calidad metodológica que evalúen la seguridad y la efectividad de las diferentes intervenciones para el abordaje terapéutico de esta entidad.
