PUESTA AL DÍA EN LAS TÉCNICAS IntroducciónEn la actualidad, se considera el acceso intraóseo (IO) una alternativa eficaz y rápida en los casos en los que el acceso intravenoso es difícil o imposible de conseguir en un tiempo preestablecido (shock grave o en la parada cardiorrespiratoria). El Grupo Español de Reanimación Cardiopulmonar Pediátrica y Neonatal (GERCPYN) recomienda el aprendizaje de las técnicas de colocación de los diferentes dispositivos de canalización intraósea que existen en el mercado en todos los cursos de reanimación cardiopulmonar (RCP), básica, instrumentalizada o avanzada. Existen varios dispositivos IO con distintas características y técnicas de colocación, pero todos tienen 2 cosas en común: Puntos claveEn las últimas normas del International Liaison Committee on Resuscitation de reanimación cardiopulmonar del 2010 se considera la vía intraósea como una vía alternativa al acceso venoso en las situaciones de parada cardiorrespiratoria y en casos de shock descompensado, cuando, después 60 s, no se ha conseguido canalizar un vía periférica, tanto en niños como adultos.Sólo debe utilizarse en situaciones de urgencia vital y por tiempo limitado.Las normas recomiendan que en recién nacidos y niños hasta 6 años se utilice preferentemente la tibia proximal. A partir de 6 años, el sitio de punción se sitúa 3 cm por encima del maléolo tibial interno.Lugares alternativos que puede usarse, cara lateral del fémur, 2-3 cm por encima del cóndilo lateral y la cara anterior de la cabeza humeral (adolescentes).Cualquier fármaco o solución que pueda ser administrada por vía intravenosa puede serlo por vía intraósea en la misma dosis y cantidad.permiten conseguir un acceso venoso en poco tiempo (en menos de 1,5 min en el caso de la RCP), y que deben ser sustituidos en cuanto sea posible por un acceso intravenoso. En estos momentos, los dispositivos de colocación manual son los más extendidos tanto en el medio hospitalario como en el extrahospitalario. La rareza de complicaciones (1%), hace que esta técnica sea imprescindible en todos aquellos lugares donde deban atenderse niños en situaciones graves y que requieran la consecución de una vía venosa de forma rápida (shock, traumatismo grave, quemaduras extensas, parada cardiorrespiratoria, etc.). Vía de acceso intraóseaEn situaciones de shock se produce una respuesta de compensación fisiológica consistente en una redistribución del flujo sanguíneo hacia órganos vitales, lo que da lugar a una vasoconstricción periférica y esplácnica 1 . Este hecho hace que las venas periféricas puedan ser inaccesibles a cualquier edad, pero más aún en lactantes y niños pequeños. Por eso, el acceso IO ha sido considerado desde hace tiempo una alternativa a la canalización venosa periférica en el soporte vital avanzado tanto pediátrico como del adulto 2-5 . Su utilización se basa en el hecho de que la cavidad medular de los huesos largos está ocupada por una rica red de capilares sinusoides que drenan a un gran seno venoso central que, a través de las venas nutricias y emisarias...
Introducción: Enmarcado dentro dentro de un proyecto de innovación educativa en el grado de enfermería, introducimos la aplicación de escenarios realista, generados a través de la herramienta EDISON para tratar temas como la RCP pediátrica y neonatal. Debido a la pandemia COVID-19 se propuso una adaptación del proyecto a la modalidad telemática.Metodología: Estudio analítico exploratorio donde se camparó la efectividad de una intervención formativa en un contexto de confinamiento. Se basó en el uso de materiales audiovisuales creados a través de EDISON que configuraron un VideoLibro que se puso a disposición del alumnado a través del Aula Virtual, evaluando su efectividad a través del examen final de la asignatura.Resultados: Entre los estudiantes de 2º curso (N=296), el 37,8% estuvieron expuestos a la intervención. Los alumnos de este primer grupo (con VideoLibro) obtuvieron en mayor porcentaje la calificación de Excelente y Notable (99,11%) que los alumnos del grupo control (98,65%). Se detectaron diferencias significativas entre los grupos (p <0,05) en la puntuación obtenida en preguntas específicas de RCP (31).
